Syringohydromyelia or HTLV-I-associated myelopathy/tropical spastic paraparesis. A diagnostic challenge. Case report.

Human T-cell lymphotropic virus type I (HTLV-I) associated myelopathy/tropical spastic paraparesis (HAM/TSP) is the most common chronic myelopathy in Brazil. We present the case of a 53 year old man that fulfilled the diagnostic criteria for HAM/TSP but had at the magnetic resonance imaging (MRI) of the spinal cord evidences of syringohydromyelia at the C6-C7 and D2-D7 levels along with Chiari type I malformation. The clinical picture was more typical of HAM/TSP than of syringohydromyelia, which was probably asymptomatic. The present case clearly demonstrates that serology and neuroimaging sho…

Sensory dysfunction in HTLV-I-associated myelopathy/tropical spastic paraparesis. A comprehensive neurophysiological study.

BACKGROUND: Direct lateral (transpsoas) lumbar interbody fusion (LLIF) reportedly achieves union by 1 year postoperatively, but how soon fusion occurs after these minimally invasive procedures is unclear. This study investigated LLIF fusion progression at 6 months and 1 year in a large-scale cohort using bone morphogenetic protein (BMP) graft and examined risk factors associated with failed fusion. METHODS: Patients undergoing primary LLIF with a single surgical team from 2015 through 2016 with polyetheretherketone (PEEK) iimplants and BMP graft were identified. Retrospective chart review inc

Virological aspects of tropical spastic paraparesis/HTLV-I associated myelopathy and HTLV-I infection.

TSP/HAM, a chronic spastic paraparesis or paraplegia with sphincter disturbances and minimal sensory loss, is characterized pathologically by a meningo-myelitis of the lower thoracic cord with axonal degeneration and demyelinisation of the lateral and anterior spinal tracts. High titer specific anti HTLV-I antibody is present in the serum and in the cerebrospinal fluid with specific intrathecal IgG synthesis, elevated IgG index and intra blood-brain barrier IgG synthesis. Most TSP/HAM patients also exhibit IgG oligoclonal bands in the CSF (also sometimes in the serum), some of those being HTLV…

Tropical spastic paraparesis occurring in HTLV-1 associated infective dermatitis. Report of two cases.

An association between HTLV-1 infection and infective dermatitis (ID) a relapsing eczematous condition of Jamaican children, was reported in 1990. These patients are at risk of developing other known HTLV-1 related diseases. We have observed the development of HTLV-1 associated myelopathy/tropical spastic paraparesis in two patients, ages 14 and 35 years, who were diagnosed with ID at ages 2 and 10 years, respectively. Infective dermatitis of children serves as an early marker of HTLV-I infection and may predict later development of either the malignant outcome, adult T-cell leukaemia/lymphoma…

Progressive spastic paraparesis associated with human T-cell leukemia virus type I (HTLV-I).

Patients with progressive spastic paraparesis (PSP), commonly middle-aged women, are distributed throughout the country of Chile. During the three years from 1987 to 1990, we collected 83 cases of PSP from among 225 patients with various neurological diseases. The clinical picture was of a bilateral pyramidal syndrome, with sensory deficits in only 15.5% of the cases, and a slow illness progression in the majority of them. In patients with PSP, antibody to human T-cell leukemia virus type I (HTLV-I) was analyzed by enzyme linked immunosorbent assay (ELISA) and confirmed by western blot analysi…

Two Novel Variants in PI4KA in a Family Presenting With Hereditary Spastic Paraparesis: A Case Report.

OBJECTIVES: To report novel biallelic PI4KA variants in a family presenting with pure hereditary spastic paraparesis. METHODS: Two affected sisters presented with unsolved hereditary spastic paraparesis and underwent clinical and imaging assessments. This was followed by short-read next-generation sequencing. RESULTS: Analysis of next-generation sequencing data uncovered compound heterozygous variants in PI4KA (NM_058004.4: c.[3883C>A];[5785A>C]; p.[(His1295Asn);(Thr1929Pro)]. Using ACMG guidelines, both variants were classified as likely pathogenic. DISCUSSION: Here, next-generation sequen…

Paraparesis after low dose administration of fluorescein for endoscopic resection of an encephalocele: a case report.

The main causes for cerebrospinal fluid (CSF) leaks are known to be traumatic, iatrogenic, neoplastic, a meningoencephalocele, congenital bone defects, and spontaneous. Off-label intrathecal administration of fluorescein is widely used to localize a CSF leak. Complications are rare and low dose administration is described to be safe. In this case report, we present a case of a patient, who showed a CSF leak due to an encephalocele. Low dose fluorescein was applied intrathecally via lumbar catheter, the CSF leaks could be identified, and multilayered closure was performed. Postoperatively, the

Novel c.C2254T (p.Q752*) mutation in ZFYVE26 (SPG15) gene in a patient with hereditary spastic paraparesis.

Hereditary spastic paraplegias are clinically and genetically heterogeneous degenerative disorders, and pathological variants in the autosomal recessive ZFYVE26 gene are considered as very rare causes. We describe a novel mutation in ZFYVE26 gene found in a patient with autosomal recessive spastic paraplegias. The use of a 'target-gene' approach allowed us to expand the clinical spectrum associated with hereditary spastic paraplegias.

