[A contemporary view on the problem of pathogenesis and treatment of phantom pain syndrome].

Phantom pain syndrome (PPS) occurs after the amputation of various organs, most commonly limbs, and is associated with maladaptive neuroplastic changes in the peripheral and central nervous systems. Since the first description of PPS clinical features by the French military surgeon Ambroise Paré in the 16th century, the number of studies on this issue has grown annually. However, despite significant advances in understanding the mechanisms of PPS development, clear predictors of its onset is still unknown, and the effectiveness of current therapies remains insufficient. Treatment requires a mu

Lower limb amputees undergo long-distance plasticity in sensorimotor functional connectivity.

Amputation in adults is associated with an extensive remapping of cortical topography in primary and secondary sensorimotor areas. Here, we … al impact of lower limb amputation in the brain. Our findings in a selective group of patients with phantom limb sensations, but without pain suggest that disinhibition of neural inputs following traumatic limb amputation disrupts sensorimotor topology, unbalancing functional brain network organization. These findings step up the description of brain plasticit …

Application of Spare Parts in Combination with Targeted Muscle Reinnervation Surgery.

SUMMARY: Targeted muscle reinnervation is a contemporary technique designed to enhance an amputee's ability to operate a myoelectric prosthesis. This technique has been shown to decrease neuropathic pain, including neuroma and phantom limb pain. In certain amputations, especially forequarter and hindlimb levels, there may be no nearby recipient muscle sites, or the residual nerve may be too short to perform targeted muscle reinnervation. Applying the spare parts concept can help solve this problem by providing nerve autograft or additional muscle recipient sites within the spare parts flap for…

NKG2D blockade impairs tissue-resident memory T cell accumulation and reduces chronic lung allograft dysfunction.

Targeted muscle reinnervation (TMR) is a powerful new tool in preventing and treating residual limb and phantom limb pain. In the adult population, TMR is rapidly becoming standard of care; however, there is a paucity of literature regarding indications and outcomes of TMR in the pediatric population. We present 2 cases of pediatric patients who sustained amputations and the relevant challenges associated with TMR in their cases. One is a 7-year-old patient who developed severe phantom and residual limb pain after a posttraumatic above-knee amputation. He failed pharmacologic measures and unde…

Phantom Radicular Pain Treated with Lumbar Microdiscectomy: A Case Report.

Phantom limb pain in amputee patients is not well defined in the literature. Also, there is still no clear information on the definition and treatment of phantom radicular pain, in which amputee patients have radicular pain. This phenomenon, called phantom radiculitis or phantom radiculopathy, has been described in a limited number of cases. If a pathological condition that may cause radicular pain is present in amputee patients, the severity of phantom pain increases. Degenerative disc disease is the most common cause of phantom radicular pain. Spinal injection can be used to control this pai…

Relief from intractable phantom pain by combining psilocybin and mirror visual-feedback (MVF).

AL's leg was amputated resulting in phantom-limb pain (PLP). (1) When a volunteer placed her foot on or near the phantom - touching it evoked organized sensations in corresponding locations on AL's phantom. (2) Mirror-visual-feedback (MVF) relieved PLP, as did, "phantom massage". (3) Psilocybin-MVF pairing produced synergistic effects, complete elimination of PLP, and reduction in paroxysmal episodes. (4) Touching the volunteer's leg where AL previously had external fixators, evoked sensation of nails boring through the leg. Using a "telescoping" nail, we created the illusion of a nail being r

Prophylactic Regenerative Peripheral Nerve Interface Surgery in Pediatric Lower Limb Amputation Patients.

OBJECTIVE: To evaluate the prophylactic effect of regenerative peripheral nerve interface (RPNI) surgery on pediatric postamputation pain. BACKGROUND: Chronic postamputation pain is a debilitating and refractory sequela of limb amputation affecting up to 83% of pediatric patients with limb loss, resulting in disability and decreased quality of life. We postulate that prophylactic RPNI surgery performed during amputation may decrease the incidence of symptomatic neuroma and development of phantom limb pain, as well as limit analgesic use among pediatric patients with limb loss. METHODS: Retro…

Targeted Muscle Reinnervation: A Paradigm Shift for Neuroma Management and Improved Prosthesis Control in Major Limb Amputees.

Targeted muscle reinnervation (TMR) is a procedure that redirects nerves severed by amputation to new muscle targets. In tandem with advances in myoelectric prosthetics, TMR surgery provides amputees with improved control of myoelectric prostheses and simultaneously prevents or treats painful neuromas. TMR also has an emerging role in the management of neuromas in a nonamputation setting, and it seems to be a powerful strategy to treat a wide variety of neuromas. Because the pattern of nerve transfers varies based on the availability of donor nerves and muscle targets, TMR is inherently nonpre

Evoking haptic sensations in the foot through high-density transcutaneous electrical nerve stimulations.

