What other biologic and mechanical factors might contribute to osteolysis?

An overwhelming consensus exists that wear particles are the primary driving force in aseptic loosening of orthopaedic implants. Nonetheless, considerable evidence has emerged demonstrating that various other factors can modulate the biologic activity of orthopaedic wear particles. Two of the most studied modulating factors are bacterial endotoxins and implant motion.

Does pelvic osteolysis occur with a nonmodular uncemented acetabular component?

We present a radiographic follow-up of an uncemented, nonmodular, pure titanium-backed acetabular component at 6 to 12 years postimplantatio … No cup has been revised for aseptic loosening. There was no radiographic evidence of periacetabular osteolysis or loosening of any of the cups, even in a subgroup of 31 patients who were 60 years or younger at the time of implantation.

Magnetic resonance imaging to evaluate osteolysis around total knee arthroplasty.

Wear-induced osteolysis remains a common problem after total knee arthroplasty and may be particularly difficult to detect clinically or radiographically around the femoral component. We describe the use of magnetic resonance imaging to identify particulate-induced osteolysis around the distal femur and proximal tibia in 2 patients with total knee arthroplasties. The extent of the osteolysis was not apparent from standard x-rays, and the information provided by the magnetic resonance imaging facilitated preoperative discussion with the patient as well as preoperative planning for the surgery.

Gene expression of bone-resorbing cytokines in rat osteolysis model.

Osteolysis after single-injection polyethylene (PE) administration has been analyzed, but this experimental model did not reflect the clinical postoperative condition. Therefore, we investigated the bone-resorbing cytokines interleukin and tumor necrosis factor (IL-1alpha, IL-6, TNFalpha) produced by continuous PE administration using histomorphologic findings and the reverse transcription polymerase chain reaction. TNFalpha mRNA was continuously detected in the PE continuous-infusion group, whereas it was not expressed in the PE single-injection group. IL-1alpha and IL-6 mRNA were detected in…

Early osteolysis following second-generation metal-on-metal hip replacement.

BACKGROUND: Total hip arthroplasty with use of metal-on-metal bearings has been reintroduced as an alternative to the use of metal-on-polyethylene bearings because of theoretical advantages such as reduced wear and a lower prevalence of osteolysis. However, we observed early osteolysis in a cohort of patients who had been managed with second-generation metal-on-metal hip replacements and investigated the possible etiologic role of metal hypersensitivity. METHODS: We retrospectively analyzed 165 patients (169 hips) who had undergone primary cementless total hip replacement with a contemporary …

Chronic cerebrospinal fluid leak into skull base causing intramedullary osteolysis.

We present the rare case of a 74-year-old woman with extensive intramedullary osteolysis of the petroclival skull base straddling both sides of the basi-occiput and basi-sphenoid. She presented with clinical features of recurrent spontaneous cerebrospinal fluid (CSF) otorrhoea despite three previous attempts at repair of the CSF leak. Recent advances in imaging techniques enable accurate radiological diagnosis of skull base lesions. We performed T1- and T2-weighted magnetic resonance (MR) images, a fluid attenuated inversion recovery (FLAIR) sequence MR, and a diffusion scan to study the chara…

Radial neck osteolysis after annular ligament reconstruction. A case report.

The treatment of the dislocated radial head in a late Monteggia lesion remains a matter of controversy. Annular ligament reconstruction using a strip of triceps tendon is a commonly used technique. Although most published series report good and excellent results, the overall complication rate of this procedure is not known. This study reports a 9-year-old boy who had avascular necrosis and bone resorption of the radial neck after a late annular ligament reconstruction using a sling of triceps tendon for a radial head dislocation that was not detected previously.

Acro-osteolysis due to protein S deficiency: A rare case.

Protein S deficiency is a hypercoagulable state due to deficiency or dysfunction of protein S resulting in an abnormally regulated clotting cascade. Anti-phospholipid antibodies (APLA) may cause protein deficiency. Most of the cases reported in the literature are associated with venous thromboembolic events. Arterial thrombosis leading to gangrene due to underlying protein S deficiency is relatively rare. We report the uncommon and first-ever case of osteolysis of the great toes in a young male, nonsmoker, APLA negative, and protein S deficient.

Increased UHMWPE Particle-Induced Osteolysis in Fetuin-A-Deficient Mice.

