PUVA has definitely proved effective in outbursts of extensive psoriasis vulgaris. Efficacy is enhanced by combination with retinoids. The pharmacokinetics of psoralen, which lead to adjustment of dosage to each case, is a concrete improvement which facilitates treatment. The side-effects of PUVA, and notably the risk of carcinogenesis, are well known and can be alleviated. UVB phototherapy, successfully used for a long time against psoriasis, has benefited from technical advances in the field of artificial sources of UV. In its conventional modality (wide UVB band), the adjunction of greasy e…

This report describes a group of patients with hyperbilirubinemia treated with phototherapy at home. An account is made of methods used, advice given, problems encountered, and results obtained. It is suggested that home phototherapy can and should be carried out by pediatricians as part of routine outpatient pediatric care.

Idiopathic neonatal jaundice derives from an initial insufficiency of all processes which metabolize hydrophobic bilirubin into diglucuronid … hence a major intervention in the process of maturation of the neonatal enzyme systems. Therefore, phototherapy must be regarded as the treatment of choice in cases of idiopathic neonatal hyperbilirubinemia. It leads to a bypassing of the hepatic enzyme insufficiency in that by interaction between light with a wavelength of around 460 nm and the bilirubin mol …

Five patients with solar urticaria were treated with ultraviolet A (UVA) radiation. Three experienced marked increases in sunlight tolerance, but two patients failed to improve. Subjective clinical improvement was documented objectively by marked increases in the minimal urtication doses obtained by provocative light testing. These results indicate that some patients with solar urticaria may respond to treatment with UVA radiation alone.

In order to investigate the main sites of action of phototherapy in the treatment of neonatal jaundice we studied (a) the in vivo and in vitro relationship between the hematocrit and the effectiveness of phototherapy, and (b) the effect of varying the skin area exposed to light. The results show that the hematocrit does not influence in vivo the efficacy of phototherapy, while they confirm that the total skin surface exposed to light is important in determining the effectiveness of light treatment. The authors have also studied the possible action of phototherapy on bilirubin solutions placed …

The management of neonatal jaundice can be made much easier using simple methods that increase the radiant energy in the range 420 to 480 nm. Two groups of infants being treated for physiological jaundice during a period of 6 months were compared. The first group received treatment from two unmodified phototherapy units and the second from two units in which the horizontal frames housing the tube lights were lowered, thus providing a higher 'dose' of phototherapy. Measurements of irradiance from the four units were compared. An improvement in treatment was noted in the group receiving the high…

This study was performed in order to evaluate possible changes in behavior in jaundiced infants without perinatal complications other than hyperbilirubinemia treated with phototherapy. Thirty jaundiced infants (mean bilirubinemia 13.3 mg/100 ml, range 8.4-17.5) born spontaneously at term and undergoing phototherapy for 6 h or more, and 30 comparison subjects similar for sex, birthweight, gestational age, Apgar score, obstetrical history and father's profession were examined during the 3rd day of life according to Brazelton Neonatal Behavior Assessment Scale (BNBAS). For 6 of 26 items we found …

Hyperbilirubinemia in the newborn (neonatal jaundice) may cause irreversible brain damage if plasma concentrations of bilirubin exceed the number of binding sites on albumin and other blood components. Phototherapy or exchange transfusions to prevent the excessive rise in concentration of the pigment should be instituted in appropriate clinical situations. In phototherapy, the jaundiced infant is exposed to visible light containing the wavelength (about 450 nm) bilirubin will absorb. Because bilirubin is quite photolabile and will readily isomerize in vivo, it is rapidly converted to excretabl…

Maldistribution of light is a potential cause of phototherapy failure in the neonate. Differences in light intensity over the surface can be measured with a radiometer as this detects the radiant energy impinging on the incubator mattress. An infant nursed on the front third of the mattress receives less than 40% of the maximal light incident on the centre of the mattress, and consequently degradation of unconjugated bilirubin will be less effective.
