This study is a description of a patient who exhibited diabetic ketosis associated with an alkalosis rather than acidosis and a review of eight previously reported cases. Precipitating factors for this syndrome are severe vomiting with loss of hydrogen, potassium, and chloride ions, and dehydration. The ingestion of alkali may also result in this mixed acid-base disturbance. Treatment consists primarily of replacement of potassium and chloride. All reported patients had received large doses of insulin for initial therapy; however, limited insulin (20 U) therapy in this patient almost completel

86Rb+ was used to study potassium uptake and efflux in type II pneumocytes freshly isolated from adult guinea-pig and rat lung. Both species exhibited a substantial ouabain-sensitive component of potassium influx. In rats, most of the ouabain-resistant influx was abolished by bumetanide and removal of extracellular chloride elicited no further effect. In contrast, only a proportion of the ouabain-insensitive uptake was inhibitable by bumetanide in guinea-pigs and this species showed an additional component of influx, which was chloride dependent and which was reduced by either the K(+)-H(+)-AT

This study was designed to examine the sodium, potassium, and chloride transport rates across the cortical and outer medullary collecting tubule and to localize the action of mineralocorticoid hormone. Rabbit collecting tubules were dissected from the cortex (CCT), the outer stripe of the outer medulla (OMCT0), or the inner stripe of the outer medulla (OMCTi). From normal rabbits, the transepithelial voltage was -20.8 +/- 2.7 mV in CCT, -4.2 +/- 1.4 mV in OMCT0, and +10.6 +/- 2.3 mV in OMCTi. From DOCA-treated rabbits, only the CCT voltage was different (-42.9 mV). Net sodium absorption across

The simultaneous effect of two weeks of potassium repletion therapy on the plasma concentration and total body content of potassium was assessed in seven patients with hypokalaemia. Glomerular filtration rate, renal plasma flow, and maximal urinary concentration were also measured both before and after therapy with potassium chloride. While the plasma potassium concentration quickly returned to normal, total body content of potassium remained depressed, emphasizing the unreliability of the plasma potassium concentration as an indicator of total body potassium status. Parameters of renal functi…

BACKGROUND: The SSaSS (Salt Substitute and Stroke Study) recently reported definitive effects of a potassium-enriched salt on cardiovascular outcomes and death. Quantifying the amount of potassium-enriched salt used by trial participants is important for understanding the magnitude of the effect of potassium-enriched salt on risk reduction and how population-wide scale-up might be achieved.
METHODS: Baseline and annual 24-hour urine samples were collected from subgroups of participants in SSaSS throughout the 5-year follow-up. The mean difference in 24-hour potassium excretion between the 2 g

Gitelman's (GS) and Bartter's (BS) syndromes are rare, inherited autosomal recessive tubulopathies characterized by hypokalemia, metabolic alkalosis, renal sodium, chloride, and potassium and magnesium-wasting. While the treatment based on potassium, sodium, chloride, and magnesium supplementation in addition to other pharmacologic options are widely established, recommendations about the dietary approach to GS and BS still remain generic. In this review we focus on the dietary strategies to increase sodium, potassium, and magnesium intake in GS and BS patients. Potassium and magnesium-rich fo

Bartter syndrome is an inherited renal tubular disorder with hypokalemia, hypochloremic metabolic alkalosis, normal blood pressure with hyper-reninemia and increased urinary loss of sodium, potassium and chloride. We report an infant with neonatal Bartter syndrome, who improved with potassium supplements.

We studied the effect of a composition of major and trace elements including calcium chloride, sodium chloride, iron (II) sulfate, zinc sulfate, chromium (III) chloride, and potassium bromide on orientation and exploratory behavior and conditioned response of animals in the open field test and conditioned drinking response in a multiple T-maze with three junctions. The test compound exhibited psychostimulant and nootropic activities.

Severe volume depletion from diuresis can produce potentially life-threatening hypotension, coronary and cerebral insufficiency, and significant renal impairment. Appropriate precautions include initiating diuretics at low doses and monitoring patients closely. Metabolic alkalosis, another frequent side effect of diuretic therapy, can usually be diagnosed by near absence of chloride in the urine. Corrective agents used include potassium chloride and sodium chloride.

Acute attacks of weakness in patients with hypokalemic periodic paralysis can usually be treated with oral potassium preparations. Occasional patients, however, require intravenous (IV) potassium administration. We studied a patient with hypokalemic periodic paralysis to determine the effect of using 5% glucose as a diluent for potassium administration during acute attacks of weakness. Administration of IV potassium chloride in 5% glucose (50 mEq/L) was associated with a worsening of strength and no rise in potassium level. Intravenous potassium in 5% mannitol was associated with a rise in pot…

OBJECTIVE: To determine whether furosemide treatment altered the blood flow properties and serum and RBC electrolyte concentrations of Thoroughbreds during submaximal treadmill exercise.
DESIGN: Thoroughbreds were subjected to submaximal treadmill exercise with and without treatment with furosemide (1 mg/kg of body weight, IV).
ANIMALS: 5 healthy Throughbreds that had raced within the past year and had no history of exercise-induced pulmonary hemorrhage.
PROCEDURE: Venous blood samples were obtained before exercise, at treadmill speeds of 9 and 13 m/s, and 10 minutes after exercise, and hem

A total of 34 children with normal renal function underwent either gastrocystoplasty or continent urinary reservoirs with stomach at our institutions. Severe hypochloremic hypokalemic metabolic alkalosis developed in 2 patients, manifested by intractable seizure disorder in 1 and altered mental status with respiratory depression in 1. Symptoms developed at 4 and 6 months, respectively. Despite severe alkalosis, urinary pH was less than 5.0 and fractional excretion of chloride remained high in both patients. Resuscitation with sodium chloride, arginine hydrochloride and potassium chloride resto…

The authors report the case of an 18 year old girl with nocturnal tetraplegia. There was a family history of Westphal periodic paralysis, and a past history of transient paralysis two years previously. The serum potassium was 2.3 mEq/l. The patient was cured by intravenous drip of potassium chloride. There was a reactionary hyperinsulinism. The red cell potassium was lowered. There was a fall in the exchangeable and total potassium pool. The E.C.G. changes; observed during the acute attack of paralysis, persisted afterwards, which is unusual.

Treatment with a combination of indomethacin and potassium chloride supplementation resulted in severe hyperkalemia in a patient with Bartter's syndrome. The clinical and electrocardiographic findings and hyperkalemia improved promptly with intravenous administration of sodium bicarbonate, glucose, and insulin, discontinuation of the potassium supplement, and reduction of the indomethacin dose. This case suggests that life-threatening hyperkalemia can occur when indomethacin and potassium supplementation are prescribed simultaneously in Bartter's syndrome.

Dietary interventions are recommended as part of hypertension (HTN) treatment. Sodium reduction has proven effective, however supplements of potassium and/or nitrate are likely also of importance. We aimed to test how these three dietary components affect blood pressure (BP) individually and in combination in patients with HTN. A double-blind randomized controlled trial of 90 participants with HTN was conducted. Participants were randomized to eight groups receiving a hand-out diet and different combinations of supplementation of sodium chloride (130 mmol/day), potassium chloride (40 mmol/day)…
