Clinical voice analysis of Carnatic singers.

BACKGROUND AND OBJECTIVE: Carnatic singing is a classical South Indian style of music that involves rigorous training to produce an "open throated" loud, predominantly low-pitched singing, embedded with vocal nuances in higher pitches. Voice problems in singers are not uncommon. The objective was to report the nature of voice problems and apply a routine protocol to assess the voice. METHODS: Forty-five trained performing singers (females: 36 and males: 9) who reported to a tertiary care hospital with voice problems underwent voice assessment. The study analyzed their problems and the clinica

Melodic Contour Identification by Cochlear-Implant Listeners With Asymmetric Phantom Pulses Presented to Apical Electrodes.

OBJECTIVES: (a) To compare performance by cochlear-implant listeners on a melodic contour identification task when the fundamental frequency (F0) is encoded explicitly by single-pulse-per-period (SPP) pulse trains presented to an apical channel, by amplitude modulation of high-rate pulse trains presented to several electrodes, and by these two methods combined, (b) to measure melodic contour identification as a function of the range of F0s tested, (c) to determine whether so-called asymmetric phantom stimulation improves melodic contour identification relative to monopolar stimulation, as has

Neural Switch Asymmetry in Feature-Based Auditory Attention Tasks.

Active listening involves dynamically switching attention between competing talkers and is essential to following conversations in everyday environments. Previous investigations in human listeners have examined the neural mechanisms that support switching auditory attention within the acoustic featural cues of pitch and auditory space. Here, we explored the cortical circuitry underlying endogenous switching of auditory attention between pitch and spatial cues necessary to discern target from masker words. Because these tasks are of unequal difficulty, we expected an asymmetry in behavioral swi

The impact of depression on musical ability.

BACKGROUND: While there is sustained effort to refine the models and to further decrypt the neuronal underpinnings of cerebral music perception, empirical investigations on the associations of music perception with diagnoses others than focal brain damage need to be amplified. METHODS: In the present study, a validated and standardized clinical test of musical ability was applied to a sample of severely depressed patients. Basic musical capacities of rhythm, melody and pitch perception, recognition of emotions, and musical memory were evaluated and compared to matched healthy controls and rea…

Isoflurane/N2O anesthesia suppresses narrowband but not wideband inhibition in dorsal cochlear nucleus.

Anesthesia alters the response properties of neurons in the dorsal cochlear nucleus (DCN). Barbiturates decrease spontaneous activity and the prevalence of inhibitory responses, so that DCN principal cells show less inhibition by narrowband stimuli (e.g. tones at best frequency). Here we present the effects on cat DCN of anesthesia using isoflurane plus nitrous oxide (N2O). Because the cellular anesthetic mechanisms of isoflurane differ from those of pentobarbital, the effects of the two anesthetics in DCN might be different. The strength of two inhibitory circuits in the DCN, the narrowband a

Cricothyroid approximation to elevate vocal pitch in male-to-female transsexuals: results of surgery.

Functional changes in voice production, including pitch elevation, may help male-to-female transsexuals assume a new gender identity. To date, there has been a paucity of objective data on the effectiveness of pitch-raising methods. Acoustic data were gathered with regard to preoperative and postoperative changes in pitch, pitch range, and perturbation in 20 patients after cricothyroid approximation. Subjective data were gathered by means of a mailed questionnaire. The mean follow-up time was 22 months. The speaking fundamental frequency was raised by half an octave without any significant cha

Early stimulus processing in dysthymia and anhedonia.

Previous event-related brain potential (ERP) research has found that dysthymic subjects differ from control subjects during later stages of information processing. An important issue that emerges from this literature is whether differences found in these ERP components, typically associated with cognitive processing, can be attributed to earlier differences in basic perceptual processing. This study was undertaken to determine whether early processing deficits are apparent in dysthymic persons. Responses of dysthymics (n = 23) were compared with those of anhedonic (n = 15) and normal control (

P300 and alcohol consumption in normals and individuals at risk for alcoholism. A preliminary report.

Pairs of college student subjects (36 male, 36 female) were matched on age, sex, and personal drinking history. One pair member had a parent who met the DSM III criteria of alcoholism, while the other pair member had no close alcoholic relative. The P300 event-related brain potential (ERP) was obtained from each subject with auditory stimuli in an "oddball" paradigm. Target stimuli occurred randomly on 20% of the trials in a frequency discrimination task, a relatively easy intensity discrimination task, and a more difficult intensity discrimination task. Subjects indicated when the target item

Auditory filter asymmetry in the hearing impaired.

Thresholds for 2-kHz sinusoidal signals were determined in the presence of a notched-noise masker, for six normal-hearing listeners and 12 listeners with cochlear hearing losses. Following Patterson and Nimmo Smith [J. Acoust. Soc. Am. 67, 229-245 (1980)], conditions were used where the notch was placed both symmetrically and asymmetrically about the signal frequency. The auditory filter shape for both the low- and high-frequency side of the filter was calculated using the rounded-exponential form of the filter. In six hearing-impaired listeners, the auditory filter shape showed a shallow low-

[A practical method to differentiate the sensations of hearing and feeling (auditory and vibration threshold) in the low-frequency range (author's transl)].

