Challenges And Opportunities For Improving Patient Safety Through Human Factors And Systems Engineering.

Despite progress on patient safety since the publication of the Institute of Medicine's 1999 report, To Err Is Human, significant problems remain. Human factors and systems engineering (HF/SE) has been increasingly recognized and advocated for its value in understanding, improving, and redesigning processes for safer care, especially for complex interacting sociotechnical systems. However, broad awareness of HF/SE and its adoption into safety improvement work have been frustratingly slow. We provide an overview of HF/SE, its demonstrated value to a wide range of patient safety problems (in par

Interventions to Improve Patient Safety During Intubation in the Neonatal Intensive Care Unit.

OBJECTIVE: To improve patient safety in our NICU by decreasing the incidence of intubation-associated adverse events (AEs). METHODS: We sequentially implemented and tested 3 interventions: standardized checklist for intubation, premedication algorithm, and computerized provider order entry set for intubation. We compared baseline data collected over 10 months (period 1) with data collected over a 10-month intervention and sustainment period (period 2). Outcomes were the percentage of intubations containing any prospectively defined AE and intubations with bradycardia or hypoxemia. We followed

Association between insurance status and patient safety in the lumbar spine fusion population.

BACKGROUND CONTEXT: Lumbar fusion is a common and costly procedure in the United States. Reimbursement for surgical procedures is increasingly tied to care quality and patient safety as part of value-based reimbursement programs. The incidence of adverse quality events among lumbar fusion patients is unknown using the definition of care quality (patient safety indicators [PSI]) used by the Centers for Medicare and Medicaid Services (CMS). The association between insurance status and the incidence of PSI is similarly unknown in lumbar fusion patients. PURPOSE: This study sought to determine th

Transesophageal echocardiographic screening before atrial flutter ablation: is it necessary for patient safety?

BACKGROUND: Transesophageal echocardiography (TEE) is commonly used before atrial flutter (AFl) ablation to detect atrial thrombus (AT) and thereby identify a heightened risk for systemic embolism both in patients with their initial episodes of AFl and in those with prior episodes whose anticoagulation has been inadequate. This treatment strategy has been extrapolated from guidelines for atrial fibrillation. In fact, limited data exist regarding the prevalence or clinical associations of AT and spontaneous echocardiographic contrast (SEC) in patients with AFl. Both AT and SEC are believed to r

Patient safety in the rehabilitation of the adult with a spinal cord injury.

There are approximately 12,000 new cases of traumatic spinal cord injury (SCI) annually. In 2010, there were approximately 265,000 individuals living with SCI. Over time, the average age of people with SCI has steadily risen, and it is now 40.7 years. There are multiple medical complications that are commonly seen in individuals with SCI. These include, but are not exclusively limited to, pneumonia, decubiti ulcers, undiagnosed fractures, urinary tract infections, autonomic dysreflexia, deep venous thrombosis, and pulmonary embolism. This article addresses the issue of patient safety in the ca

How valid is the AHRQ Patient Safety Indicator "postoperative physiologic and metabolic derangement"?

BACKGROUND: The Agency for Healthcare Research and Quality Patient Safety Indicator postoperative physiologic and metabolic derangement (PMD) uses ICD-9-CM codes to screen for potentially preventable acute kidney injury (AKI) requiring dialysis plus diabetes-related complications after elective surgery. Data on PMD's accuracy in identifying true events are limited. We examined the indicator's positive predictive value (PPV) in the Veterans Health Administration (VA). STUDY DESIGN: Trained abstractors reviewed medical records of 119 PSI software-flagged PMD cases. We calculated PPVs overall an

Safety of repeated courses of electroconvulsive therapy in a patient with Harrington rods.

BACKGROUND: Although modified electroconvulsive therapy (ECT) may rarely result in musculoskeletal complications, it has also been safely administered to patients with orthopedic morbidity. There is no literature on the safety of repeated courses of ECT in patients with Harrington rod implants. CASE REPORT: We describe a 30-year-old asthenic bipolar woman who, in 1997, had a Harrington compression assembly stabilizer fitted for spinal tuberculosis and who, between 2000 and 2004, received 3 courses of modified ECT with a total of 28 treatments. In October 2006, she experienced her index episod

The role and safety of early postoperative feeding in the pediatric surgical patient.

