Late-onset neonatal sepsis is a global issue that affects thousands of infants in the neonatal intensive care unit (NICU) every year. With mortality and morbidity remaining a significant outcome of late-onset neonatal sepsis and progress plateauing, this article will explore how artificial intelligence can be utilized as a tool to assist NICU providers and nurses in administering timely treatment. Through exploring the use of machine learning in the NICU, we can hope to see a new standard of care developed for infants worldwide and prevent unnecessary death in our tiniest patients.

Nurses are vital to facilitating patient-centered discussions on alcohol use, substance use, and contraception, yet they face significant barriers to effective communication, including time constraints, stigma, and implicit bias. This article presents an inclusive framework to support culturally safe, patient-centered dialogue, with attention to neurodiverse populations, including those with fetal alcohol spectrum disorders. Drawing on the Model of the Five Principles of Cultural Safety and the Respectful Maternity Care Framework and Evidence-Based Clinical Practice Guideline from the Associat

BACKGROUND: Low-cost bubble continuous positive airway pressure (bCPAP) systems have been shown to improve survival in neonates with respiratory distress, in developing countries including Malawi. District hospitals in Malawi implementing CPAP requested simple and reliable guidelines to enable healthcare workers with basic skills and minimal training to determine when treatment with CPAP is necessary. We developed and validated TRY (T: Tone is good, R: Respiratory Distress and Y=Yes) CPAP, a simple algorithm to identify neonates with respiratory distress who would benefit from CPAP.

Reflecting on our experience as nurses in the Neonatal Intensive Care Unit (NICU), as well as on the urgency in the assistance to hospitalized newborns (NB), we aimed at describing the interaction between nurses and NBs during the practice of care in the oral tracheal aspiration and in the blood collection for laboratory exams, with emphasis on the physiological and behavioral responses. This is an exploratory and descriptive research, in which we used the Paterson and Zderad Theory (1976). It was conducted in a public maternity ward in Fortaleza, State of Ceará, with six nurses and 21 babies

Today's healthcare system is in a state of transformation, as changes in technology, diagnostic approaches, treatments, and levels of professional practice occur on a regular basis. Nurses continue to be the most trusted profession. What has not changed in clinical practice is the amount of responsibility and level of accountability. Therefore, nurses are morally, legally, and ethically responsible for nursing judgment and clinical-based actions covered under each state's Nurse Practice Act, the American Nurses Association's core principles, and position statements as well as standard setting

Reproductive technology has made it possible for those born biologically female and who have partially transitioned to the male gender via hormones to become pregnant and give birth. This article explores the role of the perinatal nurse in providing care during the obstetric experience for a transgender male and his significant other. A carefully executed plan of care can assist the nurse in providing nonjudgmental, nondiscriminatory physical and emotional nursing care, and ensure that the family's healthcare needs are met and that their transition into parenthood is effective.

When women addicted to opioids seek prenatal care, the treatment of choice is methadone. Methadone mediates the addiction by reducing fluctuations in maternal serum opioid levels and protecting the fetus from repeated withdrawal episodes. Methadone maintenance is associated with increased maternal weight gain, decreased illegal drug use, and improved compliance with prenatal care. Although the risks are less when compared with street drugs, the risk to the fetus is physical dependence. Despite the magnitude of this national problem, there is a dearth of literature to guide NICU nurses on how t

AIM: Concerns for infant destabilisation often prohibit parental holding of infants during therapeutic hypothermia (TH). We assessed the feasibility of maternal holding during TH, as the inability to hold can impede bonding.
METHODS: Vital signs were assessed in stable infants before, at two-minute intervals during and 30 minutes after a single 30-minute holding session. The infant remained on the blanket throughout holding, and both infant and blanket were placed into the mother's arms on top of a thin foam insulating barrier. Mothers and nurses were surveyed about their experience.

OBJECTIVE: This study was undertaken to evaluate the effect of change of shift for physicians and nurses on complications associated with cesarean delivery.
STUDY DESIGN: 17,996 term women undergoing an unscheduled cesarean delivery in 13 centers from 1999-2000 were included. Maternal and neonatal morbidities were evaluated by time of infant delivery vis-à-vis nursing change of shift (6 AM-8 AM, 2 PM-4 PM, 10 PM-12 AM vs all other hours). The sample was then limited to weekdays only and physician shift changes were evaluated (physician shift change 6 AM-8 AM, 5 PM-7 PM vs all others). A compo

INTRODUCTION: Umbilical cord care constitutes a fundamental component of routine neonatal nursing. Authoritative guidelines on this practice date back to the 1980s, but these recommendations have remained unexplored in terms of clinical adoption.
OBJECTIVE: The primary objective was to identify the best available evidence regarding neonatal umbilical cord care in a neonatal intensive care unit (NICU) and evaluate its clinical effectiveness.
METHODS: A quality improvement project guided by the Knowledge-to-Action (KTA) framework was conducted in the NICU of a tertiary hospital in China. Evide

Successful oral feeding of preterm and other ill and fragile infants is an interactive process that requires (1) sensitive, ongoing assessment of an infant's physiology and behavior, (2) knowledgeable decisions that support immediate and long-term enjoyment of food, and (3) competent skill in feeding. Caregivers can support feeding success by using the infant's biological and behavioral channels of communication to inform their feeding decisions and actions. The Supporting Oral Feeding in Fragile Infants (SOFFI) Method is described here with text, algorithms, and reference guides. Two of the a

BACKGROUND: Neonates that have been subjected to perinatal asphyxia and fulfill criteria for therapeutic hypothermia are cooled to 33.5 °C for 72 h. There is no consensus regarding sedation and analgesic use during hypothermia, but there is evidence supporting the importance of pain relief and adequate sedation. There is a need for assessment of the neonates need for pain relief and sedation, and for adjustments of medication to ensure adequate treatment. There are many different scoring tools available. We found the N-PASS (Neonatal Pain, Agitation and Sedation Scale) scoring tool to be the m

Necrotizing fasciitis (NF) is a disease characterized by the destruction of the skin, subcutaneous fat, and fascia, with or without inflammation of the muscle tissue. NF carries a mortality rate in excess of 50 percent in neonates. Rapid diagnosis and initiation of antibiotics, along with meticulous nursing care, can provide an acceptable outcome for the neonate. This article reviews the pathophysiology, diagnosis, and treatment of NF; presents a case study; and provides a nursing care plan that supports treatment of this disease process.

Spinal dysraphism, an incomplete closure of the neural tube, can be open, exposing the neural elements to the environment, or can be closed, covered with skin. Abnormal development of the spine occurs early in life and also interferes with usual development of the skin. This often creates cutaneous markers or stigmata over the area. Cutaneous markers may include a subcutaneous mass, abnormal hair growth, skin dimple, tag or sinus, or unusual pigmentation. Recognizing these markers is important because, although many closed spinal dysraphisms are asymptomatic at birth, neurological sequelae can
