The Silent Surge: A Case of Rhabdomyolysis-Induced Hypercalcemia.

BACKGROUND Calcium is an essential extracellular cation that the body tightly regulates for numerous physiological functions. Rhabdomyolysis typically causes hypocalcemia due to calcium sequestration within damaged muscle tissue. However, in rare cases, it can progress to hypercalcemia, particularly during the recovery phase, often in patients with concurrent acute kidney injury. CASE REPORT A 48-year-old man with no prior medical or surgical history was admitted to the hospital after he had been found unresponsive at home. His initial toxicology screen revealed the presence of cocaine, fentan

Sensory neuron-associated macrophages as novel modulators of neuropathic pain.

The peripheral nervous system comprises an infinity of neural networks that act in the communication between the central nervous system and the most diverse tissues of the body. Along with the extension of the primary sensory neurons (axons and cell bodies), a population of resident macrophages has been described. These newly called sensory neuron-associated macrophages (sNAMs) seem to play an essential role in physiological and pathophysiological processes, including infection, autoimmunity, nerve degeneration/regeneration, and chronic neuropathic pain. After different types of peripheral ner

60 YEARS OF NEUROENDOCRINOLOGY: The hypothalamo-GH axis: the past 60 years.

At the time of the publication of Geoffrey Harris's monograph on 'Neural control of the pituitary gland' 60 years ago, the pituitary was recognised to produce a growth factor, and extracts administered to children with hypopituitarism could accelerate growth. Since then our understanding of the neuroendocrinology of the GH axis has included identification of the key central components of the GH axis: GH-releasing hormone and somatostatin (SST) in the 1970s and 1980s and ghrelin in the 1990s. Characterisation of the physiological control of the axis was significantly advanced by frequent blood

PPAR-γ activation by Tityus serrulatus venom regulates lipid body formation and lipid mediator production.

Tityus serrulatus venom (TsV) consists of numerous peptides with different physiological and pharmacological activities. Studies have shown that scorpion venom increases pro-inflammatory cytokine production, contributing to immunological imbalance, multiple organ dysfunction, and patient death. We have previously demonstrated that TsV is a venom-associated molecular pattern (VAMP) recognized by TLRs inducing intense inflammatory reaction through the production of pro-inflammatory cytokines and arachidonic acid-derived lipid mediators prostaglandin (PG)E2 and leukotriene (LT)B4. Lipid bodies (L

Diffuse panbronchiolitis in China.

OBJECTIVE: Diffuse panbronchiolitis (DPB) is a progressive inflammatory disease, well recognized in Japan, that is characterized by chronic sinusitis and obstructive small airway disease. A total of 40 patients with DPB in mainland China were reviewed in order to describe the epidemiology of DPB in China and to compare their clinical characteristics with those of cases reported in Japan. METHODOLOGY: A systematic search was conducted of the Chinese language literature published in mainland China from 1996 to 2003. The 40 subjects who fulfilled the diagnostic criteria for DPB were retrospectiv

Imaging findings of foot and ankle injuries in dancers.

Dancers are performing artists and elite athletes with unique physiological and psychological demands. Many dance styles, especially classical ballet, require extraordinary flexibility and strength throughout the foot and ankle with remarkable pain tolerance. Injuries arising from biomechanical and metabolic stresses negatively impact career progression with resultant chronic pain, permanently impaired function, and financial loss. Therefore, dancers rarely seek medical attention except for genuine problems. The unique demands of dance can result in specific injuries and distinctive radiologic

Anti-DAMP therapies for acute inflammation.

Shock, affecting a third of intensive care patients, remains a highly fatal condition despite advances in critical care, irrespective of its etiology. Cellular injury, central to shock pathophysiology, triggers the release of damage-associated molecular patterns (DAMPs), such as extracellular cold-inducible RNA-binding protein (eCIRP), high-mobility group box 1 (HMGB1), histones 3 and 4, and adenosine triphosphate (ATP). These molecules are confined within cells under normal conditions and perform essential physiological functions. However, upon their extracellular release during cellular inju

Pseudocholinesterase Deficiency in a Patient Undergoing Electroconvulsive Therapy: A Case Report.

Pseudocholinesterase deficiency (PD) is a genetic or acquired condition that impairs the metabolism of succinylcholine, leading to unpredictable and potentially prolonged paralysis. Its recognition during electroconvulsive therapy (ECT) is particularly difficult because postictal physiology, psychotropic polypharmacy, and the frequent absence of quantitative neuromuscular monitoring may make residual paralysis difficult to detect. We report a 34-year-old woman with pharmacotherapy-resistant schizophrenia who developed consistently prolonged paralysis after succinylcholine during ECT. In the fi

Apnea detection using wrist actigraphy in patients with heterogeneous sleep disorders.

Obstructive sleep apnea (OSA) and related hypoxia are well-established cardiovascular and neurocognitive risk factors. Current multi-sensor diagnostic approaches are intrusive and prone to misdiagnosis when simplified. This study introduces an enhanced single-sensor-based OSA screening method, leveraging novel signal processing and machine learning to ensure accurate detection across diverse populations. Wrist actigraphy is a widely-used and energy-efficient tool for respiratory rate estimation. The main challenge in OSA pattern recognition is handling various disturbances in real-world applic

Novel role for conceptus signals in mRNA expression regulation by DNA methylation in porcine endometrium during early pregnancy†.

During early pregnancy, porcine conceptuses (the embryos with associated membranes) secrete estradiol-17β (E2)-their major signal for maternal recognition of pregnancy-and prostaglandin E2 (PGE2). Both hormones induce prominent changes of the endometrial transcriptome in vivo. Studies on endometrial pathologies have shown that E2 affects gene expression by epigenetic mechanisms related to DNA methylation. Herein, we determined the effects of E2 and PGE2 alone, and a combined E2 + PGE2 treatment administered into the uterine lumen in vivo on the expression and activity of DNA-methyltransferases

The clinical anatomy of the conal artery.

