An unfortunate injection.

Intramuscular injection has been used to administer medications for more than a hundred years. However, despite our profession's long experience with intramuscular administration, preventable complications such as injection nerve palsies are still prevalent in developing countries. Injections account for one-fifth of all traumatic nerve injuries. These injuries largely occur due to indiscriminate use of intramuscular injections for treating common illnesses, frequently by unlicensed or undertrained practitioners administering unnecessary treatment to impoverished patients. The sciatic nerve is

[Postoperative palsies of the common peroneal nerve and the tibial nerve associated with lateral position].

A 36-year-old woman with systemic lupus erythematous secondary to diabetes underwent right total hip arthroplasty in the left lateral position. An epidural catheter was inserted at the L1-2 interspace and placed cephalad prior to induction of general anesthesia. After the operation, she complained of numbness on the dorsum and plantar of the left foot and was unable to dorsiflex and plantarflex the ankle. The symptoms persisted after discontinuation of the epidural infusion of a local anesthetic, and they became exacerbated after the second operation in the left lateral position. Electromyogra

Evaluation of Quantitative and Selective Sensory Fiber Dysfunction in Patients with Cirrhosis.

BACKGROUND: Chronic liver disease has been reported to be associated with peripheral neuropathy. However, which sensory fibers are affected remains unknown. The objective of this study was to examine the function of sensory nerve fibers in patients with cirrhosis using the current perception threshold (CPT) test, as well as the correlation between blood biochemical indicators related to cirrhosis and CPT values. METHODS: We recruited 44 patients with liver cirrhosis and 37 healthy controls of the same age and gender. The Neurometer® system for the CPT test was used to stimulate the median ner

Phrenic neuropathy in chronic renal failure.

BACKGROUND: Peripheral neuropathy and alterations in diaphragmatic muscle function are frequently caused by uraemia. Phrenic nerve function in patients with end stage renal failure, however, has not been examined to date. METHODS: An electrophysiological study of the phrenic nerve was performed to determine its possible involvement in 32 nondiabetic patients with end stage renal disease undergoing chronic haemodialysis. RESULTS: Seventeen patients had electrophysiological signs of peripheral neuropathy in at least one of the investigated nerves and 14 of the 17 showed pathological phrenic ne

Pseudotumor-Related Compression Neuropathies Following Total Hip Arthroplasty.

BACKGROUND: Pseudotumor-related compression neuropathies following total hip arthroplasty (THA) represent a severe phenotype of adverse local tissue reaction. While historically attributed to metal-on-metal bearings, other constructs such as dual-modular necks and taper corrosion with cobalt-chrome femoral heads have been implicated as well. Furthermore, inflammatory responses to polyethylene wear debris and nonforeign body sources can similarly stimulate pseudotumor formation, resulting in symptomatic nerve compression. This study aimed to characterize the etiology, clinical presentation, and

Anterior tarsal tunnel syndrome: a misunderstood and a misleading entrapment neuropathy.

Anterior tarsal tunnel syndrome (ATTS) is a rare entrapment neuropathy of the deep peroneal nerve beneath the extensor retinaculum on the top of the ankle. ATTS is often asymptomatic or olygosymptomatic. There are few reports describing the ATTS. We describe the clinical and electrophysiological features of 85 patients with unilateral or bilateral ATTS prospectively collected between January 2000 and December 2010 in our laboratory of Clinical Neurophysiology. This entrapment neuropathy remains poorly diagnosed and it might be misleading when performing a diagnostic EMG-ENG examination for sus

Carcinomatous neuropathy. An ultrastructural study of ten cases.

Superficial peroneal nerve biopsies were studied from 10 patients with carcinomatous neuropathy. There were 6 patients with the sensorimotor form and 4 patients with the sensory form. In both forms of neuropathy, histometric studies showed a variable loss of myelinated fibers which was generally most severe in the sensory form. In most of the cases of the sensorimotor form, ultrastructural studies showed segmental demyelination with onion bulb formation and regeneration of myelinated fibers. In the sensory form, there was no segmental demyelination, the regeneration of myelinated fibers was mi

Evaluation and Management of Adult Footdrop.

