OBJECTIVE: The Photographic Screening for Retinopathy of Prematurity Study (Photo-ROP) sought to evaluate the utility of digital wide-angle photographic fundus screening for retinopathy of prematurity (ROP) as compared to bedside indirect ophthalmoscopy. This syndrome arises from numerous potential causes, which may pose varying degrees of threat to patients. AIM: To investigate whether retinal changes in children with severe malaria affect visual acuity 1 month after systemic recovery.
METHODS: All children with severe malaria admitted to a research ward in Malawi during one malaria season were examined by direct and indirect ophthalmoscopy. Visual acuity was tested in those attending follow up by Cardiff cards, Sheridan-Gardiner single letters, or Snellen chart.

Patients with AIDS-related cytomegalovirus (CMV) retinitis receiving combined antiretroviral therapy (cART), but not specific anti-CMV therapy, consistently showed active retinitis for several months. Delayed diagnosis and treatment of CMV retinitis may have severe consequences. Patients first entering care with advanced HIV infection and vulnerability to reactivation of latent CMV infection should be screened immediately for CMV retinitis by dilated indirect ophthalmoscopy and treated with specific anti-CMV therapy without delay, in addition to cART.

This study shows how optical coherence tomography has been valuable in the diagnosis of congenital X-linked retinoschisis. An infant with convergent strabismus underwent serial optical coherence tomography examinations that helped to make the diagnosis of congenital X-linked retinoschisis. Optical coherence tomography is useful in the diagnosis of congenital X-linked retinoschisis and adds new information about pathogenesis. The authors recommend the use of optical coherence tomography in pediatric retinal disease, even if the macula appears normal on indirect ophthalmoscopy.

A 55-year-old woman with chronic renal failure treated with hemodialysis had severe bilateral visual loss develop due to retinal ischemia. Ophthalmoscopy showed crystals in the distribution of the retinal arteries, but not veins, and this led to a diagnosis of systemic oxalosis. Factors contributing to systemic oxalosis in addition to renal failure were ascorbic acid dietary supplementation, pyridoxine deficiency, and ileal resection. Histopathologic findings showed ocular calcium oxalate deposition limited nearly entirely to the walls of retinal blood vessels.

Ophthalmoscopy performed during a routine eye checkup on a 22-year-old black man revealed multiple intraretinal, cyst-like lesions scattered throughout the macular and extramacular areas in both eyes. Based on clinical findings, morphologic appearance, and an abnormal electro--oculogram, the diagnosis of multifocal Best's macular dystrophy was made. Hemoglobin electrophoresis showed an AS pattern confirming the presence of sickle cell trait. This is the first case of the coexistence of these two genetically transmitted diseases, Best's disease and sickle cell trait.

Altitudinal visual field defects in five cases with optic nerve lesions due to optic glioma, compression, and trauma are correlated to retinal nerve fibre degeneration, recorded with ophthalmoscopy and photography in red-free light. In one case the optic nerve was damaged at operation. Repeated fundus photography was used to record the evolution of descending optic atrophy. The results suggest that the findings in funduscopic examination of the retinal nerve fibre layer should be cautiously judged and the method cannot replace careful perimetry in cooperative patients.

Wolfram syndrome, also known as DIDMOAD syndrome, is a rare disease characterized by the association of diabetes mellitus, diabetes insipidus, optic atrophy and deafness. The ophthalmologic findings are largely dominated by optic atrophy.