ANIMAL TRAINING AND ACUPUNCTURE IN A BENGAL TIGER ( PANTHERA TIGRIS TIGRIS) WITH HIND LIMB PARAPARESIS.

An 18-yr-old, captive-born male Bengal tiger ( Panthera tigris tigris) presented a 1-yr history of chronic and progressive paraparesis in both hind limbs. Lateral and hind limb radiographs were revealed normal except for severe spondylosis deformans, forming a bony bridge between the last lumbar and the first sacral vertebra and disc mineralization between the second and third lumbar vertebra. Medical therapies were instituted, including corticosteroids, hydroacupuncture and electroacupuncture. Animal training allowed veterinarians to perform acupuncture safely without having to anesthetize th…

A Peruvian family with a high burden of HTLV-1-associated myelopathy/tropical spastic paraparesis.

Human T-lymphotropic virus 1 (HTLV-1) is frequent in Peru; an estimated 1-2% of the Peruvian population carry this retrovirus. HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) is a chronic disabling disease that affects about 1% of the carriers of HTLV-1. It is not yet known why some HTLV-1-infected people develop HAM/TSP while others do not. In this case report, we present a family with an unusually high burden of HAM/TSP: 5 (the 2 parents and 3 of their children) of 7 HTLV-1 carriers developed the same disease. We describe the clinical presentation and discuss the clusteri…

Intratumoral hemorrhage of spinal schwannoma of the cauda equina manifesting as acute paraparesis--case report.

A 64-year-old man presented with spinal schwannoma of the cauda equina at the thoraco-lumbar junction manifesting as acute paraparesis and urinary incontinence after intratumoral hemorrhage. Surgical resection of the tumor resulted in significant neurological recovery. T(2)(*)-weighted gradient-echo magnetic resonance imaging is very useful to help establish the preoperative diagnosis, leading to early surgical intervention and better outcome after surgery.

Surgical treatment of tuberculous paraparesis in the third trimester: a report of two cases and review of the literature.

BACKGROUND: Tuberculous paraplegia as a consequence of spinal infiltration in pregnancy is reported to be rare. Analysis of the current literature produces few case studies that report successful outcomes. Delay in diagnosis and treatment may ultimately result in an irreversible neurological deficit. The potential implications of progression are paraplegia and a significant associated morbidity to the fetus if delivered premature. METHODS: Two cases of spinal tuberculosis in pregnancy are reported with description of clinical presentation, neuroradiographic findings and treatment. RESULTS: B

Paraparesis in a patient with Crohn disease resulting from septic arthritis of the hip and psoas abscess.

A psoas abscess is a recognized complication of Crohn disease. Less commonly, septic arthritis has been described with this entity. The occurrence of both these complications together in Crohn disease is quite rare. A 56-year-old patient with Crohn disease presented with weakness and pain in both lower extremities. Computerized body tomograms demonstrated a large psoas abscess with fistulous connections to the bowel as well as extending into the capsule of the left hip joint. X-ray examination revealed evidence of acute septic arthritis. Electromyographic studies demonstrated lumbosacral plexu

Multimodal rehabilitation in PLP1-associated spastic paraparesis: a case report with clinical and biomechanical outcomes.

BACKGROUND: PLP1-related disorders, including the Pelizaeus-Merzbacher disease spectrum and X-linked spastic paraplegia type 2, are rare hypomyelinating conditions characterized by progressive gait impairment, lower limb spasticity, and motor disability. Evidence supporting rehabilitation strategies in these disorders remains limited, particularly for technology-assisted interventions. CASE PRESENTATION: We report the case of a 29-year-old man with progressive PLP1-related spastic paraparesis, prominent gait impairment, recurrent falls, lower limb spasticity, and preserved independence in act…

Early presentation of spastic paraparesis in individuals carrying PSEN1 mutations: a clinical and genetic analysis.

BACKGROUND: Mutations in the presenilin 1 gene (PSEN1) are well-known causes of early-onset familial Alzheimer's disease, but they can also present with atypical phenotypes such as pure spastic paraparesis. This study aims to investigate the clinical and genetic features of PSEN1 variants in patients mainly manifested with hereditary spastic paraparesis (HSP)-like phenotypes. METHODS: Mutational analysis was performed in 242 unrelated Taiwanese patients with clinically suspected HSP using a targeted resequencing panel covering the entire coding regions of PSEN1, along with 76 genes associated…

Acquisition of Childcare Skills by a Patient With Transient Paraparesis Following Epidural Anesthesia for Cesarean Section.

For first-time mothers, every aspect of childcare is a new experience, and acquiring these skills can be physically and mentally challenging. It is reasonable to assume that patients with disabilities, including those experiencing lower-limb paraparesis, may face even greater difficulty mastering the physical movements required for childcare while simultaneously striving to regain independence in daily living. We report the case of a 29-year-old woman who developed lower limb paraparesis following an elective caesarean section and describe her recovery in limb function, activities of daily liv…
