Investigating the relationship between childhood sexual abuse, self-harm repetition and suicidal intent: mixed-methods study.

BACKGROUND: Research into the association between childhood sexual abuse (CSA) and self-harm repetition is limited. AIMS: We aimed to examine the association between self-harm repetition, mental health conditions, suicidal intent and CSA experiences among people who frequently self-harm. METHOD: A mixed-methods study was conducted including consecutive patients aged ≥18 years, with five or more self-harm presentations, in three Irish hospitals. Information was extracted from psychiatric records and patients were invited to participate in a semi-structured interview. Data was collected and an

Emotion dysregulation as a mechanism linking child maltreatment exposure and self-harm behaviors in adolescents.

Although child maltreatment exposure is a recognized risk factor for self-harm, mechanisms underlying this relationship remain unclear. Self-harm may function as a compensatory strategy to regulate distressing emotions. This cross-sectional study examines if emotion dysregulation mediates between the severity of maltreatment exposure and self-harm, adjusting for demographic variables and depressive symptoms. Participants were 108 adolescent patients recruited from a psychiatric hospital in Singapore (mean age 17.0 years, SD=1.65; 59.3% female). Study measures included the Childhood Trauma Ques

Australian registered nurse medical device education: a comparison of simple vs. complex devices.

As the primary users of medical devices in direct patient care, registered nurses must be competent both with simple and complex devices bec … urethral catheter, but were more likely to have used an indwelling urethral catheter that caused a patient harm than an intravenous infusion pump that caused a patient harm. The most frequently identified reasons for incidents of patient harm resulting from use of an intravenous infusion pump were user error and inadequate device education. The most frequentl …

Defining conditions where long-term glucocorticoid treatment has an acceptably low level of harm to facilitate implementation of existing recommendations: viewpoints from an EULAR task force.

There is convincing evidence for the known and unambiguously accepted beneficial effects of glucocorticoids at low dosages. However, the implementation of existing recommendations and guidelines on the management of glucocorticoid therapy in rheumatic diseases is lagging behind. As a first step to improve implementation, we aimed at defining conditions under which long-term glucocorticoid therapy may have an acceptably low level of harm. A multidisciplinary European League Against Rheumatism task force group of experts including patients with rheumatic diseases was assembled. After a systemati

How are substance use disorder treatment providers integrating harm reduction approaches into practice?

Background Harm reduction prioritizes reducing substance use-related harms (e.g., overdose) without requiring abstinence or reducing substance use to receive services. Substance use disorder (SUD) treatment has historically operated on an abstinence-based model, but in recent years there have been efforts to integrate harm reduction approaches into SUD treatment. However, little research has examined its acceptance among individuals who provide treatment in specialty SUD care settings; this study seeks to understand providers' perceptions of harm reduction approaches including perceived and cu

A systematic review and mixed-methods synthesis of the experiences, perceptions and attitudes of prison staff regarding adult prisoners who self-harm.

BACKGROUND: Self-harm, including suicide, is common among prisoners. Staff attitudes and perceptions regarding self-harm may affect quality of care and patient safety. AIMS: To systematically review the experiences, perceptions and attitudes of staff in adult prisons regarding self-harm. METHOD: Systematic searches of EMBASE, Medline, PsycINFO and CINAHL databases were conducted, and supplemented by hand-searching and grey literature review, to identify relevant English-language articles published since the year 2000. Articles were screened by two authors and evaluated with standardised qual

Harm to Children from Prescribing and Administration Errors in Acute Care: A Multidisciplinary Panel Assessment.

INTRODUCTION: Medication errors continue to cause inpatient harm in children and can be difficult to both identify and classify. Medication error studies often focus on assessing potential harm and there is little published data on actual harm from medication errors in children. OBJECTIVE: Our aim was to use multidisciplinary panels to identify and describe the actual harm resulting from prescribing and administration medication errors occurring at a major paediatric hospital. METHODS: We reviewed medication error data collected from retrospective medication record reviews to identify prescr…

Incidence and characteristics of self-harm during the 3-year period of COVID-19-related social distancing in the Republic of Korea.

BACKGROUND: Suicide is a leading cause of mortality worldwide, and self-harm is a key risk factor. Continuous monitoring of self-harm trends facilitates effective prevention. During the coronavirus disease pandemic, social distancing significantly influenced self-harm incidence. This study aimed to compare self-harm rates and patient characteristics in 3-year prepandemic and pandemic periods (2017-2019 and 2020-2022, respectively). METHODS: We retrospectively analysed and subdivided the data over 6 years into 3-month intervals, obtained from the Emergency Department (ED)-based Injury In-depth

Feasibility and Acceptability of Restroom Motion Sensors for Detecting Overdoses.

