Paravaginal wolffian duct (mesonephros) adenocarcinoma: a light and electron microscopic study.

This is a report of an adenocarcinoma of mesonephric origin studied by light microscopy, electron microscopy, and immunocytochemistry. Unlike previous reports, our lesion was located paravaginally and not in the leaves of the broad ligament or in the cervix. The light microscopic features are similar to those of previous cases in the literature. Although not specific, the ultrastructural features of the tumor are similar to those of mesonephric structures and different from those of müllerian structures. The diagnosis always should be considered when a tumor occurs at the site where mesonephri

Colonic Angiosarcoma 16 Years After Pelvic Radiotherapy for Cervical Cancer: A Case Report.

Angiosarcomas are rare and aggressive malignant neoplasms of vascular or lymphatic endothelial origin, accounting for approximately 2% of soft-tissue sarcomas. They occur predominantly in the skin and superficial soft tissues, particularly in the head and neck region and the breast, whereas gastrointestinal involvement is exceptionally rare. Previous radiotherapy is a well-established risk factor for secondary angiosarcoma. However, radiation-associated gastrointestinal angiosarcomas are extremely uncommon and have been reported predominantly in the small bowel. We report the case of a 55-year

Requirement for novel surgical and clinical protocols for the treatment of desmoplastic small round cell tumor: A report of two cases and a review of the literature.

Desmoplastic small round cell tumor (DSRCT) is a rare and highly aggressive neoplasm that was initially described in 1989. DSRCT predominantly affects young men and typically occurs in the intra-abdominal area. The present study describes the cases of two patients with DSRCT. The first patient was a 23-year-old male who presented with abdominal pain in the right flank, coupled with difficulty urinating and bowel dysfunction. The second patient was 12-year-old female who presented with abdominal pain, emesis and loss of appetite. A computed tomography scan of the abdomen revealed the presence o

Preoperative diagnosis of ovarian malignancy: preliminary results of the use of 3-dimensional vascular ultrasound.

OBJECTIVE: To estimate the accuracy of 3-dimensional (3D) vascular ultrasound in the preoperative assessment of pelvic masses for malignancy. METHODS: We performed a chart review of 66 patients who underwent 3D ultrasound for pelvic masses. Each mass was preoperatively judged to be benign or malignant based upon a study of vascularity within an ovarian mass using 3D ultrasound. Masses with orderly vascular architecture were categorized as probably benign, and masses with chaotic vascular patterns were categorized as malignant. The predictive value of 3D vascular ultrasound was compared with t

Primary Penile Squamous Cell Cancer-Related Malignant Priapism in a Cystectomized Patient: A Case Report.

Priapism is one of the most common urologic emergencies and is characterized by a prolonged and painful erectile state unrelated to sexual stimulation or sexual desire. Neoplasm-associated priapism is a rare condition and is usually caused by corporeal metastases of other pelvic area malignancies. Primary penile malignancy-related malignant priapism is extremely rare. In this reported case, an 82-year-old male presented with priapism. The penile doppler ultrasound and pelvic magnetic resonance imaging were compatible with ischemic priapism and corporal mass. Subsequently, the patient underwent

Adenoid cystic carcinoma cytology: Salivary gland and nonsalivary gland.

OBJECTIVES: Adenoid cystic carcinoma (ACCA) is an uncommon primary malignancy of salivary glands and rarely nonsalivary tissue. This study aims to evaluate the diagnostic accuracy of ACCA on fine needle aspiration (FNA) material and the associated challenges. METHODS: A search on electronic pathology database from 2006 to 2016 at The Johns Hopkins Hospital found 83 cytology specimens diagnosed as ACCA, 49 with histology follow-up. RESULTS: Fifty-two females and 31 males were found ranging from 37 to 95 years old (mean 62.5). The tumor size was 1 to 11.5 cm (mean 3.4). FNAs were performed on

Hydrosalpinx in patients with hysterectomy without salpingo-oophorectomy referred for pelvic magnetic resonance imaging.

