Subacute cutaneous lupus erythematosus induced and exacerbated by proton pump inhibitors.

BACKGROUND: Subacute cutaneous lupus erythematosus (SCLE) can be induced by numerous drugs. We report 3 cases of SCLE induced by proton pump inhibitors (PPIs). OBJECTIVE: To highlight a rare cutaneous side effect induced by a frequently prescribed drug such as a PPI. CASE REPORTS: Case 1 was a 30-year-old man who developed multiple annular plaques over the trunk and lower limbs 1 month after the initiation of pantoprazole. Antinuclear antibodies (ANA) were positive with anti-Ro/SSA and anti-La/SSB antibodies, and histology confirmed the diagnosis. Clinical improvement was achieved 8 weeks af

Serum magnesium and proton-pump inhibitors use: a cross-sectional study.

OBJECTIVE: The aim of this study was to evaluate the association of serum magnesium levels with proton pump inhibitors (PPIs) use and other factors. METHODS: This was a cross-sectional study of 151 patients admitted with acute diseases in the Internal Medicine Division of the Hospital de Clinicas de Porto Alegre, after the exclusion of conditions that are commonly associated with hypomagnesemia: diarrhea; vomiting; chronic alcohol use; severely uncompensated diabetes mellitus; and chronic use of laxatives, diuretics or other drugs causing magnesium deficiency. RESULTS: All patients had norma

Obscure bleeding colonic duplication responds to proton pump inhibitor therapy.

We report the case of a 17-year-old male admitted to our academic hospital with massive rectal bleeding. Since childhood he had reported recurrent gastrointestinal bleeding and had two exploratory laparotomies 5 and 2 years previously. An emergency abdominal computed tomography scan, gastroscopy and colonoscopy, performed after hemodynamic stabilization, were considered normal. High-dose intravenous proton pump inhibitor (PPI) therapy was initiated and bleeding stopped spontaneously. Two other massive rectal bleeds occurred 8 h after each cessation of PPI which led to a hemostatic laparotomy a

Are proton pump inhibitors safe during pregnancy and lactation? Evidence to date.

Symptoms of gastro-oesophageal reflux disease (GORD or GERD) are estimated to occur in 30-50% of pregnancies, with the incidence approaching 80% in some populations. As with many other conditions in pregnancy, medical therapy with pharmaceutical agents is a concern, as the potential teratogenicity of medications is not well known. Although prevalence numbers are high, many patients have mild and infrequent symptoms, which often respond to lifestyle and dietary modifications. The exact mechanism and pathogenesis of GERD associated with pregnancy is likely multifactorial. Treatment strategies fo

Guidance on the use of over-the-counter proton pump inhibitors for the treatment of GERD.

OBJECTIVE: The aim of this paper was to develop a guideline on the over-the-counter management of gastroesophageal reflux disease with proton pump inhibitors (i.e. omeprazole). SETTING: A meeting of internationally renowned gastroenterologists in January 2009, in Berlin, Germany. METHODS: An expert panel group of gastroenterologists convened to develop a consensus-based algorithm for pharmacists for over-the-counter (OTC) treatment with proton pump inhibitors (PPIs). Key considerations were the short-term safety and efficacy of PPIs, and the extent of the risk to the sufferer, owing to the t

Which is the best choice for gastroesophageal disorders: Melatonin or proton pump inhibitors?

Melatonin is used in many countries to improve sleep disorders. Melatonin is a hormone produced by the pineal gland and enterochromaffin cells which control sleep and gastrointestinal motility. Low levels of melatonin lead to gastroesophageal reflux disease (GERD). Most of patients with GERD have a sleep disorder. So, low melatonin levels is the main cause of insomnia. Beyond this, it has an inhibitory action on gastric acid secretion and seems to control the lower esophageal sphincter. Proton pump inhibitors (PPIs) are a group of drugs whose main action is a pronounced and long-lasting reduct

[An unknown but potentially serious side effect of proton pump inhibitors: hypomagnesaemia].

Long-term use of proton pump inhibitors can lead to serious hypomagnesaemia. Intestinal magnesium absorption takes place by passive paracellular and active transcellular transport. It has been hypothesized that proton pump inhibitors impair the active transcellular magnesium transport. The resulting hypomagnesaemia may cause hypoparathyroidism, hypocalcaemia and hypokalaemia. Proton pump inhibitor-induced hypomagnesaemia is reversible: it resolves when proton pump inhibitors are stopped. The indication for long-term proton pump inhibitor treatment should be evaluated periodically.

Regulation of vacuolar proton pumping ATPase-dependent luminal acidification in the epididymis.

Luminal acidification in the epididymis is an important process for the regulation of male fertility. Low pH and low bicarbonate concentration are among key factors that keep spermatozoa in a dormant state while they mature and are stored in this organ. Although significant bicarbonate reabsorption is achieved by principal cells in the proximal regions of the epididymis, clear and narrow cells are specialized for net proton secretion. Clear cells express very high levels of the vacuolar proton pumping ATPase (V-ATPase) in their apical membrane and are responsible for the bulk of proton secreti

Systematic review: Proton pump inhibitor-associated acute interstitial nephritis.

BACKGROUND: A number of recent case reports and case series suggest that proton pump inhibitors may cause acute interstitial nephritis. AIM: To establish the nature of the relationship (cause or association) between proton pump inhibitor use and development of interstitial nephritis. DATA COLLECTION: Two researchers independently searched electronic databases (MEDLINE, EMBASE, GOOGLE, LILACS, COCHRANE) for articles from 1970 to 2006, including all study designs, populations and languages. Two independent reviewers assessed study quality and collected the data. SELECTION CRITERIA: absence of

Metabolic alkalosis in a hemodialysis patient--successful treatment with a proton pump inhibitor.

Hemodialysis patients develop metabolic acidosis due to their impaired excretion of daily produced protons (H+). The following report will show a rare case of severe metabolic alkalosis (predialysis pH 7.52, base excess (BE) +17) in a hemodialysis woman caused by self-provoked upper gastrointestinal H+ losses based on an eating disorder. Treatment with a proton pump inhibitor resulted in the normalization of acid/base homeostasis (predialysis pH 7.40, BE +1.6).
