[State of ophthalmology in ancient Rome].

The article presents the state of medical knowledge of the peoples forming Etruscan and Roman civilization circa 600 years B. C. Medical practices of these peoples are discussed. It is underlined that some Etruscan views were adopted by the Romans who cared much about hygiene. However, the development of Roman medicine was strictly related to the Greek And Egyptian physicians to this country. Various conditions of ophthalmic inflammation and many ocular diseases like pterygium, entropionand tumor of the lids were known in the Roman society. We owe to Celsus the description of the couching of t

Neuro-ophthalmology of eye-movement disorders.

Magnetic resonance and functional brain imaging continue to expand their roles in complementing the clinical examination to localize the causes of eye-movement disorders. A retrospective review from county-based clinical practices emphasized generally benign causes of third, fourth, and sixth cranial nerve palsies in children, but a study from a tertiary referral center found a high incidence of neoplasms causing sixth-nerve palsies in children. A prospective study found that 38% of diabetic patients with third-nerve palsies had anisocoria up to 2.5 mm. An initial attempt to develop an evidenc

The ophthalmology of intracranial vascular abnormalities.

PURPOSE: To provide a practical review of the ophthalmologic manifestations of intracranial vascular abnormalities. METHODS: We reviewed ocular manifestations of the most common intracranial vascular abnormalities: intracranial aneurysms, carotid-cavernous fistulas, arteriovenous malformations, and cavernous malformations. RESULTS: Unruptured aneurysms can compress the third cranial nerve and the anterior visual pathways. Ruptured aneurysms and subarachnoid hemorrhage can result in Terson syndrome and papilledema. Direct and indirect carotid-cavernous fistulas most commonly cause the classic

Preventative ophthalmology. Age-related macular degeneration.

PURPOSE: This report was designed to review the current knowledge about preventing the development or progression of age-related macular degeneration (AMD) and visual loss associated with this entity in practical terms for the comprehensive ophthalmologist. METHODS: Animal studies, epidemiologic studies, and clinical trials identified through the use of MEDLINE, a reference list of articles reviewed, and personal contact with experts in this area provided information reviewed for this report. RESULTS: Reports concerning ultraviolet and visible light provide limited, inconsistent, and conflic

[How to use glucocorticoids in ophthalmology].

Administration of glucocorticoid, one of the corticosteroid hormones, is one of the most important therapeutic strategy in ophthalmological treatment. Almost all kinds of glucocorticoid agents are commercially available for various ocular diseases, including cortisone, hydrocortisone, prednisolone, methylprednisolone, triamcinolone, dexamethasone, paramethasone, betamethasone and fluolometholone. Glucocorticoids are administrated in preparations of eye drops, ointments, injections and tablets for local and systemic use. Systemic glucocorticoid treatment should, in general, only be considered w

Neuro-ophthalmology: the afferent visual system.

Recent advances in neuro-ophthalmology have been made in both diagnosis and therapy. New discoveries in molecular genetics have resulted in the identification of abnormalities in the nuclear and mitochondrial genomes likely responsible for retinitis pigmentosa and Leber's hereditary optic neuropathy, respectively. Neuroimaging continues to broaden our diagnostic abilities in both optic nerve disease and lesions of the retrochiasmal visual pathways. The role of steroids in idiopathic optic neuritis has been clarified by the Optic Neuritis Treatment Trial. The optic nerve sheath decompression pr…

Botulinum A toxin (Oculinum) in ophthalmology.

Botulinum A toxin has been used to treat strabismus and a variety of spasmodic neuromuscular diseases. Botulinum toxin treatment of strabismus is not as definitive and stable as the traditional surgical approach, but it has been found most useful in postoperative overcorrection, small deviations, sensory deviations, and acute sixth nerve palsy. This toxin has been effective in the treatment of essential blepharospasm and hemifacial spasm, for which it produces temporary relief of symptoms. In addition, this treatment has been applied to lower lid entropion, myokymia, aberrant regeneration of t

Seven hundred medicolegal cases in ophthalmology.

Seven hundred medicolegal claims in ophthalmology were reviewed by one ophthalmologist who served as an expert for four decades. The ophthalmologist was personally involved in 620 claims. The 700 cases have been categorized and analyzed. The reasons for the claims and some lessons derived from them are presented. Familiarity with the claims encountered by others may enable ophthalmologists to avoid similar claims.

[Use of botulinum toxin in ophthalmology].

Botulin toxin A was introduced as a treatment in ophthalmology by Dr. Scott of San Francisco. One important application is in cases of blepharospasm, where the toxin is injected into the lateral parts of the lower and upper lid and, if necessary, over the eyebrows in a single dose of 1-2 nanograms, preferably using a needle under electromyographic control. The effect on the blepharospasm is visible after a few days and lasts for several months. The procedure can be repeated several times. The second application is in cases of strabismus. In paralytic strabismus, contracture of the antagonist o…

[Clinical trials of interferon in ophthalmology].

The clinical trials of interferon in ophthalmology have been carried out mainly using a method of topical administration to viral keratoconjunctivitis. Eye infections by herpes simplex virus in humans have been shown to be benefited by interferon. Topical combination therapy of herpetic keratitis by interferon with debridement or other antiviral agents would be more effective. Epidemic keratoconjunctivitis have failed to respond favourably to interferon except some symptoms.

Noncorticosteroidal anti-inflammatory agents in Ophthalmology.

Efforts have been made to develop noncorticosteroidal anti-inflammatory therapy. The goals of such therapy have been to eradicate the primary cause, prevent the initial tissue injury, moderate inflammatory response, and enhance tissue repair while minimizing and eliminating the undesirable aspects of the therapy.