Coronary arteries have been extensively described and recognized by gross anatomic studies. However, in the clinical setting, the recognition of the conal artery is essential during coronary angiography, as well as certain congenital heart conditions such as tetralogy of Fallot. In order to provide a complete anatomic and physiologic correlation of the actual incidence and distribution of the conal artery we examined 300 formalin fixed hearts with gross dissections and 300 coronary angiograms. The conal artery was identified in all hearts examined and five main patterns were recognized. In Typ

Inhaled pattern recognition receptor agonists promote inducible epithelial resistance against murine betacoronavirus disease.

Coronaviruses can cause serious disease in humans and animals. They are often difficult to study due to the variability of the disease as well as safety concerns related to biosafety requirements. Here, we use a biological safety level (BSL) 2-compatible murine betacoronavirus (MHV-A59), which infects multiple organs, to study the respiratory phase of disease and subsequent systemic dissemination. We asked whether inducible epithelial resistance, activated by the combination of pattern recognition receptor agonists Pam2CSK4 (Pam2), synthetic diacylated lipopeptide, + oligodeoxynucleotide (ODN)

Floating fibula with associated multiligament knee injury: a case report and literature review.

BACKGROUND: A "floating fibula" is a rare and severe injury pattern characterized by complete disruption of both proximal and distal tibiofibular attachments. Its association with a multiligamentous knee injury (MLKI) has not been previously documented. METHODS AND RESULTS: We report the case of a 27-year-old male who sustained high-energy trauma in a motorcycle accident, resulting in a complex MLKI with anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial collateral ligament (MCL), and posterolateral corner (PLC) injuries, associated with complete disruption of the pro

The body before the dance somatic codification, elemental self-regulation, and contemplative recognition in the Naṭarāja tradition.

This article introduces the Somatic Codification Hypothesis (SCH), a theoretical framework that models the karaṇa system of the Nāṭyaśāstra-the foundational Sanskrit treatise on performance-as a systematic codification of pre-reflective, stimulus-driven bodily responses, alongside, rather than in place of, its established expressive, devotional, and aesthetic readings. The claim advanced here is epistemic and functional, not ontological: I do not argue that the body generates culture, nor that historical practitioners worked outward from physiology to notation. Rather, I argue that treating th

Spectrum of Darkness, Agent of Light: Myopia, Keratoconus, Ocular Surface Disease, and Evidence for a Profoundly Vitamin D-dependent Eye.

Serial observations obtained over more than eight years and 10,000 patient encounters in a general ophthalmology practice serving a population highly prone to chronic vitamin D (D3) deficiency, facilitated by the Oculus Pentacam Scheimpflug imaging system (Oculus Optikgeräte GmbH, Wetzlar, Germany), resulted in the recognition of consistent, predictable, and highly reproducible patterns of mechanical, optical, and physiologic change in the cornea and other ocular structures correlated to adequate vs. inadequate vitamin D availability. These patterns were identified from an analysis of more tha

Imaging and Non-Imaging Approaches for the Diagnosis and Monitoring of Necrotizing Enterocolitis-What Lies Ahead?

Necrotizing enterocolitis (NEC) remains one of the most serious gastrointestinal emergencies in preterm infants, and imaging plays a central role in diagnosis and clinical management. Historically, evaluation has relied primarily on abdominal radiography, which remains widely available and embedded in established diagnostic frameworks. However, the hallmark radiographic signs of NEC (i.e., pneumatosis intestinalis, portal venous gas, and free air) reflect relatively advanced manifestations of intestinal injury that indicate established mucosal disruption or transmural necrosis. [Recognition and classification of posture and gait patterns of rollator users by distance measurements-a comparison between clinical assessment and automatic classification].

BACKGROUND: This article describes the development of an add-on module for wheeled walkers dedicated to sensor-based posture and gait pattern recognition with the goal to develop an everyday aid for fall prevention. The core contribution is a clinical study that compared single gait parameter assessments coming from medical staff to those obtained from an automatic classification algorithm, i. e. the Mahalanobis distance over time series of sensor measurements. METHODS: The walker-module described here extends an off-the-shelf wheeled walker by two depth cameras that observe the torso, pelvic

A 1D Snoring Waveform and 2D Composite Acoustic Feature Graph-Based Multi-Modal Fusion Network for Obstructive Sites Recognition.

As a critical factor in diagnostic work-up and treatment decision-making process of sleep-related breathing disorders, accurate localization of obstructive sites in the upper airway is in dire need. Snoring, as a dynamic acoustic signal, carries informative information relating to the sites and degree of obstruction in the upper airway, offering a non-invasive, cost-effective solution for obstructive sites recognition. However, most of existing snoring-based methods for recognizing obstructive sites only involve limited information (either mainly concentrated on traditional acoustic characteri

Biosignal-based transferable attention Bi-ConvGRU deep network for hand-gesture recognition towards online upper-limb prosthesis control.

BACKGROUND AND OBJECTIVE: Upper-limb amputation can significantly affect a person's capabilities with a dramatic impact on their quality of life. As a biological signal, surface electromyogram (sEMG) provides a non-invasive means to measure underlying muscle activation patterns, corresponding to specific hand gestures. This project aims to develop a real-time deep learning based recognition model to automatically and reliably recognise these complex signals of a wide range of daily hand gestures from amputees and non-amputees. METHODS: This paper proposes an attention bidirectional Convolutio
