Minimally invasive evacuation for intracerebral hemorrhage: current practice and perspectives after ENRICH.

BACKGROUND: The results of the Early Minimally Invasive Removal of Intracerebral Hemorrhage (ENRICH) trial have renewed interest in minimally invasive hematoma evacuation (MIE) as a treatment option for intracerebral hemorrhage (ICH). However, variability persists in the surgical management of ICH among clinicians, and the impact of ENRICH on current practice is unclear. We conducted a survey to better understand current practices and perspectives on MIE for ICH. METHODS: We surveyed vascular neurologists, neurointensivists, and neurosurgeons who routinely manage patients with ICH. RESULTS: …

[Pituitary adenoma].

In this review, we discuss pituitary adenomas (PA), which account for 10-25% of the intracranial tumours. Despite their benign nature, PA often show invasive growth. Pressure on neighbouring structures may cause hypopituitarism or vision field impairment. For PA, except prolactinomas, surgical treatment is first choice. The primary surgical technique is transsphenoidal surgery. Pituitary function, vision and post-operative magnetic resonance imaging scan is evaluated in a multidisciplinary team of neurosurgeons and endocrinologist 6-8 weeks post-operatively. Follow-up and treatment of pituitar…

Strengthening neurosurgical care in Latin America: The Case for a regional framework.

Latin America faces a critical neurosurgical workforce shortage, with an estimated deficit of 924 neurosurgeons and nearly 40% of the populatio living more than two hours from neurosurgical care. Access is concentrated in urban centers, while rural and underserved communities remain disproportionately affected. Training opportunities are inconsistent, with a lack of standardized curricula and limited regional accreditation mechanisms, further exacerbating inequities. Progress has been hindered by fragmented data collection, inadequate policy coordination, and unequal resource distribution. Des…

Evaluating the Performance and Fragility of Large Language Models on the Self-Assessment for Neurological Surgeons.

BACKGROUND AND OBJECTIVES: The Congress of Neurological Surgeons Self-Assessment for Neurological Surgeons questions are widely used by neurosurgical residents to prepare for written board examinations. Recently, these questions have also served as benchmarks for evaluating large language models' (LLMs) neurosurgical knowledge. LLMs show significant promise for transforming neurosurgical practice; however, they are susceptible to in-text distractions and confounding factors. Given the increasing use of generative artificial intelligence and ambient dictation technologies, clinical text is at a

Challenges with segmenting intraoperative ultrasound for brain tumours.

Objective - Addressing the challenges that come with identifying and delineating brain tumours in intraoperative ultrasound. Our goal is to both qualitatively and quantitatively assess the interobserver variation, amongst experienced neuro-oncological intraoperative ultrasound users (neurosurgeons and neuroradiologists), in detecting and segmenting brain tumours on ultrasound. We then propose that, due to the inherent challenges of this task, annotation by localisation of the entire tumour mass with a bounding box could serve as an ancillary solution to segmentation for clinical training, enco…

Neurosurgery at Pearl Harbor: Ralph Cloward's legacy.

Ralph B. Cloward (1908-2000) was the sole neurosurgeon present during the Japanese attack on Pearl Harbor on December 7, 1941. Cloward operated on 42 patients in a span of 4 days during the attacks and was awarded a commendation signed by President Franklin D. Roosevelt in 1945 for his wartime efforts. During the attacks, he primarily treated depressed skull fractures and penetrating shrapnel wounds, but he also treated peripheral nerve and spine injuries in the aftermath. His techniques included innovative advancements such as tantalum cranioplasty plates, electromagnets for intracranial meta

Designing Penfield: Inside the Montreal Neurological Institute.

Neurosurgeon Wilder Penfield (1891-1976) envisioned hospital architecture as a powerful medical tool. Focusing on two key interiors in the 1934 Montreal Neurological Institute (MNI)-the operating room and the foyer-this article engages newly accessible textual and material evidence to show Penfield's intense involvement in the design of the building. A unique, tri-level surgical room, with a sophisticated setup for photography, made the MNI's surgery interactive. The OR is discussed with regard to the relationship of doctors and architects and Penfield's penchant for architectural travel. Subs

#Fake news: a systematic review of mechanical thrombectomy results among neurointerventional stroke surgeons on Twitter.

OBJECTIVE: Twitter is a popular social media platform among physicians. Neurointerventionalists frequently document their lifesaving mechanical thrombectomy cases on Twitter with very favorable results. We fear that there may be some social media publication bias to tweeted mechanical thrombectomy cases with neurointerventionalists being more likely to tweet cases with favorable outcomes. We used these publicly documented cases to analyze post-intervention Twitter-reported outcomes and compared these outcomes with the data provided in the gold standard literature. METHODS: Two reviewers perfo
