Mayo Adhesive Probability Score Is Associated with the Operative Time in Laparoscopic Adrenalectomy.

Background: Laparoscopic adrenalectomy (LA) is the standard treatment for adrenal benign tumors, including primary aldosteronism (PA) or Cushing's syndrome (CS). Several obesity-related factors were associated with prolonged total operative time (OT), but perinephric fat characteristics were not assessed. We investigated whether the Mayo adhesive probability (MAP) score, which evaluates perinephric fat characteristics, was associated with OT for LA. Methods: This single-center, retrospective cohort study examined 141 consecutive patients who underwent LA for PA or CS. We reviewed patients' cha

Unexpected renal hemorrhage after endovascular repair of complicated type B aortic dissection: two cases report.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) is the therapeutic choice for type B aortic dissection. One of the most unfavored complications of this procedure is hemorrhage, which has a low incidence but high mortality. Renal hemorrhage (RH) after endovascular aortic repair has been rarely reported. We presented two cases of unexpected RH after TEVAR for complicated type B aortic dissection, and the potential causes, diagnosis and therapeutic management were discussed. CASE PRESENTATION: A 67-year-old female developed hypotension and progressively decrease of hemoglobin within 5 h

The impact of obesity and adiponectin signaling in patients with renal cell carcinoma: A potential mechanism for the "obesity paradox".

Although obesity increases the risk of renal cell carcinoma (RCC), obese patients with RCC experience longer survival than non-obese patients. However, the mechanism of this "obesity paradox" is unknown. We examined the impact of preoperative BMI, serum total adiponectin (sAd) level, total adiponectin secretion from perinephric adipose tissue, and intratumoral expression of adiponectin receptors on RCC aggressiveness and survival. We also investigated the mechanism underlying enhanced cancer aggressiveness in RCC cells stimulated with exogenous adiponectin. Overweight and obese patients had si

Significance of TNM staging, Demographic and Histologic Features in Predicting the Prognosis of Renal Cell Carcinoma.

BACKGROUND: The aim of this study was to evaluate the prognostic significance of clinical, demographic and detailed histopathological parameters in renal cell carcinoma (RCC). METHODS: A total of 102 patients who underwent partial or radical nephrectomy for a renal mass between 2008 and 2013 were evaluated retrospectively. Fuhrman grade, TNM stage, macroscopic satellite tumor nodule formation, histopathological subtype, renal vein invasion (RVI), necrosis, microvessel invasion (MVI), sarcomatoid differentiation and overall survival (OS) were evaluated to determine prognostic factors. RESULTS

MRI and CT characteristics of successfully ablated renal masses: Imaging surveillance after radiofrequency ablation.

OBJECTIVE: The objective of our study was to evaluate the evolution of the appearances of successfully ablated renal masses on CT and MRI. MATERIALS AND METHODS: We conducted a retrospective review of 28 solid renal masses in 25 patients who underwent percutaneous radiofrequency ablation (RFA) between July 2003 and July 2006 in whom there was no evidence of residual tumor during at least 1 year of imaging follow-up and there was postablation biopsy proof of nonviable tissue within the ablation cavity. Three radiologists assessed the size, morphology, and CT or MRI characteristics of the initi

A multivariate analysis of risk factors associated with subcapsular hematoma formation following electromagnetic shock wave lithotripsy.

PURPOSE: Subcapsular or perinephric hematoma is one of the most frequent and potentially serious complications of extracorporeal shock wave lithotripsy (SWL). We determined the incidence of and risk factors for renal hematomas following electromagnetic shock wave lithotripsy. MATERIALS AND METHODS: Between February 1999 and August 2003, 570 SWL treatments were performed using a Modulith SLX electromagnetic lithotriptor (Storz, St. Louis, Missouri). A total of 415 of these treatments in 317 patients were performed for stones in the renal pelvis or calices and these treatment episodes represent

From the spectrum of PEComa: a renal angiomyolipoma.

PEComas or Perivascular Epithelioid Cell neoplasms are uncommon mesenchymal origin neoplasms composed of perivascular epithelioid cells. They affect various anatomic sites, kidneys being one of them. Renal PEComas are further rare, posing a challenge to include them in the differential diagnoses. We present a case of a type of renal PEComa in a 54-year old post-menopausal female with vague abdominal pain. Imaging revealed a large left perinephric mass initially suspected to be a liposarcoma. Surgical excision and subsequent histopathological evaluation confirmed a benign, fat predominant renal

Minimally invasive intervention of forniceal rupture in a solitary functioning kidney: A case report.

Renal Forniceal rupture is a common urological emergency, most commonly due to an underlying ureteral obstruction. There is no standardized management for forniceal rupture. We report a 70-year-old female who presented with vague abdominal symptoms, and acute on Chronic Kidney Disease. Plain Computed Tomography scan of the abdomen/pelvis revealed grossly disturbed renal anatomy, with a large perinephric collection, and an obstructing pelvic-ureteric junction stone. She was initially managed with intravenous antibiotics, percutaneous drain and percutaneous nephrostomy placement. She subsequentl

Lymphoscintigraphy - Beyond Lymphedema.

