INTRODUCTION: Priapism is a urologic emergency with a tendency to recur in some patients. The frequency of, time to, and risk factors for priapism recurrence have not been well characterized.
AIM: To identify predictors of priapism readmission.
METHODS: We used the New York Statewide Planning and Research Cooperative System database to identify patients presenting to emergency departments with priapism from 2005 through 2014. Patients were tracked up to 12 months after initial presentation. Proportional hazards regression was used to identify risk factors for priapism readmission.

A 36-year-old Caucasian man was diagnosed with a 72-h-lasting priapism that occurred after the assumption of a Herbal supplement based on Tribulus terrestris, which is becoming increasingly popular for the treatment of sexual dysfunction. The patient underwent a cavernoglandular shunt (Ebbehoj shunt) in order to obtain complete detumescence, from which derived negative post-episode outcomes on sexual function. All patients consuming non-FDA-approved alternative supplements such as Tribulus terrestris should be warned about the possible serious side effects.

INTRODUCTION: To our knowledge, here we report the first case of nonischemic priapism following penile tattooing.
AIM: To report the first case of nonischemic priapism following penile tattooing.
METHODS: A case with tattooing-induced priapism is presented including subjective reporting, physical examination, and laboratory/radiologic evaluations.
RESULTS: A 21-year-old man, presented with partially rigid penis of 3-month duration. On examination, the penis was half rigid, with a tattoo on its dorsal surface, and a smaller tattoo on the glans (Figure 1). The patient initially stated that th…

AIM: To provide standard operating procedures for the diagnosis and management of priapism.
METHODS: Review of the literature.
MAIN OUTCOME MEASURES: Reduction of priapism and preservation of erectile function.
RESULTS: Priapism is a persistent penile erection that continues hours beyond, or is unrelated to, sexual stimulation. Priapism requires prompt evaluation and usually requires emergency management. There are two types of priapism: (i) ischemic (veno-occlusive or low flow), which is found in 95% of cases, and (ii) nonischemic (arterial or high flow). Stuttering (intermittent) priapism…

INTRODUCTION: Surgery is a mainstay in the management of ischemic priapism. The surgical armamentarium for this condition has recently been expanded with the introduction of several innovative procedures.
AIM: To review surgical procedures offered in the treatment of ischemic priapism and present a rational framework for their use.
METHODS: Medline searches through July 2010 were conducted using the terms priapism, surgery, shunt, and prosthesis.
MAIN OUTCOME MEASURE: Expert opinion was based on review of the medical literature related to this subject matter.
RESULTS: A host of surgical pr…

INTRODUCTION: Priapism describes a persistent erection arising from dysfunction of mechanisms regulating penile tumescence, rigidity, and flaccidity. A correct diagnosis of priapism is a matter of urgency requiring identification of underlying hemodynamics.
AIMS: To define the types of priapism, address its pathogenesis and epidemiology, and develop an evidence-based guideline for effective management.
METHODS: Six experts from four countries developed a consensus document on priapism; this document was presented for peer review and debate in a public forum and revisions were made based on r…

Priapism, a pathologic condition of persistent penile erection in the absence of sexual excitation, is a true erectile disorder. Although it may seem to affect only a small population of male individuals, it carries major significance. Possible complications of the disorder include penile fibrosis and permanent erectile dysfunction. All health care professionals should appreciate the importance of the disorder and be prepared to follow current principles of diagnosis and treatment to reduce or avert its complications.

Tamsulosin is the most potent adrenergic alpha-1 antagonist used for the treatment of benign prostatic hyperplasia. Priapism has been reported rarely in patients taking Prazosin, Doxazosin and Terazosin. We describe an otherwise healthy man with recurrent and then persistent unresolved priapism after the use of tamsulosin. Initial treatment consisted of aspiration and intracavernosal irrigation of iced saline and vasoconstrictive agent, but in vain. We then performed Winters procedure but that too failed and the priapism persisted. Health-care professionals should inform all patients taking su…

Abscesses of the prostate are infrequently encountered now as a result of effective antibiotics. The clinical diagnosis remains difficult. Herein we report an unusual case of a prostate abscess presenting as priapism. The diagnosis and management of the case is discussed, and the literature is reviewed.

Priapism seen occasionally in mice used in various mycoplasmal studies over several years prompted further investigation. Of six strains of young adult male mice inoculated intravenously with Mycoplasma pulmonis, of murine origin, priapism was seen only in CBA mice, penile erections persisting in some for seven to 44 weeks. A few mice given broth medium without mycoplasmas also developed priapism, and mice given five mycoplasmal species, of human origin, developed the condition in a way similar to these controls. However, those given M. pulmonis developed priapism earlier and of longer duratio…

Priapism, an unwanted painful erection of the penis, is a little discussed but common complication of sickle cell disease. What is known about the prevalence of priapism, efficacy of management approaches, and outcome is drawn primarily from retrospective and single-center reports. Priapism occurs in two patterns: prolonged and stuttering (ie, recurrent brief episodes that resolve spontaneously). If priapism persists for 4 hours or more without detumescence, the patient is at risk for irreversible ischemic penile injury, which may terminate in fibrosis and impotence. Large multicenter studies …

Priapism, in which penile erection persists in the absence of sexual excitation, is an enigmatic yet devastating erectile disorder. Current endeavors to manage the disorder suffer from a poor fundamental knowledge of the etiology and pathogenesis of priapism. These endeavors have remained essentially reactive, which commonly fail to avert its pathological consequences of erectile tissue damage and erectile disability, not to mention its psychological toll. The role of preventative management seems paramount with respect to priapism. As a prerequisite to formulating prevention strategies, gaini…

Priapism is a rare disease, characterized by prolonged, painful and irreducible erection, not resulting in ejaculation. It is an andrological emergency with a poor prognosis, as the risk of impotence is 50% despite appropriate management. Treatment is essentially surgical. The authors report their experience of 16 cases of priapism observed over a 15-year period. The mean age of the patients was 36 years (range: 21 to 56 years). The diagnosis was clinical. The mean time to consultation was 8 days (range: 1-28 days). Priapism was secondary to neuroleptics in 6 cases (37%), chronic myeloid leuka…

OBJECTIVES: Priapism induced by intracavernous injection of vasoactive drugs is uncommon, easily managed and carries a very low risk of possible sequelae. The pathophysiology, diagnosis and treatment of postinjection priapism is discussed.
METHODS/RESULTS: The series of Schramer, Padma-Nathan, Levine and other authors are reviewed. The incidence of postinjection priapism has been found to be less than 0.5% in general, although the incidence has been reported to range from 3 to 12% when papaverine is used alone.
CONCLUSIONS: Priapism induced by intracavernous injection of vasoactive drugs is …

Priapism is a condition of prolonged penile erection which often causes pain and is unrelated to sexual desire. There is a high risk of impotence despite immediate intervention. The incidence has doubled since the introduction of intracorporeal injection therapy for impotence. Two subtypes of priapism have been described, depending on the underlying cause. The more common type, termed low flow, is characterised by inadequate venous outflow, leading to a hypoxic painful prolonged erection. The etiology is either idiopathic or related to intracorporeal injection therapy. Treatment consists of as…
