Restraint use in acute and extended mental health services for older persons.

Restraint of older persons in inpatient and residential care is used to control aggression, and prevent falls and other adverse outcomes. Initiatives to reduce these practices are being implemented worldwide. However, there has been little examination of restraint practice in psychiatric services for older persons. This paper reports a retrospective comparative analysis of restraint use in three acute and two extended care psychiatric inpatient wards in Australia. The analysis involved examination of restraint incidents and comparison of restrained and non-restrained patients. There was signif

Endocrine treatment of transsexual persons: extensive personal experience.

OBJECTIVE: The Endocrine Society's recently published clinical practice guidelines for the treatment of transsexual persons acknowledged the need for further information on transsexual health. We report here the experience of one provider with the endocrine treatment of transsexual persons over the past 2 decades. METHODS: Data on demographics, clinical response to treatment, and psychosocial status were collected on all transsexual persons receiving cross-sex hormone therapy since 1991 at the endocrinology clinic at Albany Medical Center, a tertiary care referral center serving upstate New Y

STRESS AND TRAUMA: Psychotherapy and Pharmacotherapy for Depersonalization/Derealization Disorder.

Depersonalization/derealization disorder is characterized by depersonalization often co-occurring with derealization in the absence of significant psychosis, memory, or identity disturbance. Depersonalization/derealization is categorized as one of the dissociative disorders, which also includes dissociative amnesia, dissociative fugue, dissociative identity disorder, and forms of dissociative disorder not otherwise specified. Although these disorders may be under-diagnosed or misdiagnosed, many persons with psychiatric illness who have experienced trauma report symptoms consistent with dissoci

Cocaine/"crack" dependence among psychiatric inpatients.

The authors studied 40 cocaine-dependent subjects admitted to psychiatric inpatient wards of a metropolitan hospital because of general psychiatric symptoms. The results indicate that the predominant form of cocaine administration (88%) was freebasing "crack." DSM-III-R cluster B personality disorders (N = 17) and schizophrenia (N = 13) constituted the diagnoses for 75% of the sample. Compared to the schizophrenic patients in this cohort, the patients with cluster B personality disorders used cocaine in greater quantities and more frequently and began abuse of the drug at an earlier age. A population-based study of familial coaggregation and shared genetic etiology of psychiatric and gastrointestinal disorders.

BACKGROUND: It has been proposed that having a psychiatric disorder could increase the risk of developing a gastrointestinal disorder, and vice versa. The role of familial coaggregation and shared genetic loading between psychiatric and gastrointestinal disorders remains unclear. METHODS: This study used the Taiwan National Health Insurance Research Database; 4,504,612 individuals born 1970-1999 with parental information, 51,664 same-sex twins, and 3,322,959 persons with full-sibling(s) were enrolled. Genotyping was available for 106,796 unrelated participants from the Taiwan Biobank. A logis

Interpersonal dependency in borderline personality disorder: clinical context and empirical evidence.

Personality theorists and practicing clinicians agree that high levels of interpersonal dependency play a role in Borderline Personality Disorder (BPD), and this link has been codified in several editions of the Diagnostic and Statistical Manual of Mental Disorders (DSM). Although there is widespread agreement that dependency is linked to BPD, there has never been a systematic review of empirical evidence bearing on this issue. This article reviews research in three areas: (1) the comorbidity of Dependent Personality Disorder (DPD) and BPD; (2) the association between trait dependency and BPD;

Living arrangements and mental health in Finland.

BACKGROUND: Non-married persons are known to have poor mental health compared with married persons. Health differences between marital status groups may largely arise from corresponding differences in interpersonal social bonds. However, official marital status mirrors the social reality of persons to a decreasing extent, and living arrangements may be a better measure of social bonds. Little is known about mental health in different living arrangement groups. This study aims to establish the extent and determinants of mental health differences by living arrangement in terms of psychological d

Personality assessment of patients with complex regional pain syndrome type I.

OBJECTIVE: There is controversy regarding the importance of psychological/psychiatric factors in the development of the Complex Regional Pain Syndrome (CRPS). Our objective was to determine whether CRPS type I patients were psychiatrically different from other chronic pain patients, with particular attention to personality pathology. DESIGN: A standardized clinical assessment of all major psychiatric categories, including personality disorders, was performed on 25 CRPS type I patients and a control group of 25 patients with chronic low back pain from disc-related radiculopathy. Psychiatric symptoms endorsed by somatization disorder patients in a psychiatric clinic.

Patients with somatization disorder (SD) endorse high rates of psychiatric symptoms. However, prior studies have not addressed whether these endorsed symptoms reflect underlying psychiatric illness or whether they represent symptom overendorsement mirroring somatic complaints in patients with SD. Thirty-two female outpatients with SD and 101 with other psychiatric disorders completed a checklist of current and lifetime psychiatric symptoms. These findings were analyzed with respect to the diagnoses given by their treating psychiatrists. Patients with SD displayed significantly more current and

Is there a specific psychiatric background or personality profile in functional dystonia?

OBJECTIVE: The aim of this cross-sectional study was to identify if there was a specific difference between patients with functional dystonia (DysF) and those with adult-onset, isolated idiopathic ("primary") dystonia (DysP) in terms of psychiatric disorders, psychological stressor, dissociation correlates, and personality traits. METHODS: Thirty-nine clinically definite DysF and 30 DysP patients matched by age, gender and dystonia distribution underwent psychiatric interview based on DSM-5 criteria and additional testings for global cognitive and psychiatric functions (Mini-Mental State Exam

Validation of schizoid personality scales using indices of schizotypal and borderline personality disorder in a criminal population.

The external validity of 10 schizoid personality scales was assessed against dimensional measures of DSM-III borderline (BPD) and schizotypal (SPD) personality disorders in a sample of 37 top-security prisoners. Significant relationships with SPD or BPD emerged for schizophrenism, withdrawn-disturbed relationships, hallucinatory predisposition, schizoidia, disordered thinking and perceptual aberration (r = 0.30-0.66). The first four of these scales were significantly related to SPD (r = 0.29-0.51) after partialling out the effects of BPD, indicating an intrinsic link between these scales and S

Is death from natural causes still excessive in psychiatric patients? A follow-up of 1593 patients with major affective disorder.

A follow-up of 1593 Iowans with major affective disorder showed excessive mortality from unnatural causes in primary and secondary depression, and bipolar depression, but not mania, compared with age- and sex-matched controls from the general population. Excessive death from natural causes was found in women with secondary unipolar depression and bipolar depression and in manics (men and women combined) who had concurrent organic mental disorders or serious medical illnesses. Natural death was not excessive in the absence of these conditions. We conclude that excessive natural death reported i

Internal Structure and Consistency of the Level of Personality Functioning Scale-Brief Form: A Systematic Review and Meta-analysis.

The present systematic review and meta-analysis sought to assess the internal structure and consistency of the Level of Personality Functioning Scale-Brief Form 2.0 (LPFS-BF 2.0) across different populations and cultures. Studies identified through systematic research were subjected to eligibility criteria, resulting in 30 included studies and 110 effect sizes. Study characteristics were tabulated, their methodological quality was assessed, and findings were synthesized using a random-effects meta-analysis. Findings overall supported the internal structure of the LPFS-BF 2.0 and indicated soun

RESULTS: We assessed 164 former patients and 80 controls. The me
