Clinical and neurophysiologic characteristics of the pronator syndrome.

Nine patients were clinically diagnosed as having a pronator syndrome, i.e., high median nerve compression. The main symptom was pain at the proximal volar aspect of the forearm increasing for several hours after exercise. All patients showed local tenderness over the median nerve 4-5 cm distal to the elbow and pain on active forearm pronation against resistance. Two patients had been previously operated upon for carpal tunnel syndrome. Preoperative routine neurographic-electromyographic studies were normal. In the differential diagnosis, the exclusion of carpal tunnel syndrome and anterior in

Neurophysiological investigations in GM1 and GM2 gangliosidoses.

Neurophysiological studies (EEG, ERG, VEP and BAEP) have been carried out on a total of fifty-four patients (fourty-five GM2 and nine GM1 gangliosidosis) at various stages of the disease process. In infantile GM2 gangliosidosis, the EEG was midly abnormal from an early age but by the age of one year there was a rapid and progressive deterioration. EEG changes in late onset GM2 gangliosidosis were very variable and unrelated to age or enzyme defect. In both Type 1 and Type 2 GM1 gangliosidosis there was a progressive deterioration of the EEG. Paroxysmal features were not prominent in any of the

Neurophysiological evaluation of newborns with congenital hydrocephalus.

EEGs and visual and auditory evoked potentials (VEP and AEP) were studied in 27 newborns with congenital hydrocephalus. Asynchronous sleep patterns were the main EEG abnormality in the newborn as well as in the older infants and children. Asynchronous tracé alternant patterns, the most common finding, were more frequently encountered in infants with more severe hydrocephalus. Asynchrony of bifrontal humps was another finding observed in a few infants. Intermittent semirhythmic theta or delta waves were noted in some infants. Depression of the EEG background activity was an infrequent finding o

Vestibular neuronitis; a neurological and neurophysiological evaluation.

Neurological and neurophysiological findings were retrospectively reviewed in a group of 50 patients with vestibular neuronitis (VN). The onsets of VN were found to be clustered in the period from August to January. A preceding infection was reported by 36% of the patients. Neurological examinations did not reveal any other relevant signs than a spontaneous nystagmus during the acute phase. The results of routine laboratory tests and cerebrospinal fluid tests were within normal limits. EEG was recorded in 37 patients; 15 patients had a definitely abnormal EEG, with a slowing of the dominant oc

Neurophysiological study of normal pressure hydrocephalus.

A neuropsychological Mental Deterioration Battery (MDB) was used to identify deterioration profiles of 43 patients afflicted with normal pressure hydrocephalus (NPH) (n = 18) or other forms of dementia (n = 25). The NPH patients submitted to a shunt-intervention (n = 10) were also evaluated after surgery. A comparison of profiles, obtained from the experimental and control groups, shows that NPH patients seem to be more impaired in tests designed to detect frontal lobe involvement. Some implications of the relatively greater impairment of frontal functions in NPH dementia are discussed.

Neurophysiologic syndromes in stroke as predictors of outcome.

Outcomes in self-care following rehabilitation in 226 patients were correlated with 11 stroke syndromes, reflecting several pathophysiologic disturbances subsequent to either infarction or hemorrhage in cerebral or vertebro-basilar vessels. Self-care was scored on a 20-point scale for bed movements, transfers, feeding, dressing, personal hygiene, and bathing. Interjudge error among therapists did not exceed 2.5%. Mean score in left cerebral infarction without aphasia was used as a referent value. Scores in left cerebral infarction with aphasia and right parietal lobe syndrome with and without

Emergent technologies in clinical neurophysiology to study the neuromuscular system: IFCN handbook chapter.

The present chapter of the new third series IFCN Handbook focuses on how peripheral neurophysiology may develop in the next five years or more. While some are clinical relevant, others are more experimental. The first three sections detail the possible futures of EMG, motor unit number estimation, surface EMG and movement. In a section on magnetoneurography, electrically induced signals are visualized in sensory nerves whilst that on magnetomyography shows how motor units may be identified in greater detail than with surface EMG. Electrical impedance methods for assessing nerve and muscle are …

From guillotine to EEG: Revisiting the neurological controversies of decapitation through modern neurophysiology.

