BACKGROUND & AIMS: Patients with folate deficiency and elevated homocysteine (Hcy) levels have increased risk of cerebrovascular and cardiovascular disease. They may also be at increased risk of complications after surgery because nitrous oxide interferes with folate metabolism. The aim of this study was to assess the incidence of folate deficiency and hyperhomocysteinemia in patients presenting for major surgery. We also tested the utility of a brief preoperative medical and dietary questionnaire to identify those with low folate or elevated Hcy levels.

BACKGROUND: Some drugs used for sedation and anesthesia produce histopathologic central nervous system changes in juvenile animal models. These observations have raised concerns regarding the use of these drugs in pediatric patients. We summarized the findings in developing animals and describe the steps that the Food and Drug Administration (FDA) and others are taking to assess potential risks in pediatric patients. Methylmalonic acid (MMA) can assess tissue B12 more accurately. NICE published clinical guidelines on Vitamin B12 deficiency in adults in 2024. Renal effects of anesthetic doses of morphine (2 mg./kg.) administered intravenously (I.V.) were determined in 15 mongrel dogs before and after addition of 50 percent nitrous oxide (N2O).
