Management of an unerupted dilacerated maxillary central incisor: a case report.

A case with a dilacerated maxillary permanent central incisor, treated with forced eruption technique is illustrated and the results of the 4-year follow up are presented. After the successful eruption of the tooth, the root development was completed and the root canal was obliterated. The 4-year follow-up results revealed the tooth to be still functional and the radiographic evaluation showed that the periodontal and periapical tissues were intact and healthy. In conclusion, the impacted dilacerated incisor diagnosed in the early mixed dentition should be treated with the aid of orthodontic t…

The challenges of treating a fused tooth.

This paper describes and discusses the multidisciplinary treatment involving a permanent maxillary lateral incisor fused to a supernumerary tooth, both presenting pulp necrosis and periapical lesion. A 15-year-old male patient sought treatment complaining of pain, swelling and mobility on the maxillary right lateral incisor. After clinical and radiographic examination, root canal preparation was performed according to the crown-down technique and a calcium hydroxide dressing was placed for 15 days. The patient returned and the definitive endodontic filling was done with thermomechanical compac

The Dynamic Expression Changes of Neutrophil Extracellular Traps in Mouse Apical Periodontitis: A Potential Correlation With IL-17.

Neutrophil extracellular traps (NETs) consist of decondensed chromatin and antimicrobial proteins, which are released from neutrophils and h … 7 and increased gradually until day 21, primarily located in inflammatory infiltration areas of the periapical tissue. Additionally, western blot analysis showed increased CitH3 expression in periapical lesions on day 21 after pulp exposure, further confirming the presence of NETs. Both RT-qPCR and enzyme-linked immunosorbent assay (ELISA) revealed increased IL-17 …

Protective Actions in Apical Periodontitis: The Regenerative Bioactivities Led by Mesenchymal Stem Cells.

Resulting from bacterial infection, apical periodontitis (AP) is a common inflammatory disease of the periapical region of the tooth. The regeneration of the destroyed periapical alveolar bone and the surrounding periodontium tissues has long been a difficult task in clinical practice. These lesions are closely related to pathogen invasion and an overreactive immune response. It is worth noting that the protective healing process occurs simultaneously, in which mesenchymal stem cells (MSCs) have a crucial function in mediating the immune system and promoting regeneration. Here, we review the r

Management of large preiapical lesion with the combination of second generation platelet extract and hydroxyapatite bone graft: a report of three cases.

The pulp tissue necrosis and extensive periodontal diseases leads to the development of the inflammatory periapical lesion which causes a local response of bone around the apex of the tooth. Depends upon the nature of wound and available biological growth factors the outcome will be either regeneration or repair. Being a rich source of growth factors, platelet rich fibrin (PRF) posses many advantages in bone regeneration. The purpose of this case report is to present an attempt to evaluate the healing potential of the combination of PRF and Hydroxyapatite bone graft as opposed to using these m

Periapical actinomycosis of the mandible: A diagnostic challenge mimicking conventional apical periodontitis- A case report.

Actinomycosis is a rare, chronic suppurative infection caused by filamentous, Gram-positive anaerobic bacteria, primarily Actinomyces israelii, with cervicofacial involvement being the most common presentation. Periapical actinomycosis, a rare central subtype of cervicofacial actinomycosis accounting for approximately 1-2% of central cases, is characterized by an indolent, less invasive course. This case report describes an unusual presentation of mandibular periapical actinomycosis in a 26-year-old immunocompetent female with no history of trauma or predisposing comorbidities. The patient pre

Scanning electron microscopy of cells from periapical lesions.

Examination of lymphocytes from peripheral blood with the scanning electron microscope (SEM) has shown differences between B cells and T cells on the basis of their surface architecture. This study was initiated to determine whether the cellular components of periapical lesions could be identified with the use of similar criteria. Cells were dispersed from lesions by aspiration of fragments of tissue through syringe needles of decreasing diameters. The liberated cells were filtered on silver-coated Flotronic membranes and examined under the SEM. Lymphocytes, macrophages, epithelial cells, and

Successful Nonsurgical Resolution of a Large Periapical Lesion With Pre-Existing Sealer Extrusion and Adjacent External Inflammatory Root Resorption: A Case Report.

The primary aim of root canal treatment is to eliminate intracanal infection, achieve an apical and coronal seal, and create conditions that allow repair of the periapical tissues. In root-filled teeth, persistent apical periodontitis may be associated with extensive periapical lesions, external inflammatory root resorption affecting the treated or adjacent teeth, and extrusion of endodontic materials. These complications may lead clinicians to consider surgical approaches. However, when canals are adequately cleaned, disinfected, and hermetically obturated, favorable healing may occur followi…

Inflammation of rat molar pulp and periodontium causes increased calcitonin gene-related peptide and axonal sprouting.