Particle-induced osteolysis is a major cause of aseptic prosthetic loosening. Implant wear particles stimulate tissue macrophages inducing an aseptic inflammatory reaction, which ultimately results in bone loss. Fetuin-A is a key regulator of calcified matrix metabolism and an acute phase protein. We studied the influence of fetuin-A on particle-induced osteolysis in an established mouse model using fetuin-A-deficient mice. Ten fetuin-A-deficient (Ahsg−/−) mice and ten wild-type animals (Ahsg+/+) were assigned to test group receiving ultra-high molecular weight polyethylene (UHMWPE) particle i…

Breast Cancer Induced Bone Osteolysis Prediction Using Temporal Variational Autoencoders.

Objective and Impact Statement. We adopt a deep learning model for bone osteolysis prediction on computed tomography (CT) images of murine breast cancer bone metastases. Given the bone CT scans at previous time steps, the model incorporates the bone-cancer interactions learned from the sequential images and generates future CT images. Its ability of predicting the development of bone lesions in cancer-invading bones can assist in assessing the risk of impending fractures and choosing proper treatments in breast cancer bone metastasis. Introduction. Breast cancer often metastasizes to bone, cau…

Pathogenesis, Evaluation, and Management of Osteolysis After Total Shoulder Arthroplasty.

Radiographic osteolysis after total shoulder arthroplasty (TSA) remains a challenging clinical entity, as it may not initially manifest clinically apparent symptoms but can lead to clinically important complications, such as aseptic loosening. A thorough consideration of medical history and physical examination is essential to rule out other causes of symptomatic TSA-namely, periprosthetic joint infection-as symptoms often progress to vague pain or discomfort due to subtle component loosening. Once confirmed, nonoperative treatment of osteolysis should first be pursued given the potential to a…

Metal-Organic Framework Nanoparticles for Ameliorating Breast Cancer-Associated Osteolysis.

Breast cancer metastases to bone poses a significant challenge for the administration of treatment strategies. The bone microenvironment, metastatic tumor cells, osteoclasts, and tumor-associated macrophages (TAMs) all play crucial and synergistic roles in creating a favorable environment for the proliferation, progression, and survival of the metastatic tumor, which in turn induces osteoclast-mediated bone destruction. In this study, we functionalized immunostimulatory cytosine-phosphate-guanosine (CpG)-loaded metal-organic framework (MOF) nanoparticles with bone targeting capabilities by sur

Screw Track Osteolysis in the Cementless Total Knee Replacement Design.

BACKGROUND: There is a paucity of reports on osteolysis associated with tibial screw fixation in cementless total knee arthroplasty (TKA), and the pathophysiology is not clear. This study aimed to describe the pathology related to screw track osteolysis around the tibia in cementless TKA. METHODS: The study cohort comprised 100 revised cementless TKAs with tibial screw fixation. Screw track osteolysis and various screw angles were analyzed radiologically. Tissue samples from the joint capsule and the osteolytic cavity were investigated for metal/polyethylene wear. The type of tissue response …

Evidence for Gender-Specific Bone Loss Mechanisms in Periprosthetic Osteolysis.

Osteolysis adjacent to total hip replacement (THR) prostheses is a major cause of their eventual failure. Periprosthetic osteolysis is associated with the production of bioactive particles, produced by the wear of articulating prosthesis surfaces. Wear particles invade the periprosthetic tissue, inducing inflammation and bone resorption. Previous studies have shown that osteocytes, the most numerous cell type in mineralised bone, can respond to wear particles of multiple orthopaedic material types. Osteocytes play important roles in bone resorption, regulating bone resorption by osteoclasts an…

Three cases of multicentric carpotarsal osteolysis syndrome: a case series.

BACKGROUND: Multicentric carpotarsal osteolysis syndrome (MCTO) is characterized by progressive destruction and disappearance of the carpal and tarsal bones associated with nephropathy. MCTO is caused by loss-of-function mutations in the MAF bZIP transcription factor B (MAFB) gene. CASE PRESENTATION: This report describes three unrelated patients with MAFB mutations, including two male and one female patient. Osteolytic lesions in the carpal and tarsal bones were detected at 2 years, 12 years, and 14 months of age, respectively. Associated proteinuria was noted at 4 years, 12 years, and 3 mon…