Footdrop is a common musculoskeletal condition defined by weakness in ankle joint dorsiflexion. Although the etiology varies, footdrop is characterized by specific clinical and gait abnormalities used by the patient to overcome the loss of active ankle dorsiflexion. The condition is often associated with deformity because soft-tissue structures may become contracted if not addressed. Patients may require the use of special braces or need surgical treatment to address the notable level of physical dysfunction. Surgical treatment involving deformity correction to recreate a plantigrade foot alon

Mirror-Based Robotic Therapy for Ankle Recovery with a Serious Game: A Case Study with a Neurological Patient.

Neuromuscular disorders, such as foot drop, severely affect the locomotor function and walking independence after a brain injury event. Mirror-based robotic therapy (MRT) has been a promising rehabilitation strategy favouring upper limb muscle strength and motor control in the last years. However, there are still no studies validating this technique in lower limb experimental protocols. This paper presents an innovative visual and motor feedback strategy based on serious games and MRT modalities. Thus, a preliminary system validation with a healthy participant is performed. Moreover, the strat

Peripheral nerve palsy in a case of leptospirosis.

We describe an unusual case of leptospirosis in a 54-year-old man presenting peripheral nerve palsy. The diagnosis of leptospirosis was confirmed by ELISA IgM and the microscopic agglutination test. Electrophysiological studies showed that no response could be obtained from the right fibular nerve. At 7 months after the initiation of treatment, additional electrophysiological studies and a neurological examination showed, respectively, a chronic axonal lesion of right fibular nerve with signs of re-innervation and a nearly complete clinical recovery. We feel that this case may serve to remind

Entrapment neuropathies of the foot. Electromyographic techniques.

Entrapment neuropathies frequently involve the foot. The diagnosis can be established by electrophysiological techniques for the exploration of muscles and nerves. Needle recording from the intrinsic foot muscles is a valuable asset in the detection of neurogenic syndromes. The motor and sensory nerve conduction velocities are determined using surface electrodes. The sensory nerve conduction study can be supplemented with somatosensory-evoked-potential studies. Based on these investigations, the location, severity and prognosis of the nerve lesion can be determined. In the presence of tunnel a

Nerve entrapment syndromes of the lower limb: a pictorial review.

Peripheral nerves of the lower limb may become entrapped at various points during their anatomical course. While clinical assessment and nerve conduction studies are the mainstay of diagnosis, there are multiple imaging options, specifically ultrasound and magnetic resonance imaging (MRI), which offer important information about the potential cause and location of nerve entrapment that can help guide management. This article overviews the anatomical course of various lower limb nerves, including the sciatic nerve, tibial nerve, medial plantar nerve, lateral plantar nerve, digital nerves, commo

Combined peroneal and proximal tibial nerve palsies.

Combined compression of both the common peroneal nerve and the proximal tibial nerve at the level of the popliteal fossa is rare. Recently, an anatomic site of compression of the proximal tibial nerve at the soleal sling (originating arch for the soleus muscle) has been described in cadavers. The present report includes three patients who had a combined compression of the common peroneal nerve at the fibular neck (fibular tunnel syndrome) and compression of the proximal tibial nerve at the soleal sling (soleal sling syndrome). In each case, blunt trauma was the precipitating event. Neurolysis

Common peroneal and posterior tibial ischemic nerve damage, a rare cause.

A 56-year old gentleman presented to our orthopaedic foot and ankle clinic, with unusual symptoms in his left foot. He described a tight sensation over his toes, "like sandpaper under his skin". The pain had started post operatively following a bilateral aorto-femoral bypass. He was subsequently investigated and found to have an ischemic lesion Identified in his common peroneal and posterior tibial nerve with associated muscle atrophy on EMG. This represents a previously unreported complication of aorto-femoral bypass surgery.

Lower extremity nerve entrapments in athletes.

Nerve entrapments are a potential cause of lower extremity pain in athletes. Signs and symptoms suggestive of nerve entrapment include anesthesia, dysesthesias, paresthesias, or weakness in the distribution of a peripheral nerve. The physical examination may reveal an abnormal neurologic examination finding in the distribution of a peripheral nerve, positive nerve provocative testing, and positive Tinel sign over the area of entrapment. Electrodiagnostic studies, radiographs, magnetic resonance imaging studies, and sonographic evaluation may assist with the diagnosis of these disorders. Initia