BACKGROUND: Most US drug use events that lead to a fatal overdose are unwitnessed. Given the scale of the overdose crisis, interventions to help detect and respond to overdoses in community settings are urgently needed. Overdose detection technologies (ODT), including reverse-motion restroom sensors, are a promising set of interventions that are underutilized. This study explored the perspectives of frontline harm reduction workers, clinicians, housing shelter staff, and their clients on restroom sensors. METHODS: Staff and program client focus groups conducted between December 2022 and Janua

Incidents reported by nurse anaesthetists in the operating room.

The quality of health care and patient safety in the operating room is a major concern for nurse anaesthetists. However, few studies have fo … rses. Of all incidents only 23 (10%) had harmed patients or nurses. Few of these incidents involved patient harm (n = 6), while a greater number involved harm to nurses (n = 17). The findings reveal lack of communication and interprofessional teamwork as the two most common areas for the reported incidents, followed by problems related to lack of compliance wi …

Impulsivity and problem awareness predict therapy compliance and dropout from treatment for gambling disorder.

This study investigates the predictive value of impulsivity traits (as measured by the UPPS-P impulsive behaviour scale) and relevant covariates (sociodemographics, gambling severity, dysphoric mood, other potentially addictive behaviours, and non-verbal intelligence) with regard to treatment dropout and level of adherence to therapy guidelines and instructions in patients with gambling disorder. Sixty-six patients seeking treatment for gambling disorder, and recruited to participate in a larger protocol (G-Brain), were initially assessed in impulsivity traits and relevant covariates in the fi

Risk management of medication errors: a novel conceptual framework.

BACKGROUND: Medication error is a common cause of patient harm. The study aims to propose a way to manage the risk of medication errors in a novel way, by identifying practice areas where mitigating patient harm should be prioritized using a risk management approach. METHODS: Suspected Adverse Drug Reactions (sADRs) in Eudravigilance database over three years were reviewed to identify preventable medication errors. These were classified using a new method based upon the root cause underlying pharmacotherapeutic failure. The correlation between severity of harm and type of medication error, an…

Safe Patient Care in CKD: Core Curriculum 2025.

Quality and safety in health care have garnered scrutiny in recent decades. Complete prevention of unintended harm remains elusive. Patients with chronic kidney disease are at risk for adverse events because of the complexity of this illness and the impact of declining kidney function on choices for diagnostic studies and therapies. Nephrologists must be knowledgeable about patient safety principles and methods for evaluating harm or near harm events and be prepared to learn from investigations to make improvements. Medication safety is particularly challenging in the context of progressive ki

Death due to delirium: a case of a self-cut hemodialysis dialysis catheter - a case report.

BACKGROUND: Neuropsychiatric conditions such as depression, delirium and cognitive impairment are common in patients with end-stage kidney disease (ESKD) and individuals suffering from ESKD are more likely to commit suicide than members of the general population. Self-harm gestures are not infrequent for ESKD patients suffering from depression, but not well described in other conditions. CASE PRESENTATION: We present a case of self-harm in a patient with ESKD suffering from acute delirium. A man in his mid-seventies was admitted with fungal peritoneal dialysis (PD) associated peritonitis. On

Creating a Palliative Care Clinic for Patients with Cancer Pain and Substance Use Disorder.

BACKGROUND: Opioids are a first-line treatment for severe cancer pain. However, clinicians may be reluctant to prescribe opioids for patients with concurrent substance use disorders (SUD) or clinical concerns about non-prescribed substance use. MEASURES: Patient volume, 60-day retention rate, and use of sublingual buprenorphine to treat opioid use disorder. INTERVENTION: We created the Palliative Harm Reduction and Resiliency Clinic, a palliative care clinic founded on harm reduction principles and including formal collaboration with addiction psychiatry. OUTCOMES: During the first 18 month

Incident reporting by acute pain service at a tertiary care university hospital.

BACKGROUND AND AIMS: Provision of effective and safe postoperative pain management is the principal responsibility of acute pain services (APSs). Continuous quality assurance is essential for high-quality patient care. We initiated anonymous reporting of critical incidents by APS to ensure continuous quality improvement and here present prospectively collected data on the reported incidents. Our objective was to analyze the frequency and nature of incidents and to see if any harm was caused to patients. MATERIAL AND METHODS: Data were collected from January 1, 2012 to September 30, 2013. Borne of necessity: Pharmacy-based harm reduction and express sexually transmitted infection services.

BACKGROUND: HIV, hepatitis C virus (HCV), Sexually Transmitted Infections (STIs), and substance use disorder are interrelated epidemics. Augmented services to respond to this "syndemic" are hampered by shortages of health care workers, especially in rural areas. In an Indian Health Service hospital in rural Minnesota, the pharmacy sought to integrate harm reduction and express STI services into its scope of practice. OBJECTIVES: Provide pharmacy-based harm reduction and express STI services to increase access to care for community members, especially those without a primary care provider. ME
