Endoscopic ossiculoplasty in traumatic conductive hearing loss with intact tympanic membrane: a five-year experience.

BACKGROUND: Exploratory tympanotomy in cases of traumatic ossicular disruption with intact tympanic membrane is crucial for both diagnostic and therapeutic purposes. Performing this procedure using the endoscope is gaining popularity. Hence, this study aimed to demonstrate varieties of ossicular pathology and their management in our institution. METHODS: A retrospective evaluation was conducted of 136 ears in patients with traumatic ossicular disruption with an intact tympanic membrane, who underwent endoscopic exploratory tympanotomy. A proposed algorithm was followed, to incorporate differe

Performance of the round window soft coupler for the backward stimulation of the cochlea in a temporal bone model.

The round window vibroplasty is a feasible option for the treatment of conductive, sensorineural and mixed hearing loss. Although clinical data suggest a satisfying clinical outcome with various coupling methods, the most efficient coupling technique of the floating mass transducer to the round window is still a matter of debate. For this, a soft silicone-made coupler has been developed recently that aims to ease and optimize the stimulation of the round window membrane of this middle ear implant. We performed a temporal bone study evaluating the performance of the soft coupler compared to the

Vibroplasty combined with tympanic membrane reconstruction in middle ear ventilation disorders.

Although the Vibrant Soundbridge is one of the most frequently used active middle ear implants, data regarding how middle ear ventilation disorders may affect the transmission behavior of its floating mass transducer are still insufficient. Studies involving coupling the floating mass transducer to the stapes head are particularly lacking. This temporal bone study evaluated the influence of simulated middle ear ventilation disorders on the middle ear transfer function in the reconstructed middle ear. The middle ear transfer function was measured using Laser Doppler Vibrometry after vibroplasty

WITHDRAWN: Management of Congenital Incudostapedial Anomalies in Stapes Surgery.

OBJECTIVES:: The management of developmental anomalies of the ossicular chain and otic capsule in stapes surgery tests the otologist's diagnostic skills and flexibility in technique and prosthesis choice. Although congenital malleus fixation and perilymph gusher are important clinical entities that have received considerable attention in the otolaryngology literature, congenital anomalies of the incus and stapes are less well described, can often be subtle, and have a distinct influence on management choice and hearing outcome. STUDY DESIGN AND PATIENTS:: A review of more than 200 stapes proce

A new double-cartilage block ossiculoplasty: long-term results.

OBJECTIVES: To determinate whether ossicular chain reconstruction performed with modified double-cartilage block (DCB) ossiculoplasty used by authors, leaving the intact perichondrium layer on the cartilage surface in contact with tympanic membrane, results in significant hearing results and prevents DCB displacement and DCB loss of stiffness when compared with the original DCB technique described by Luetje and Denninghoff. MATERIALS AND METHODS: Sixty-five ossicular reconstructions using DCB ossiculoplasty were performed from 1996 to 2001 in the whole study group. Eight patients had an inade

A new self-retaining titanium clip stapes prosthesis.

OBJECTIVE: Crimping of the stapes prosthesis might result in injury to the incus. Attachment is often not tight. Facilitated fixation of a stapes prosthesis could improve surgery and provide more stable results. MATERIALS AND METHODS: After the development of a unique titanium clip over a 6-year period (Kurz AG, Dusslingen, Germany) and reception of a CE mark, the first implantation was performed in September 2000. Full FDA approval was received in June 2002. RESULTS: Only 60% of the circumference of the incus is touched by the clip permitting adequate mucosal blood supply to the lenticular

Tympanoplasty type II with ionomeric cement and titanium-gold-angle prostheses.

OBJECTIVE: The pronounced isolated erosion of the long incus process is a challenging situation during tympanoplasty. Here, we report two methods for reconstruction of the ossicular chain as tympanoplasty type II with the incus remaining in situ. (1) lonomeric cement is used, resulting in two characteristics: a direct link between stapes head and the long incus process is achieved. At the stapes head the link possesses a joint allowing physiological movements between incus and stapes. (2) Titanium-gold-angle prosthesis according to Plester were crimped to the long process of the incus and posi

Ossiculoplasty with a cartilage-connecting hydroxyapatite prosthesis for tympanosclerotic stapes fixation.

Many cases of tympanosclerotic stapes fixation are accompanied by fixation or erosion of malleus and/or incus. This status of the ossicular chain is one of the reasons that ossiculoplasty for tympanosclerotic stapes fixation is more difficult than that for otosclerosis. We conducted a retrospective review of seven patients who were operated on for tympanosclerotic stapes fixation between 2002 and 2006. All of the patients had abnormal conditions of the malleus and/or incus and underwent stapedectomy and total ossiculoplasty with hydroxyapatite prosthesis (Apaceram T-7 type), which has a planar

A novel implantable hearing system with direct acoustic cochlear stimulation.

A new implantable hearing system, the direct acoustic cochlear stimulator (DACS) is presented. This system is based on the principle of a power-driven stapes prosthesis and intended for the treatment of severe mixed hearing loss due to advanced otosclerosis. It consists of an implantable electromagnetic transducer, which transfers acoustic energy directly to the inner ear, and an audio processor worn externally behind the implanted ear. The device is implanted using a specially developed retromeatal microsurgical approach. After removal of the stapes, a conventional stapes prosthesis is attach

Isolated Traumatic Dislocation of the Malleus into the External Auditory Canal.

We report a rare case of isolated malleus dislocation into the external auditory canal with lateralized intact tympanic membrane following a head trauma. The patient was a 63-year-old woman who presented at the outpatient department of our institute with hearing loss of 10 years' duration after a bicycle accident. During physical examination of the patient, total dislocation of the malleus-like bony structure into the external auditory canal on the right side was observed. In the computed tomography scan, an isolated malleus dislocation with intact incus-stapes articulation was identified. The

Active middle ear implants: Vibroplasty™ in children and adolescents with acquired or congenital middle ear disorders.

CONCLUSION: Active middle ear implant (AMEI) implantation in children and adolescents is safe and provides improved hearing results. No statistical difference in hearing outcomes was shown in the group of patients affected by chronic middle ear diseases versus aural atresia. Also, the transducer location (round window versus oval window placement) did not lead to different outcomes in hearing abilities. OBJECTIVES: (1) To assess the hearing outcomes with the active implant Vibrant Soundbridge (VSB) in children and adolescents. (2) To evaluate whether functional results of the subjects in the