An uncomplicated method to differentiate between an auditory and a vibration threshold. It is a well-known problem that it is very difficult or almost impossible to distinguish in a deaf person between the genuine auditory threshold, especially the bone conduction threshold, and a vibration threshold in the range of low frequencies up to 1000 cps. The studies conducted by the authors show that a frequency-modulated tone is extremely suitable for this purpose, because it is very easy to recognize a 5-6% frequency modulated tone of 5 dB (SL) by hearing, whereas it is almost impossible to detect

Music Enjoyment in Hearing Loss: Beyond Auditory Perception.

Background/Objectives: Music is an important component of quality of life and is increasingly recognized as a meaningful patient-centered outcome in hearing healthcare. Although individuals with hearing loss frequently report reduced enjoyment of music, research in this area has focused on auditory perception, including pitch, timbre, melody, and sound quality. Consequently, music enjoyment is often conceptualized and measured primarily through perceptual performance, despite growing evidence that enjoyment represents a broader subjective experience. Methods: This narrative review examines the

Evaluation of a Stereo Music Preprocessing Scheme for Cochlear Implant Users.

BACKGROUND: Although for most cochlear implant (CI) users good speech understanding is reached (at least in quiet environments), the perception and the appraisal of music are generally unsatisfactory. PURPOSE: The improvement in music appraisal was evaluated in CI participants by using a stereo music preprocessing scheme implemented on a take-home device, in a comfortable listening environment. The preprocessing allowed adjusting the balance among vocals/bass/drums and other instruments, and was evaluated for different genres of music. The correlation between the preferred settings and the pa

[New aspects of hearing aid fitting].

Improved methods to diagnose a hearing loss and the enormous progress in the development of digital and programmable hearing aids have been helpful in promoting the social integration of patients, even of those affected by a profound deafness. Besides the hearing loss, recruitment effects and alteration of sound can also be compensated by the new hearing aids. However, one precondition is that the hearing aid fitting is carried out professionally, with plenty of time, and including the new hearing aid fitting methods. The new techniques developed in our hospital to optimize hearing aid fitting

Threshold sensitivity and frequency selectivity measured with round window whole nerve action potentials in the awake, restrained chinchilla.

Chronic electrodes, placed on the round windows of four adult chinchillas, were used to measure whole-nerve action potential (AP) thresholds and tuning curves. The AP quiet thresholds were within 17 dB of thresholds measured by behavioral methods and within 3 dB of those measured by other evoked response procedures. The AP tuning curves, obtained by a simultaneous masking procedure, were also similar to previously measured tuning curves in the chinchilla. These results indicate that long-term indwelling electrodes can be successfully placed on the chinchilla round window and used to measure th

[Psychoacoustic tests for perceptual assessment of processor fitting in patients with cochlear implants].

THE AIM OF THE STUDY: Was to study the possibility of using a battery of psychoacoustic tests to assess the tuning of the cochlear implant processor (CI) in deaf patients. METHODOLOGY: The study involved 60 prellingually deaf patients aged 10 to 23 years with oral speech skills. To assess the quality of the CI processor tuning, in addition to traditional methods, a special battery of psychoacoustic tests was used. The first block of tests assessed the perception of the basic characteristics of sound signals (duration, temporal structure, spectrum, timbre) and was used to assess the initial se

Neurophysiologic bases of speech discrimination.

The mismatch negativity (MMN) is an automatic cortical evoked potential that signifies the brain's detection of acoustic change. In other words, the MMN reflects the neurophysiologic processes that underlie auditory discrimination. As such, the MMN provides an objective tool for evaluating central auditory mechanisms involved in speech perception. We are using the MMN to study the central auditory processes that encode acoustic changes important for speech perception in 1) normal-hearing adults and children, 2) individuals with impaired auditory systems (including persons with learning disabil

Masking of speech by amplitude-modulated noise.

The masking of speech by amplitude-modulated and unmodulated speech-spectrum noise has been evaluated by the measurement of monaural speech recognition in such noise on young and elderly subjects with normal-hearing and elderly hearing-impaired subjects with and without a hearing aid. Sinusoidal modulation with frequencies covering the range 2-100 Hz, as well as an irregular modulation generated by the sum of four sinusoids in random phase relation, was used. Modulation degrees were 100%, +/- 6 dB, and +/- 12 dB. Root mean-square sound pressure level was equal for modulated and unmodulated mas

The energy spectrum of an impulse: its relation to hearing loss.

Permanent threshold shifts obtained from 242 chinchillas that were exposed to various impulse noise paradigms have been related to the energy spectra of the impulses. The impulses were generated by three different shock tubes that produced impulse noise spectra whose A-weighted energies showed peaks at 0.25, 1, and 2 kHz. The results show that there is an increasing susceptibility to NIPTS as the audiometric test frequency increases from 0.5 to 16 kHz. This increase in susceptibility to NIPTS is further accentuated by approximately 5 to 10 dB for impulses whose spectra peak at 2 kHz.

Auditory temporal integration and the power function model.

The auditory temporal integration function was studied with the objective of improving both its quantitative description and the specification of its principle independent variable, stimulus duration. In Sec. I, temporal integration data from 20 studies were subjected to uniform analyses using standardized definitions of duration and two models of temporal integration. Analyses revealed that these data were best described by a power function model used in conjunction with a definition of duration, termed assigned duration, that de-emphasized the rise/fall portions of the stimuli. There was a s