The authors report their experience with early postoperative feedings in a variety of pediatric surgical patients utilizing a needle catheter jejunostomy. A small caliber catheter, similar to that used for antecubital central venous cannulation, is inserted in the antimesenteric border of the jejunum providing a subserosal tunnel. A purse-string suture is placed around the catheter and then it is secured to the abdominal wall, and the catheter is brought out through the abdominal wall via a needle puncture wound. Our experience with 27 insertions in 25 patients ranging in age from 1 day to 17

Financial Waste is Patient Harm: Reducing Wasteful Healthcare Spending Increases Safety and Health.

Wasteful healthcare spending has more than economic costs-there are clinical costs as well. Waste increases out-of-pocket costs, delays necessary care, and worsens outcomes. We argue that, while most quality frameworks do not consider this patient harm, they should. Financial waste is clinically on par with medical errors and adverse events, as overtreatment and low-value care injure patients, care coordination failures lead to preventable complications and death, pricing failures cause delayed and complicated care, and low-value procedures cause complications. We reviewed the literature to ti

Medical device errors in intensive care units and patient safety: a systematic review.

INTRODUCTION: Medical device errors in Intensive Care Units (ICUs) position a critical threat to patient safety due to the complexity of devices, high patient alertness, and demanding clinical situations. Despite rising support on medical technologies, evidence on the nature of causes and prevention of device-oriented errors in ICUs remains fragmented. METHODS: The PRISMA guidelines were followed throughout the systematic review, and the review was registered in PROSPERO (CRD42023424716). Studies published between 2010 and 2025 were accounted through 17 databases. The studies on medical devic

MoTRANet: Safety-Aware Drug Recommendation via Molecular Representation Learning and Dynamic Patient Modeling.

With the rapid development of artificial intelligence in healthcare, medication recommendation systems have become important tools for achieving personalized treatment. However, existing methods still have limitations in modeling drug molecular functions and perceiving dynamic changes in patient conditions. To address these issues, this paper proposes MoTRANet, a safety-aware combinatorial medication recommendation model. The model designs a dynamic patient modeling module that integrates GRU, Transformer, and diagnosis-procedure cross attention to effectively capture the dynamic evolution of

Patient safety incidents in childbirth: analysis of reported cases from 2015 to 2025.

BACKGROUND: Patient safety incidents occur globally, with one in ten hospitalized patients in high-income countries experiencing an adverse event, most preventable. In obstetrics, harm is rare but can have severe consequences, and research highlights human and system errors as key contributors. Incident reporting is essential for identifying risks, yet studies focusing on childbirth-related reports remain limited. AIM: This study aimed to describe the incidents related to childbirth at one birth unit to gain important information about the obstetric safety in Finland. METHODS: A retrospectiv

Transforming caregivers into partners: advancing WHO patient safety goals in Singapore acute hospital.

Engaging caregivers as active partners in care is a cornerstone of Strategic Objective 4 in the WHO's Global Patient Safety Action Plan 2021-2030. Despite its recognised importance, caregiver integration in acute care remains inconsistent, with significant gaps in registration processes, role clarity and institutional support. At Singapore General Hospital (SGH), a 1900-bed academic medical centre, caregiver involvement was previously informal and varied across clinical settings. This hospital-wide quality improvement initiative aimed to improve the caregiver-to-admission ratio from 1:6 to 1:4

Risk Management and Patient Safety in the Artificial Intelligence Era: A Systematic Review.

BACKGROUND: Healthcare systems represent complex organizations within which multiple factors (physical environment, human factor, technological devices, quality of care) interconnect to form a dense network whose imbalance is potentially able to compromise patient safety. In this scenario, the need for hospitals to expand reactive and proactive clinical risk management programs is easily understood, and artificial intelligence fits well in this context. This systematic review aims to investigate the state of the art regarding the impact of AI on clinical risk management processes. To simplify
