Most of the data for this paper were taken from a 30-year follow-up of septic arthritis and a 25 year series of osteomyelitis seen in Dallas, Texas. A specific bacterial aetiology was determined in about 70% of patients with septic arthritis, utilizing culture of multiple fluids. The aetiological agent was influenced by the age of the patient. In the newborn Staph. aureus, group B streptococci and gram-negative organisms are found most commonly. In the older infant H. influenzae becomes a prominent pathogen, and in those over 2 years of age staphylococci, streptococci, H. influenzae and N. gon…

Two cases of acute hematogenous osteomyelitis of the rib are presented. In one case the etiologic agent was Staphylococcus aureus coagulase-positive and in the other it was Bacteroides corrodens. Although an uncommon disease, hematogenous osteomyelitis should be considered in the differential diagnosis of destructive lesions of the rib. Anaerobic and aerobic cultures should be obtained for bacteriologic analysis.

A protocol of treatment for acute haematogenous osteomyelitis has been evaluated in 75 children. Intravenous cloxacillin and benzylpenicillin were given in hospital until the child had improved after which oral antibodies and immobilisation were continued at home for a total of six weeks. Oral cloxacillin was used most frequently as Staphylococcus aureus was the major pathogen. Simple drainage of subperiosteal pus was carried out in the 17 children with clinical evidence of an abscess. Ninety-two per cent of the 55 children with acute osteomyelitis diagnosed early were cured by a single course…

A case is reported of acute osteomyelitis of the lumbar spine presenting with a psoas abscess which extended into the epidural space mimicking an epidural abscess. The patient also had severe septicaemia and a complete cauda equina lesion. The septicaemia was controlled by draining the abscess by an anterior approach to the lumbar spine. Decompression of the cord was achieved by removing the sequestrated discs, the necrotic body of the fourth lumbar vertebra and draining the epidural abscess by opening the posterior longitudinal ligament.

Five children with pneumococcal bone and joint infections are described. Three of the children were less than one year of age and two had sickle cell disease. All but one had a primary or coincident focus of acute pneumococcal infection elsewhere. In the four children with septic arthritis, osteomyelitis was diagnosed subsequently in bones contiguous to the infected joint. All patients were successfully treated, although one developed permanent joint dysfunction. Although an unusual infection, pneumococcal infection of bones and joints should be sought in the appropriate clinical setting since…
