Momentary pain and coping in temporomandibular disorder pain: exploring mechanisms of cognitive behavioral treatment for chronic pain.

The purpose of this study was to determine whether cognitive-behavioral treatment (CBT) operates by effecting changes in cognitions, affects, and coping behaviors in the context of painful episodes. Patients were 54 men and women with temporomandibular dysfunction-related orofacial pain (TMD) enrolled in a study of brief (6 weeks) standard conservative treatment (STD) or standard treatment plus CBT (STD+CBT). Momentary affects, pain, and coping processes were recorded on a cell phone keypad four times per day for 7 days prior to treatment, and for 14 days after treatment had finished, in an ex…

The Role of Transitional Pain in Bridging Acute and Chronic Pain.

Transitional pain describes the clinically important phase in which acute pain resolves or progresses to chronic pain. This phase is particularly relevant after surgery and trauma, where the onset of tissue injury can be identified. Evidence suggests that chronic postsurgical or post-traumatic pain may arise through the interaction of biological, psychological, and social mechanisms, including peripheral and central sensitization, neuroimmune activation, psychological distress, pain catastrophizing, functional avoidance, and prolonged opioid exposure. Transitional Pain Services address this ri…

Debunking myths in facial pain diagnosis for the pain practitioner.

Facial pain disorders, most notoriously trigeminal neuralgia, cause substantial suffering, functional impairment, disability, and impaired quality of life. However, most pain medicine practitioners have had only limited education on the evaluation, diagnosis, and treatment of facial pain. Furthermore, this limited education often contains commonly held myths about facial pain diagnosis. These myths can result in misdiagnoses that lead to ineffective medications, interventions, or surgeries and prolonged pain. Therefore, the American Society of Regional Anesthesia and Pain Medicine's Headache a…

Pain and Analgesia in Newborns-Development of a Neonatal Pain Ladder.

Preventing and relieving pain in newborns represent humane and ethical imperatives. Unrelieved and repetitive pain can cause adverse short- and long-term consequences including neurodevelopmental impairment. Increased awareness and routine assessment of pain can help direct the use of nonpharmacologic interventions and/or analgesic and sedative drugs. A Pain and Analgesics In Newborns ladder is proposed to facilitate appropriate interventions commensurate with the severity of pain. Although neonatal pain assessment tools remain highly subjective and under scrutiny, pain intensity scores using …

Antidepressants to Manage Osteoarthritic Pain: The Value of Pain Phenotyping.

Osteoarthritis (OA) is a chronic condition in which pain significantly affects quality of life, often leading to reduced physical activity and disability. Globally, an estimated 595 million people are affected, with the numbers likely to increase owing to an aging population and rising obesity rates. Effective pain management is crucial, yet current treatments, such as nonsteroidal anti-inflammatory drugs (NSAIDs) and opioids, often provide limited relief and come with risks. One reason for this limited success is the insufficient recognition of the importance of psychosocial factors and heter…

Human pain neuroscience and the next generation of pain therapeutics.

The recent approval of suzetrigine for acute pain treatment highlights both the success of targeting peripheral sensory neurons for pain management and the potential of developing new pain therapies primarily in human-based systems. To realize this transformative potential, further research into somatosensation and pain neuroimmunology in human systems is essential.

Pain in the brain: Psychological correlates of chronic pain and fibromyalgia.

Fibromyalgia is a chronic disorder causing widespread pain, fatigue, and cognitive issues, linked to central nervous system dysfunction. This cross-sectional study compared the psychological profiles of women with fibromyalgia (N = 76) and women with non-fibromyalgic chronic pain (N = 73). Using cluster analysis, scale score comparisons using MANCOVA, and correlation difference tests, we examined whether these conditions present distinct psychological profiles in terms of touch avoidance, anxiety, depression, psychotic symptoms, self-criticism, pain acceptance, pain impact, and positive emotio…

Low back pain and osteoarthritis pain: a perspective of estrogen.

Low back pain (LBP) is the world's leading cause of disability and is increasing in prevalence more rapidly than any other pain condition. Intervertebral disc (IVD) degeneration and facet joint osteoarthritis (FJOA) are two common causes of LBP, and both occur more frequently in elderly women than in other populations. Moreover, osteoarthritis (OA) and OA pain, regardless of the joint, are experienced by up to twice as many women as men, and this difference is amplified during menopause. Changes in estrogen may be an important contributor to these pain states. Receptors for estrogen have been …

The pain conductor: brainstem modulation in acute and chronic pain.

PURPOSE OF REVIEW: It is well established in experimental settings that brainstem circuits powerfully modulate the multidimensional experience of pain. This review summarizes current understanding of the roles of brainstem nuclei in modulating the intensity of pain, and how these circuits might be recruited therapeutically for pain relief in chronic and palliative settings. RECENT FINDINGS: The development of ultra-high field magnetic resonance imaging and more robust statistical analyses has led to a more integrated understanding of brainstem function during pain. It is clear that a number o…

The Genesis of Pain in Osteoarthritis: Inflammation as a Mediator of Osteoarthritis Pain.

Chronic pain is a substantial personal and societal burden worldwide. Osteoarthritis (OA) is one of the leading causes of chronic pain and is increasing in prevalence in accordance with a global aging population. In addition to affecting patients' physical lives, chronic pain also adversely affects patients' mental wellbeing. However, there remain no pharmacologic interventions to slow down the progression of OA and pain-alleviating therapies are largely unsuccessful. The presence of low-level inflammation in OA has been recognized for many years as a major pathogenic driver of joint damage. I…

Persistent Idiopathic Dentoalveolar Pain: Is It a Central Pain Disorder?

The International Classification of Orofacial Pain (ICOP) describes idiopathic pain as "unilateral or bilateral intraoral or facial pain in the distribution(s) of one or more branches of the trigeminal nerve(s) for which the etiology is unknown. Pain is usually persistent, of moderate intensity, poorly localized and described as dull, pressing or of burning character." Several diagnoses are included in the ICOP Idiopathic pain section, burning mouth syndrome and persistent idiopathic facial and dentoalveolar pain. This article, with a representative case presentation, briefly discusses common …

Validation of the Pain Resilience Scale in a Chronic Pain Sample.

UNLABELLED: Psychosocial factors that protect against negative outcomes for individuals with chronic pain have received increased attention in recent years. Pain resilience, or the ability to maintain behavioral engagement and regulate emotions as well as cognitions despite prolonged or intense pain, is one such factor. A measure of pain-specific resilience, the Pain Resilience Scale, was previously identified as a better predictor of acute pain tolerance than general resilience. The present study sought to validate this measure in a chronic pain sample, while also furthering understanding of …

Characterizing the Pain Narratives of Parents of Youth With Chronic Pain.

OBJECTIVES: Questionnaire-based research has shown that parents exert a powerful influence on and are profoundly influenced by living with a child with chronic pain. Examination of parents' pain narratives through an observational lens offers an alternative approach to understanding the complexity of pediatric chronic pain; however, the narratives of parents of youth with chronic pain have been largely overlooked. The present study aimed to characterize the vulnerability-based and resilience-based aspects of the pain narratives of parents of youth with chronic pain. METHODS: Pain narratives o…