OBJECTIVE: Women with ovarian-sparing hysterectomy before 2007 are more likely to have retained Fallopian tube remnants which can become fluid-filled, distended masses, potentially mistaken for a cystic adnexal neoplasm on imaging. Here we assess the prevalence and appearance of hydrosalpinx in women with ovarian-sparing hysterectomy prior to 2007 referred for pelvic MRI. METHODS: A total of 3044 consecutive pelvic MRI exams performed over a two-year period (2003-2004) were selected from our radiology database and retrospectively reviewed. Examinations performed on male patients (N = 858), du

Beyond the Bones: Primary Prostatic Synovial Sarcoma in a Young Adult.

Synovial sarcoma is a very rare, highly malignant mesenchymal tumor and predominantly affects adolescents and young adults. This neoplasm tends to develop near the large joints in the extremities. The incidence of this type of neoplasm involving the prostate gland is extremely rare. Prostate sarcoma can often be misdiagnosed as a consequence of its infrequent occurrence. This is a case presentation of a 37-year-old man who had complained of a weak urinary stream for three weeks. Digital rectal examination showed an enlarged, hard, and nodular prostate. On imaging, there was a huge heterogeneou

Fallopian Tube Hemangioma Discovered on Follow-up for Uterine Leiomyoma.

Hemangioma in female reproductive organs, particularly in the fallopian tube (FT), is a sporadic disease. In this report, we describe a case of hidden capillary hemangioma in FT in a 39-year-old woman who suffered from uterine leiomyoma. During the preoperative stage, pelvic sonography, computed tomography, and diagnostic laparoscopy revealed a subserous leiomyomatous nodule located along the posterior wall of the uterus. Despite this, intraoperatively, a benign vascular neoplasm was diagnosed. Histologically, it is characterized by multiple thin-walled vascular spaces lined with a single laye

Whole-Genome and Segmental Homozygosity Confirm Errors in Meiosis as Etiology of Struma Ovarii.

Strumae ovarii are neoplasms composed of normal-appearing thyroid tissue that occur within the ovary and rarely spread to extraovarian sites. A unique case of struma ovarii with widespread dissemination detected 48 years after removal of a pelvic dermoid provided the opportunity to reexamine the molecular nature of this form of neoplasm. One tumor, from the heart, consisting of benign thyroid tissue was found to have whole-genome homozygosity. Another tumor from the right mandible composed of malignant-appearing thyroid tissue showed whole-genome homozygosity and a deletion of 7p, presumably t

Paratesticular cystadenomas with ovarian stroma, metaplastic serous Müllerian epithelium, and male adnexal tumor of probable wolffian origin: A series of 5 hitherto poorly recognized testicular tumors.

We present 5 paratesticular tumors, which manifested ovarian-type stroma and various serous müllerian epithelial structures including serous fallopian-like epithelium and proliferations closely mimicking cystic serous borderline tumors of the ovary. In addition, 3 of the tumors in our series revealed a solid epithelial component, which was morphologically and immunohistochemically similar to so called "female adnexal tumor of probable wolffian origin," which is a rare neoplasm described so far only in the female genital tract, retroperitoneum, and the pelvic cavity. In analogy with mixed epith

Cystic mesothelioma of the peritoneum.

Cystic mesothelioma (CM) of the peritoneum is a rare, benign neoplasm that occurs predominantly in women and tends to recur locally. It has received little attention (to our knowledge, a single case report) in the radiology literature. Five cases of CM are presented. Computed tomography (CT) was performed in three cases, ultrasound (US) in four cases, and magnetic resonance (MR) imaging in one case. Twenty-eight cases reported in the literature are reviewed for comparison. CM shows a clear predilection for the surfaces of the pelvic viscera but is seen in other areas of the peritoneum and retr

Overuse Injuries of the Hip and Pelvis: Apophysitis, Stress Fractures, and Tendinopathies.

Hip and pelvic overuse injuries are a frequent source of pain and lost playing time in athletes and may be challenging to diagnose because of overlapping symptoms and complex regional anatomy. Injury patterns vary according to skeletal maturity: in adolescents, open apophyses are particularly susceptible to traction-related apophysitis and avulsion injuries, whereas in skeletally mature individuals repetitive loading more often results in stress fractures and chronic tendinopathies. Clinical findings are often nonspecific, and misinterpretation may delay appropriate management or safe return t

Retroperitoneal solitary fibrous tumour arising from the pelvis in women--a case report and review of literature.

