Head rotation as an effective compensatory technique for dysphagia caused by unilateral cervical osteophytes.

OBJECTIVE: Dysphagia is common in patients with cervical osteophytes. We aimed to determine whether head rotation as a compensatory technique is effective for dysphagia caused by unilateral cervical osteophytes. METHODS: We retrospectively analyzed videofluoroscopic swallowing study (VFSS) data obtained in one university hospital. Patients whose VFSS showed pharyngeal stasis by mechanical obstruction due to cervical osteophytes were selected. They were divided into a unilateral skewed osteophyte group and a diffuse central osteophyte group as confirmed by laryngoscopy or computed tomography. …

Aggravation of dysphagia after surgical removal of anterior cervical osteophytes: a case report.

Dysphagia induced by anterior cervical osteophytes (ACOs) is frequently reported in older individuals. Surgical resection of ACOs is considered when conservative treatment fails, but its effectiveness is controversial owing to side effects after surgery. We present the case of a 78-year-old man who complained of progressive dysphagia that started 10 months previously. A videofluoroscopic swallow study (VFSS) showed prominent ACOs along C2-C6, which translocated the upper hypopharynx anteriorly, impinging the lumen and impairing epiglottic folding and laryngeal closure. Aspiration of a soft die…

Osteophytes on the zygapophyseal (facet) joints of the cervical spine (C3-C7): A skeletal study.

Previous studies have reported that osteophytes in the cervical vertebrae may cause immobility, neck stiffness, osteoarthritis, headaches, nerve entrapment syndromes, and compression of the vertebral artery. Our objective was to explore the osteophytes' expression on zygapophyseal joints C3-C7. This is a cross-sectional observational skeletal study. The study sample comprised 273 human skeletons of both sexes, aged 20-93, housed at the Natural History Museum, OH, USA. A grading system assessed the presence and severity of osteophytosis on the zygapophyseal joints. The chi-square test (SPSS 25.…

Osteoarthritis, Osteophytes, and Enthesophytes Affect Biomechanical Function in Adults With X-linked Hypophosphatemia.

CONTEXT: X-Linked hypophosphatemia (XLH) is a lifelong metabolic disease with musculoskeletal comorbidities that dominate the adult clinical … between individual laboratory instrumentation. RESULTS: Enthesophytes, degenerative arthritis, and osteophytes were found to be consistently bilateral and diffusely present at the spine and synovial joints across participants, with predominance at weight-bearing joints. Passive ROM in adults with XLH was decreased at the cervical spine, hip, knee, and ankle …

Ultrasound Diagnosis of Osteophytic Impingement of the Popliteus Tendon After Total Knee Replacement.

We report our experience with 6 patients who had total knee replacements with lateral-sided knee pain, referred for ultrasound (US) assessment and US-guided injection. All cases showed an osteophyte within the popliteus sulcus of the lateral femoral condyle impinging on the adjacent tendon. Five of 6 patients reported improvement of symptoms immediately after US-guided injection of an anesthetic and a steroid. Ultrasound has a unique role in the imaging of knee replacements because of its real-time capabilities and absence of artifacts at the popliteus tendon origin.

Substantial vertebral body osteophytes protect against severe vertebral fractures in compression.

Recent findings suggest that vertebral osteophytes increase the resistance of the spine to compression. However, the role of vertebral osteophytes on the biomechanical response of the spine under fast dynamic compression, up to failure, is unclear. Seventeen human spine specimens composed of three vertebrae (from T5-T7 to T11-L1) and their surrounding soft tissues were harvested from nine cadavers, aged 77 to 92 years. Specimens were imaged using quantitative computer tomography (QCT) for medical observation, classification of the intervertebral disc degeneration (Thomson grade) and measuremen…

Postsurgical recurrence of osteophytes causing dysphagia in patients with diffuse idiopathic skeletal hyperostosis.

Although cervical anterior osteophytes accompanying diffuse idiopathic skeletal hyperostosis (DISH) are generally asymptomatic, large osteophytes sometimes cause swallowing disorders. Surgical resection of the osteophyte has been reported to be an effective treatment; however, little study has been given to the recurrences of osteophytes. A prospective study was performed for seven patients who underwent surgical resection of cervical anterior osteophytes for the treatment of recalcitrant dysphagia caused by osteophytes that accompanied DISH. The seven patients were six men and one woman rangi…

Case report: bilateral vocal cord paralysis caused by cervical spinal osteophytes.

Thyroid and cervical surgery, tracheal intubation, trauma and neurodegenerative and neuromuscular diseases may cause bilateral vocal cord paralysis. There are only a few reported cases of bilateral cord paralysis associated with cervical hyperostosis in the English literature. We report the MR and CT findings of a case of bilateral vocal cord paralysis caused by cervical spinal osteophytes compressing the recurrent laryngeal nerves.

Management of respiratory compromise caused by cervical osteophytes: a case report and review of the literature.

BACKGROUND CONTEXT: Diffuse idiopathic skeletal hyperostosis (DISH) is a common condition in the aging spine. DISH is associated with large anterior osteophytes of the cervical spine, which can cause complications by compressing adjoining structures. Dysphagia is reported in up to 28% of patients, but respiratory compromise is rare. There have been no published recommendations for treatment. PURPOSE: To report that resection of cervical osteophytes, without cervical fusion, can be successful in the treatment of severe respiratory distress. STUDY DESIGN: This report describes the management o…

Cervical osteophytes impinging on the pharynx: importance of size and concurrent disorders for development of aspiration.