Lymphoscintigraphy is an established modality for imaging the lymphatic system using radiocolloids and is routinely indicated to find the cause of limb lymphedema. However, in this case series, we are highlighting other less-known indications of lymphoscintigraphy like chylothorax and chyluria which present as lymphatic leaks in the thorax and abdomen, respectively. Once the site of the lymphatic leak is established by lymphoscintigraphy, definitive management like thoracic duct ligation or sclerotherapy can be done. The other indication discussed is postrenal transplant perinephric fluid coll

High rising flames: a sign of complex extraperitoneal bladder rupture.

Extraperitoneal bladder rupture is the most common type of bladder injury following pelvic trauma and is seen in 80-90% of cases. Generally, the contrast extravasation seen on computed tomography (CT) cystography is confined to the perivesical space, giving a characteristic flame-shaped appearance. Occasionally contrast extravasation can extend to the scrotum, thigh or anterior abdominal wall, depending on the extent of injury. Here we report an unusual CT cystogram finding of complex extraperitoneal bladder rupture with associated pelvic fracture urethral distraction defect, showing large-vol

Horseshoe Kidney Blunt Trauma With Double Laceration: Endovascular Management.

A 78-year-old male patient was admitted to our hospital after abdominal trauma. Contrast-enhanced computed tomography (CT) scan demonstrated a horseshoe kidney with a perinephric hematoma and evidence of arterial hemorrhage. An anomalous renal arterial anatomy was noted as well, with a renal artery originating from the left common iliac artery. He was successfully treated via an endovascular approach. Varying forms of vascularization may complicate angiographic treatment of patients with abdominal trauma in a setting of kidney anomalies. Obtaining and evaluating contrast-enhanced CT angiograph

Bilateral psoas muscle abscess associated with emphysematous cystitis.

Psoas muscle abscess associated with emphysematous urinary tract infection is very rare. There were very few reports about urinary tract infections such as renal abscess, perinephric abscess, and emphysematous pyelonephritis complicated with psoas muscle abscess; however, psoas muscle abscess associated with emphysematous cystitis has not yet been reported. Here, we report a case of bilateral posas muscle abscess following emphysematous cystitis in an 81-year-old nondiabetic man, who was treated successfully with prolonged antibiotic therapy and supportive care. Early recognition of psoas musc

Bear's Paw Sign: A Classic Presentation of Xanthogranulomatous Pyelonephritis.

A 41-year-old woman with recurrent urinary tract infections presented with right flank pain, fever, and anorexia. Physical examination revealed right-sided flank pain and a right-sided unilateral renal mass. Computed tomography of the abdomen and pelvis revealed xanthogranulomatous pyelonephritis with perinephric extension. The bear's paw sign was visualized. The classic radiology triad (unilateral large kidney, renal pelvis stone, and a nonfunctioning or poorly functioning kidney) was also found on computed tomographic imaging. She completed a course of antimicrobial therapy and underwent a n

Emphysematous pyelonephritis in a renal transplant recipient--is it possible to salvage the graft?

BACKGROUND: Emphysematous pyelonephritis (EPN) is a rare but potentially devastating complication after renal transplantation. It carries the risk of graft loss and is associated with a high mortality. The majority of cases reported thus far have undergone graft nephrectomy. CASE REPORT: We report the case of a live-related renal transplant recipient who presented 10 years later with fever, graft tenderness and worsening serum creatinine. Computed tomography (CT) revealed emphysematous pyelonephritis, with a large perinephric collection. The case was managed successfully by percutaneous drain

Pregnancy complicated by emphysematous pyonephrosis.

Emphysematous pyonephrosis is a rare, rapidly progressive infection characterized by the presence of exudative material and fermentative gases within a dilated renal collecting system. This unusual clinical entity is noted almost exclusively in diabetic patients, often in conjunction with chronic urinary tract infection and obstruction. In most instances, E. coli is the etiologic agent; however other enterobacteriaceae may be isolated. High fever, leukocytosis, renal parenchymal necrosis and the accumulation of gases in the kidney and perinephric tissues are the hallmark of this condition. Sur

18 F-FDG PET/CT Findings in a Case of Plasmablastic Myeloma: A Rare Entity.

Plasmablastic myeloma (PBM) is an extremely rare variant of multiple myeloma, having morphologic, clinical, and radiologic overlap with plasmablastic lymphoma (PBL). Thus, distinguishing both becomes vital due to different treatment strategies. Due to limited literature on imaging findings in this rare entity, here we present a unique case of PBM developing in the setting of chronic light chain myeloma, with extensive extramedullary involvement and severe osteopenia, in addition to pleural deposits, pleural effusion, and perinephric fascial deposit. Thus, 18F-FDG PET/CT can be utilized as an i

A Diabetic Patient with Prolonged Hyperammonemia Due to Urinary Tract Infection Caused by Urease-producing Bacteria.

Obstructive tract infection caused by urease-producing bacteria can lead to hyperammonemia. We herein report a 73-year-old woman with diabetes and compensated liver cirrhosis who developed obstructive pyelonephritis. Her consciousness level declined on day 3 of hospitalization. Blood tests revealed an elevated ammonia level of 140 μg/dL. Urine and blood cultures identified Proteus mirabilis as a urease-producing bacterium. The treatment included double J (WJ) catheter insertion and administration of antimicrobial agents. On day 7 of hospitalization, the ammonia level gradually decreased, and t