A solitary fibrous tumour is an unusual spindle cell neoplasm that most frequently occurs in the pleura based intrathoracic region. In recent years attention has been drawn towards solitary fibrous tumours arising in extrathoracic sites. They are usually benign but malignant solitary fibrous tumours have also been reported (Nielson et al. 1997). There is far less information about the clinical behaviour of an extra thoracic solitary fibrous tumour unlike intrathoracic tumours which is well reported in many case series (England et al. 1989). Although solitary fibrous tumours are well described

Incidental Detection of Ovarian Thecoma by 99mTc-MDP Whole-Body Bone Scan: Contribution of Hybrid SPECT/CT.

Ovarian thecoma is a benign and rare neoplasm that accounts for 0.5% to 1% of all ovarian tumors. A 55-year-old woman with known breast cancer underwent a metastatic workup before surgery. The Tc-MDP whole-body bone scan revealed intense uptake in the left pelvic region. Hybrid SPECT/CT imaging showed that the elevated Tc-MDP activity was in a tumor with calcification in the left adnexa. Dynamic enhanced MRI revealed marked enhancement of the tumor. Resection of the tumor was subsequently performed, and pathologic analysis confirmed the diagnosis of an ovarian thecoma.

Various sonographic appearances of the hemorrhagic corpus luteum cyst.

Although hemorrhagic corpus luteum cysts are frequently seen during sonography of the female pelvis, their diagnosis is often challenging as a result of variations in size, thickness of the cyst wall, and internal echo pattern depending on the formation and lysis of the clot. There are cases in which hemoperitoneum is the most obvious finding. The differential diagnosis is extensive and includes ectopic pregnancy, adnexal torsion, neoplasm, and pelvic inflammatory disease. This review describes and illustrates the diverse appearances of the hemorrhagic corpus luteum, as well as other etiologie

Medium-term Ovarian Volume and Anti-Müllerian Hormone After Ovarian-sparing Surgery for Benign Neoplasms in Pediatric Patients.

BACKGROUND AND AIMS: Benign ovarian neoplasms are common in the pediatric population. In young adult women, oophorectomy has been shown to negatively impact long-term ovarian endocrine function. Recently, ovarian-sparing surgery (OSS) has been proposed as it offers similar results to oophorectomy in terms of recurrence rates. However, the impact of OSS on functional preservation in girls and adolescents remains unclear due to limited follow-up data. The aim of this study was to evaluate ovarian function and its morphological correlation in the medium term in girls undergoing OSS for benign neo

Unusual pelvic mass in a young adult: prostatic Ewing's sarcoma with distant metastases.

Ewing's sarcoma is an aggressive small round cell tumour that usually arises from bone or soft tissues in children and young adults. Primary origin within the prostate is exceptionally rare, with only isolated cases documented. We describe a man in his 20s who presented with urinary obstruction, constipation and leg pain. Imaging revealed a bulky pelvic mass arising from the prostate and bilateral hydroureteronephrosis. Histology demonstrated a round cell neoplasm with strong CD99 and FLI1 positivity, confirming Ewing's sarcoma. Staging PET-CT identified widespread metastases involving lung, l

Breast malignant phyllodes tumor with rare pelvic metastases and long-term overall survival: A case report and literature review.

BACKGROUND: Malignant phyllodes tumor (PT) is a rare fibro epithelial neoplasm of the breast, which is poor prognosis due to high risk of recurrence and distant metastasis. METHODS: We report a case of malignant PT. It had recurred locally five times, and the sixth relapse was occurred 54 months after first diagnosis, presenting a huge pelvic mass (14 cm × 11 cm) by CT scan. Histopathological examination has demonstrated a metastatic phyllodes tumor. After postoperative chemotherapy treatment, a longer survival has been achieved, which is more than 72 months. RESULTS: Our case report describ

Cotyledonoid dissecting leiomyoma of the uterus with intravascular growth in postmenopausal woman: a case presentation.

Cotyledonoid dissecting leiomyoma ("Sternberg tumor") is an unusual type of a benign uterine smooth-muscle tumor with a distinctive gross appearance. A 57-year-old woman (gravida 7, para 6), in menopause for 3 years, presented with a 3-4-year history of increased pelvic pain. The transvaginal ultrasound scan showed a 2.5 cm sized heterogenic mass in the uterus cavity and 4 × 5 cm sized irregular contoured mass in the right lateral aspect of the uterus.