The question of whether consciousness persists after decapitation has fascinated scientists, philosophers, and the public for over two centuries, from 18th-century debates over the guillotine to modern neurophysiological studies of cerebral ischemia. This review synthesizes historical controversies-such as the anecdotal reports of facial movements in severed heads and early galvanism experiments-with modern neurophysiological evidence to reinterpret the mechanisms underlying the loss of consciousness. We trace the evolution of hypotheses from initial philosophical speculations to electroenceph

Sacral Tarlov cysts: Neurophysiology abnormalities and correlation with pelvic sensory and visceral symptoms.

BACKGROUND AND PURPOSE: Recent studies suggest a possible association between Tarlov cysts (TCs), usually considered as incidental radiological findings, and neurological symptoms such as pain, numbness and urogenital complaints. The aim was to explore the relationship between TCs and sacral nerve root functions using pelvic neurophysiology tests, and to correlate changes with clinical symptoms and magnetic resonance imaging (MRI) findings. METHODS: Consecutive patients with sacral TCs, referred for pelvic neurophysiology testing and presenting with at least one symptom related to the pelvic …

Transcranial magnetic stimulation maps the neurophysiology of chronic noncancer pain: A scoping review.

BACKGROUND: Chronic noncancer pain is a global public health challenge. It is imperative to identify biological markers ("biomarkers") to understand the mechanisms underlying chronic pain and to monitor pain over time and after interventions. Transcranial magnetic stimulation (TMS) is a promising method for this purpose. OBJECTIVES: To examine differences in TMS-based outcomes between persons with chronic pain and healthy controls (HCs) and/or before versus after pain-modulating interventions and relationships between pain measures and TMS outcomes; To summarize the neurophysiological mechani

Light Activated Escape Circuits: A Behavior and Neurophysiology Lab Module using Drosophila Optogenetics.

The neural networks that control escape from predators often show very clear relationships between defined sensory inputs and stereotyped motor outputs. This feature provides unique opportunities for researchers, but it also provides novel opportunities for neuroscience educators. Here we introduce new teaching modules using adult Drosophila that have been engineered to express csChrimson, a red-light sensitive channelrhodopsin, in specific sets of neurons and muscles mediating visually guided escape behaviors. This lab module consists of both behavior and electrophysiology experiments that ex

The effect of isocapnic hyperoxia on neurophysiology as measured with MRI and MEG.

The physiological effect of hyperoxia has been poorly characterize d, with studies reporting conflicting results on the role of hyperoxia as a vasoconstrictor. It is not clear whether hyperoxia is the primary contributor to vasoconstriction or whether induced changes in CO2 that commonly accompany hyperoxia are a factor. As calibrated BOLD fMRI based on hyperoxia becomes more widely used, it is essential to understand the effects of oxygen on resting cerebral physiology. This study used a RespirAct™ system to deliver a repeatable isocapnic hyperoxia stimulus to investigate the independent effe

Beyond the bolus: transgenic tools for investigating the neurophysiology of learning and memory.

Understanding the neural mechanisms underlying learning and memory in the entorhinal-hippocampal circuit is a central challenge of systems neuroscience. For more than 40 years, electrophysiological recordings in awake, behaving animals have been used to relate the receptive fields of neurons in this circuit to learning and memory. However, the vast majority of such studies are purely observational, as electrical, surgical, and pharmacological circuit manipulations are both challenging and relatively coarse, being unable to distinguish between specific classes of neurons. Recent advances in mol

Type and frequency of peripheral nerve injuries encountered in a clinical neurophysiology laboratory.

Clinical and electrophysiologic findings on 136 patients (39 women and 97 men; mean age = 33.2 years) with traumatic peripheral nerve injuries were reviewed. Motor vehicular collisions (MVCs) caused 39.7% and gunshot wounds (GSWs) produced 32.4% of the nerve injuries studied. Other factors such as falls, lacerations, work-related injuries, knife wounds, and blunt traumas produced the remaining 27.9% of the nerve injuries in the study group. Peripheral nerves of the upper limbs (79.6%) were disproportionately damaged in MVCs compared with those of the lower limbs (20.4 %). In MVCs, the brachial

Role of intraoperative neurophysiology in primary surgery for obstetrical brachial plexus palsy (OBPP).

OBJECTIVE: Management of conducting neuroma-in-continuity in primary surgery for obstetrical brachial plexus palsy (OBPP) is still discussed controversially. We present our experience with intraoperative neurophysiological recordings in the management of lesions in continuity in OBPP. METHODS: A series of ten children with lesions in continuity of the upper brachial plexus is presented. Due to recordable compound nerve action potentials (CNAPs) and muscle response to motor stimulation across the neuroma, five children underwent external neurolysis of neuroma only (neurolysis group). Due to la
