REM sleep behavior disorder in sleep-disordered patients with versus without Parkinson's disease: is there a need for polysomnography?

We reviewed the polysomnograms (PSGs) of 292 consecutive patients with sleep disorders (Parkinson's disease (PD), n=19, other sleep disorders, n=273) to investigate the sensitivity and specificity of the clinical diagnosis of rapid eye movement behavior disorder (RBD) compared with polysomnographic diagnosis. Patients with dementia, multiple system atrophy, or any other neurodegenerative disease were excluded. RBD was diagnosed clinically if the minimal criteria, according to the guidelines given in the International Sleep Disorders Classification, were fulfilled. The following PSG criteria we

Uvulopalatopharyngoplasty for the obstructive sleep apnoea syndrome: value of polysomnography, Mueller manoeuvre and cephalometry in predicting surgical outcome.

Sixty consecutive patients with the obstructive sleep apnoea syndrome (53 men and seven women) were analysed by questionnaire, polysomnography, röntgenographic cephalometry and the Mueller manoeuvre before and 6 months after uvulopalatopharyngoplasty (UPPP), to assess the surgical outcome and the prognostic value of preoperative evaluation. Seventy-three per cent of patients reported improvement of snoring and 55% reported improvement of excessive daytime sleepiness. Thirty-five per cent showed a decrease of at least 50% in the desaturation index, and 13% had a postoperative desaturation index…

Validation of British Thoracic Society guidelines for the diagnosis of the sleep apnoea/hypopnoea syndrome: can polysomnography be avoided?

BACKGROUND: The British Thoracic Society report on the diagnosis and treatment of the sleep apnoea/hypopnoea syndrome (SAHS) suggests that, if the pulse oximetry baseline oxygen saturation is above 90%, then 15 4% oxygen desaturation/hour in bed will diagnose SAHS requiring treatment. The diagnostic outcome of applying these guidelines has been studied. METHODS: One hundred patients referred to a district general hospital sleep clinic were recruited. After initial clinical assessment, overnight pulse oximetry measurements were performed, followed by full polysomnography at the regional labora…

[Ambulatory monitoring of patients with suspected sleep apnea syndrome using a thermistor sensor in comparison with nocturnal polysomnography].

Simple ambulatory monitoring methods can be used in step-by-step diagnosis of sleep apnoea syndrome to differentiate between high-risk and low-risk patients, or to exclude the syndrome for achieving more efficient utilisation of sleep laboratory facilities. The question was whether a new method using a thermistor sensor measuring respiration-conditioned thermal convection at the mouth and nose can reliably record the frequency of apnoea (ambulatory thermistor method = ATM). The study was subdivided into two sections. In phase I the respiratory impulses measured via ATM were polygraphically rec

Portable polysomnography-derived multi-domain EEG markers of anesthetic state transitions during propofol anesthesia for gastrointestinal endoscopy.

BACKGROUND: Propofol anesthesia is widely used for gastrointestinal (GI) endoscopy, but electroencephalographic (EEG) features of anesthetic state transitions in this brief and stimulus-rich setting remain incompletely characterized. This study aimed to describe clinically interpretable EEG markers across anesthetic stages using portable polysomnography (PSG)-derived six-channel EEG. METHODS: In this prospective observational study, 31 adults undergoing GI endoscopy under propofol-based anesthesia underwent continuous portable PSG recording. EEG analyses focused on six scalp channels (F3, F4,…

MAF-SleepNet: A Multimodal Attention-Enhanced Fusion Network for Automatic Multi-Class Sleep Disorder Classification from Polysomnography.

Background/Objectives: Sleep disorders are heterogeneous conditions with diverse neural, muscular, and ocular manifestations, making polysomnography (PSG) the gold standard for accurate diagnosis. Artificial intelligence-based approaches, particularly deep learning (DL) models capable of integrating heterogeneous information, offer a promising solution for reliable decision-making in such clinical scenarios. However, most existing DL studies have focused on a single disorder, relied on limited datasets, or employed epoch-level labeling strategies that overlook the episodic nature of sleep path…

Can portable sleep monitors replace polysomnography for diagnosis of pediatric OSA: a systematic review and meta-analysis.

BACKGROUND: Obstructive sleep apnoea (OSA) is an increasing health problem in children. The "gold standard" for OSA diagnosis at the moment is overnight polysomnography (PSG). Some researchers think portable monitors (PMs) are promising methods for diagnosing OSA, which make children more comfortable and lower costs. Compared with PSG, our comprehensively evaluated the diagnostic accuracy of PMs for diagnosing OSA in pediatrics. RESEARCH QUESTION: This study aims to determine whether PMs can replace PSG in pediatric OSA diagnosis. STUDY DESIGN AND METHODS: The PubMed, Embase, Medline databas…

A polysomnography study examining the association between sleep and postoperative delirium in older hospitalized cardiac surgical patients.

Hospitalized older patients who undergo elective cardiac surgery with cardiopulmonary bypass are prone to postoperative delirium. Self-reported shorter sleep and longer sleep have been associated with impaired cognition. Few data exist to guide us on whether shorter or longer sleep is associated with postoperative delirium in this hospitalized cohort. This was a prospective, single-site, observational study of hospitalized patients (>60 years) scheduled to undergo elective major cardiac surgery with cardiopulmonary bypass (n = 16). We collected and analysed overnight polysomnography data using…