OBJECTIVE: Evoking haptic sensation on upper limb amputees via peripheral nerve stimulation has been investigated intensively in the past decade, but related studies involving lower limb amputees are limited. This study aimed to evaluate the feasibility of using non-invasive transcutaneous electrical nerve stimulation to evoke haptic sensation along the phantom limb of the amputated foot of transtibial amputees. APPROACH: A high-density electrode grid (4 × 4) was placed over the skin surface above the distal branching of the sciatic, tibial, and common peroneal nerves. We hypothesized that el…

[Use of the Myo Plus system in transradial amputation patients].

Upper limb amputations are uncommon but cause severe disability. It is usually of traumatic origin and in young males less than 40 years-old, and active. The frequency of amputation at transradial level is around 35% of cases, with the myoelectric prosthesis achieving greater functionality. Around 25% of the patients give up using of the prosthesis, due to its complexity of use by means of 2 conventional electrodes, one of the most common causes for giving up. The Myo Plus system with an 8-channel electromyograph helps the patient to control the prosthesis using the phantom limb and achieving …

Thermal Symmetry Between Residual and Intact Limbs in Individuals with Lower Limb Amputation: Resting and Post-Activity Conditions.

Background and Objectives: Individuals with lower limb amputation are at increased risk of developing post-amputation complications that may affect rehabilitation outcomes. Assessing thermal symmetry between the residual and intact limbs, as well as activity-related temperature changes, represents a non-invasive and cost-effective approach for preliminary clinical evaluation. This study aimed to investigate regional skin temperature symmetry between residual and intact limbs at rest and following activity, and to examine whether the presence of phantom limb sensation or pain influences thermal…

Fibromyalgia: a real phantom pain?

Fibromyalgia and phantom limb pain have in common the perception of chronic pain and paresthesia located in areas of the body that have no underlying tissue damage. Debate remains over whether the pain in these two conditions originates in the central nervous system (central sensitization) or dorsal root ganglia. This perspective examines current hypotheses on the pathogenesis of fibromyalgia and phantom limb pain, proposing that knowledge sharing on the mechanisms leading to these two conditions may help to better appraise chronic non-nociceptive pain.

Estimating the Impact of Postamputation Pain.

BACKGROUND: Neuromas, neuralgia, and phantom limb pain commonly occur after lower-extremity amputations; however, incidence of these issues is poorly reported and understood. Present literature is limited to small cohort studies of amputees, and the reported incidence of chronic pain after amputation ranges as widely as 0% to 80%. We sought to objectively investigate the incidence of postamputation pain and nerve-related complications after lower-extremity amputation. METHODS: Patients who underwent lower-extremity amputation between 2007 and 2017 were identified using a national insurance-ba…

Pulsed radiofrequency of brachial plexus under ultrasound guidance for refractory stump pain: a case report.

The post-amputation (pain) syndrome, including stump pain, phantom limb sensation, and phantom limb pain is common but difficult to treat. Refractory stump pain in the syndrome is an extremely challenging and troublesome clinical condition. Patients respond poorly to drugs, nerve blocks, and other effective treatments like spinal cord stimulation and surgery. Pulsed radiofrequency (PRF) technique has been shown to be effective in reducing neuropathic pain. This report describes a patient with persistent and refractory upper limb stump pain being successfully relieved with PRF of brachial plexu…

Phantom angina.

A 49-year-old man with a history of traumatic left above-elbow amputation was hospitalized for surgical management of a phantom pain syndrome. Evaluation revealed a history of exertional chest pain radiating into the phantom limb. Exercise testing reproduced the pain symptoms and demonstrated electrocardiographic ischemic S-T segment depression. Coronary angiography revealed severe three-vessel coronary artery disease. The patient underwent coronary artery bypass graft surgery which eliminated the anginal component of his phantom pain syndrome and abolished evidence of exercise-induced myocard…

Behavioral and electrophysiological evidence of motor cortex activation related to an amputated limb: a multisensorial approach.

Phantom limb sensations may be linked to motor activities in the deafferented cortices of amputees, with artificial visual feedback of an amputated limb leading to enhanced phantom sensations. The present study was designed to verify if cortical motor activity related to an amputated limb can be triggered by visual input using an objective behavioral measure and with a neurophysiological correlate. Trauma amputees and normally limbed subjects showed superior performance in a mirror-drawing task when the mirror was placed sagittally (giving visual feedback of the amputated/inactive limb) compar…

Postamputation pain: epidemiology, mechanisms, and treatment.

Postamputation pain (PAP) is highly prevalent after limb amputation but remains an extremely challenging pain condition to treat. A large part of its intractability stems from the myriad pathophysiological mechanisms. A state-of-art understanding of the pathophysiologic basis underlying postamputation phenomena can be broadly categorized in terms of supraspinal, spinal, and peripheral mechanisms. Supraspinal mechanisms involve somatosensory cortical reorganization of the area representing the deafferentated limb and are predominant in phantom limb pain and phantom sensations. Spinal reorganiza…