We have studied the response of nerve fibers containing calcitonin gene-related peptide immunoreactivity (CGRP-IR) to inflammation using a rate dental experimental system. Inflammation was induced by drilling tooth cusps to create pulpal exposures; the induced pulpitis and subsequent periapical lesions were studied 1-35 days later using standard CGRP immunohistochemistry and the avidin-biotin peroxidase method. The injury and resulting inflammation caused a disruption of CGRP-IR nerve fiber location and arborization that varied depending on whether the initial injury was limited to the pulp ti

Effect of traumatic occlusion on periapical lesions in rats.

The effect of traumatic occlusion on periapical lesions in rats was investigated histologically and histometrically. Rats were divided equally into groups A to D. Rats in group A received no treatment; in group B, rats received pulpal exposure of the left mandibular first molar; in group C, a resin plate was cemented onto the occlusal surface of the corresponding maxillary molar; and in group D, the molar pulp was exposed and the resin plate was installed. At 1 and 2 wk, compression of the periodontal ligament and inflammation were less in group D than in group B. Lesions in the periapical per

T lymphocyte activation and cytokine expression in periapical granulomas and radicular cysts.

INTRODUCTION: Radicular cysts (RCs) are periapical lesions resulting in jaw bone destruction. The inflammatory dental periapical granuloma (PG) is considered to be the origin of RC formation; however the mechanism of RC development remains unclear. METHODS: Cell suspension from the surgically extirpated tissue of 27 RCs and 25 PGs was obtained. Bacteriological analysis of the PG tissue samples was performed in order to define two major groups of PG according to the prevailing causative bacterial infection: the streptococcal PG (PG-S, n=10) and the anaerobe PG (PG-A, n=9) group. The inflammato

Allogeneic Bone Marrow Mesenchymal Stromal Cell Transplantation Induces Dentin Pulp Complex-like Formation in Immature Teeth with Pulp Necrosis and Apical Periodontitis.

INTRODUCTION: Dental pulp regeneration is challenging in endodontics. Cellular therapy is an alternative approach to induce dental pulp regeneration. Mesenchymal stromal cells (MSCs) have the capacity to induce dental pulp-like tissue formation. In this study, we evaluated the capacity of allogeneic bone marrow MSCs (BM-MSCs) to regenerate pulp following necrosis and apical periodontitis in children's permanent immature apex teeth. METHODS: Patients aged 8 to 12 years with pulp necrosis and apical periodontitis were evaluated. The study included 15 teeth (13 incisors and 2 molars) from 14 pat

Endodontic management of immature teeth with spontaneous apical closure and periapical lesions: case series and review of the literature.

Spontaneous apical closure in non-vital immature teeth has been rarely encountered and outcome of non-surgical endodontic treatment of related teeth associated with periapical lesions has not yet been adequately elucidated. The aim of this article was to report endodontic management of spontaneous apical closure of infected untreated immature teeth with periapical lesions and to review previously proposed mechanisms for the development of spontaneous hard tissue barrier. Three patients were referred at different time intervals to the endodontic clinic for treatment of their maxillary anterior

[Bacteriologic study of the contents of radicular cysts].

INTRODUCTION: Radicular cysts are, most often, oral tissue cysts. The basis for their development are the remains of Hertwig's epithelial sheath, which is stimulated to proliferate by infectious agents, mostly periapical granuloma or pulp necrosis. The cyst contents can be liquid, either clear or milky-white, or thick and yellowish-caseous pus. Bacteriological and histopathological researches have shown that development of radicular cysts from periapical lesions is a consequence of penetration of normal bacterial flora of the oral cavity. MATERIAL AND METHODS: The study has bacteriologically

Management of an Adenomatoid Odontogenic Tumor Mimicking a Periapical Lesion in a Maxillary Lateral Incisor: A Multidisciplinary Regenerative Approach.

Periapical radiolucency in endodontically treated teeth is typically managed via conventional retreatment or apical surgery. However, non-inflammatory odontogenic tumors, such as an adenomatoid odontogenic tumor (AOT), can mimic common periapical pathologies, potentially leading to diagnostic errors. This case report describes the multidisciplinary management of an AOT in a 24-year-old male, highlighting the significant diagnostic challenge where a benign tumor mimicked persistent endodontic failure. The patient presented with a 7 × 8 mm hard labial swelling and throbbing pain in the maxillary