OBJECTIVE: This study evaluated the clinical significance of cervical osteophytes impinging on the pharynx in patients with dysphagia and the importance of concurrent disorders that may affect swallowing function. MATERIALS AND METHODS: On videofluoroscopy, anterior cervical osteophytes were found in 55 (32 men, 23 women; mean age, 69 years) of 3318 patients with dysphagia (1.7%). Coexisting diseases that affected swallowing function were found in 28 patients (stroke, n = 7; thyroidectomy, n = 7; tongue base or laryngeal cancer surgery, n = 5; other diseases, n = 9). Swallowing function was a…

The relation between cartilage damage and osteophyte size in a murine model for osteoarthritis in the knee.

The aim of this study was to investigate the relationship between location and size of osteophytes and cartilage loss in an instability-induced experimental model for osteoarthritis. Osteoarthritis was induced in murine knee joints by injection of highly purified bacterial collagenase, causing joint instability. The size of the osteophytes and the cartilage loss were measured at different locations in the joint using image analysis on histological sections of total kees. Cartilage damage did not occur without osteophytes. Osteophytes were located on both medial and lateral sides, independent o…

Differential expression of collagen types I, II, III, and X in human osteophytes.

BACKGROUND: Osteophytes are neoplastic cartilaginous and osseous protrusions growing at the margins of osteoarthritic joints. Their formation involves complex patterns of cellular proliferation, differentiation, as well as matrix synthesis and turnover that are poorly understood. EXPERIMENTAL DESIGN: Here we report on an experimental approach using in situ hybridization and immunohistology to elucidate pathways of chrondrocyte differentiation in human osteophytes. Ab and cDNA probes for collagen types were used as specific parameters for chondrocyte phenotypes. RESULTS: In early precartilagi…

Bottom-to-top: A case report for treating esophageal cervical spondylosis with giant osteophytes.

RATIONALE: Esophageal cervical spondylosis is a rare condition characterized by dysphagia due to anterior cervical osteophyte compression. Existing surgical methods carry risks of esophageal injury and recurrence. This study introduces a novel "bottom-to-top" technique designed to safely resect large, adherent osteophytes while preserving esophageal integrity. PATIENT CONCERNS: A 67-year-old male presented with progressive dysphagia over one year, severely affecting food intake. His preoperative Eating Assessment Tool-10 score was 18, indicating significant swallowing impairment. DIAGNOSES: …

C1-2 Stenosis Caused by Posterior Osteophytes: An Unusual Cause of Cord Compression.

Cervical spinal stenosis most commonly occurs at the subaxial spine. C1-C2 stenosis is relatively unusual but can be present in certain congenital or syndromic conditions such as rheumatoid arthritis. In this manuscript, we highlight the case of a 42-year-old lady who presented with neck pain and signs of high cervical myelopathy. Imaging studies showed severe stenosis at C1-C2 caused by posterior osteophytes originating from the C1 arch and C2 lamina. Decompression and stabilization of the segment were performed. This case is unique as this pathology occurred at an uncommon location, leading …

A Case of Dysphagia Due to Large Osteophytic Lesions in the Cervical Spine: A Conservative Approach.

Dysphagia is a common symptom with various underlying etiologies, making its management challenging even for experienced physicians. The presence of osteophytes in the cervical spine may often impede swallowing, displace the larynx, and cause a sore throat. We describe a case of an 85-year-old male who presented with a two-year history of progressive dysphagia, exacerbated over the last two months, especially with solid foods and liquids, prompting an ENT evaluation. Despite prior investigations, including normal gastroscopy and empirical pain management, further assessment revealed bulging ma…

Spatial Distribution of Microcracks in Osteoarthritic Femoral Neck: Influence of Osteophytes on Microcrack Formation.

Osteophytes have been suggested to influence the bone mechanical properties. The aim of this study was to compare the microcrack density in osteophytes with that in the other parts of the osteoarthritic femoral neck (FN). The presence of microcracks was investigated in the ultra-distal FN and in the osteophytes in samples obtained during hip arthroplasty in 24 postmenopausal women aged 67 ± 10 years. Furthermore, the 3D microarchitecture and the collagen crosslinks contents were assessed by high-resolution peripheral quantitative computed tomography and high-performance liquid chromatography, …

Rotator cuff tear incidence association with critical shoulder angle and subacromial osteophytes.

BACKGROUND: The concomitant presence of a heel-type osteophyte may affect the critical shoulder angle (CSA) correlation with rotator cuff tears (RCT). METHODS: We retrospectively reviewed patients with and without a full-thickness RCT who underwent magnetic resonance imaging (MRI) and radiographic imaging of the shoulder. The patients were divided into 3 groups according to the CSA as high CSA group, >38°; middle CSA group, 33°-38°; and low CSA group, <33°. We confirmed the presence of heel-type osteophytes, quadrangular osteophytes protruding inferiorly from the undersurface of the anterolat…
