Optic neuritis following chickenpox in adults.

Three cases of bilateral optic neuritis, one of which also had transverse myelitis, are described in young adults following chickenpox. Severe bilateral loss of vision developed in all cases and was followed by good visual recovery in the two with isolated optic neuritis, but poor recovery in the patient with concomitant transverse myelitis. The interval between the initial rash and subsequent optic neuritis suggests an immunological pathogenesis for this complication.

Malignant external otitis with optic neuritis.

Malignant external otitis (MEO) is a progressive necrotizing infection which spreads to the skull base. The causative organism is usually Pseudomonas aeruginosa and 90% of the patients are diabetic. The infection gains access to the skull base at the temporal bone. Cranial nerve involvement is common. We present a case of malignant external otitis causing blindness due to optic neuritis. Progressive vascular involvement along the skull base is the pathogenic mechanism that best explains spread from the temporal bone to the orbital apex.

Optic neuritis in cat scratch disease.

A patient with optic neuritis is described whose associated lymphadenopathy, cat exposure, and positive cat scratch antigen skin test suggested the diagnosis of cat scratch disease. Optic neuritis and other neurologic abnormalities, especially encephalopathy, may be associated with, or the presenting manifestation of, cat scratch disease. To our knowledge, this is the fourth case of cat scratch disease-related optic neuritis to be reported.

Optic neuritis due to acquired toxoplasmosis.

A pediatric patient with well documented acquired toxoplasmic optic neuritis is presented. Use of immunoglobulin M immunofluorescent antibody studies allow easier identification of recently acquired toxoplasmosis. Toxoplasmic optic neuritis and retinitis should be considered along with toxoplasmic meningitis or encephalitis as complications of acquired toxoplasmosis. Therapy is available which may decrease the duration of visual symptomatology and reduce residual visual impairment.

Optic neuritis in inflammatory bowel disease.

It is not unusual for various ocular diseases to be associated with inflammatory bowel disease. Ocular involvement includes the inflammatory entities of keratitis, conjunctivitis, episcleritis, orbital pseudotumor, iritis and neuroretinitis , as well as those states in which the etiology remains undefined, i.e., scleromalacia perforans, peripheral corneal ulceration and furrow, retinal artery occlusion, and optic neuropathy. We report five cases of optic neuropathy associated with ulcerative colitis to emphasize that this may be the sole ocular manifestation of inflammatory bowel disease.

Computed tomography of chiasmal optic neuritis.

Three cases of chiasmal optic neuritis are described with bitemporal visual field loss and enlargement of the optic chiasm as demonstrated by computed tomography (CT). Exploratory craniotomies were performed in two of the patients, but no tumor was found. After corticosteroid treatment, all three patients experienced virtually complete restoration of normal vision. Although the CT presentation in these patients was indistinguishable from that of a chiasmal glioma, the clinical presentation of acute visual loss in an older child or adult should suggest chiasmal optic neuritis. A period of obser…

Suprathreshold vision in acute optic neuritis.

The perception of contrast was measured in patients with acute unilateral optic neuritis by a technique of subjective suprathreshold contrast matching, and was compared with contrast sensitivity as defined by threshold measurements. The suprathreshold apparent contrast and threshold contrast sensitivity was repeatedly assessed during the recovery phase. Generally, an attenuation of suprathreshold apparent contrast was found for high and intermediate spatial frequencies in the eye with optic neuritis. At a low spatial frequency, however, the suprathreshold contrast vision was spared. The thresh…

Optic neuritis in systemic lupus erythematosus.

Four patients with systemic lupus erythematosus (SLE) and optic neuritis are described. Optic neuritis is uncommon in SLE but may be the presenting feature of the disease. A review of 8 similar cases in the literature indicates that myelopathy is frequently associated, prompting the diagnosis of multiple sclerosis. Visual loss may be permanent despite therapeutic intervention, but rapid improvement occurs in some patients who receive early treatment with corticosteroids.

Computed tomographic features in optic neuritis.

A computed tomography scan obtained in two patients with monocular visual loss showed diffuse enlargement and enhancement of the intraorbital portion of the involved optic nerve. Although these tomographic features were initially interpreted as an optic nerve glioma by the neuroradiologist, the ensuing clinical course established the diagnosis of optic neuritis.

[Optic neuritis in childhood (author's transl)].

The symptoms and signs of twenty-one children under 15 years of age with optic neuritis are presented here. The optic neuritis often was bilateral and accompanied by papilledema. In the acute stage there was however no typical central scotoma in every case. Some children had only peripheral visual field defects. The visual disorder will not improve so much as is general assumed: a slight decrease of visus and visual field defects in static perimetry usually persist. Half of the children developed signs of multiple sclerosis within a few years.

Pupil cycle time in optic neuritis.

A thin slit-lamp beam illuminating the pupil margin produced clearly visible pupil oscillations. These oscillations were timed with a stopwatch, thus producing a measurement of the "pupil cycle time". The pupil cycle time was remarkably stable in various testing situations and repeatable within +/- 3% over extended periods of time. When the iris muscles were normally innervated and responsive, the pupil cycle time was dependent on the speed of conduction and the number and strength of optic nerve impulses. Pupil cycle time can be measured in most persons with active or inactive optic neuritis.…
