BACKGROUND: Intertrochanteric femur fractures are common in the elderly and a growing public health concern. Surgical fixation remains the gold standard, with proximal femoral nail (PFN) and trochanteric fixation nail (TFN) being the most commonly used implants. This study compares the functional outcomes and complications of PFN and TFN in the Indian population.
METHODS: A prospective study was conducted over 16 months at Max Super Specialty Hospital, New Delhi, involving 52 skeletally mature patients with intertrochanteric fractures. Patients were divided into two groups: PFN (n=26) and TFN

PURPOSE: This study seeks to (1) describe the management of civilian ballistic extra-capsular proximal femur fractures (2) assess the rate of nonunion and complications and (3) compare the time to union of ballistic and blunt pertrochanteric femur fractures. Given the enhanced and widened extensive inflammatory response with ballistic trauma, we hypothesized that hip fractures from ballistic mechanisms would have faster times to union.
METHODS: Patients were included if they were skeletally mature with extra-capsular pertrochanteric femur fractures and radiographic and clinical follow-up of a

OBJECTIVES: Unstable intertrochanteric (IT) fractures, particularly in elderly patients with low bone mineral density, pose significant treatment challenges. Proximal femoral nail anti-rotation-II (PFNA-II) is widely used, but the optimal implant length (short vs. long) remains debated. The objective of this study was to compare the clinical and functional outcomes of short versus long PFNA-II implants in unstable IT fractures.
METHODS: A prospective comparative study was conducted at a tertiary hospital from November 2018 to November 2020. Adult patients (age ≥18) with recent (≤3 weeks) unst

INTRODUCTION: Femoral shaft fractures are highly prevalent. Surgical treatment is the optimal approach for managing them as it greatly decreases the rates of illness and death in injured people. The optimal therapy for these diseases entails the utilization of nail, plates, screws, and external fixation devices. At present, reamed intramedullary nailing is regarded as the optimal approach and universally recognized treatment indicated for fractures occurring in the shaft of the femur.
EXPERIMENTAL PROCEDURES AND TECHNIQUES: The study done at Krishna Hospital in Karad between February 2022 and

The purpose of this study was to compare the clinical results and the complication rates of a new generation of two intramedullary fixation devices: Proximal Femoral Nail Antirotation® (PFNA) and Gamma3®. We conducted a prospective randomised multicentre trial of 61 patients who underwent a PFNA fixation treatment (31 patients) or a Gamma3 nail (30 patients). We documented the fixation failure complications as well as data of the surgical procedure, the clinical and functional outcomes (the EuroQoL5 questionnaire, the Katz index score, the Short Form 36 (SF-36), and the Harris Hip Score) and t

BACKGROUND: Intramedullary nails for stabilizing intertrochanteric proximal femoral fractures have been available since the early 1990s. The nails are inserted percutaneously and have theoretical mechanical advantages over plates and screws, but they have not been demonstrated to improve patient outcomes. Still, use of intramedullary nails is becoming more common. The goal of this study was to examine trends in the use and associated outcomes of intramedullary nailing compared with sliding hip screws in Veterans Affairs (VA) hospitals.

INTRODUCTION: Ipsilateral proximal and shaft femoral fractures are rare, high-energy injuries associated with substantial morbidity. Evidence on predictors of adverse outcomes after surgical treatment remains limited.
METHODS: We conducted a retrospective cohort study at a tertiary trauma center including all skeletally mature patients who underwent surgical treatment for ipsilateral proximal femoral fractures (femoral neck or trochanteric region) associated with femoral shaft fractures between January 2019 and December 2024. The primary outcome was a composite of major complications, includi

INTRODUCTION: Intertrochanteric fractures in elderly individuals require stable fixation to achieve union, maintain alignment, and prevent post-operative collapse. Although proximal femoral nailing (PFN) provides reliable fixation in most cases, loss of reduction may still occur during fracture healing, leading to alteration in the femoral neck-shaft angle (NSA) and horizontal femoral offset, thereby affecting hip biomechanics and gait.
AIMS: The aim of the study was to evaluate the early radiological outcomes of reamed short PFN in intertrochanteric fractures among patients aged 50 years and

BACKGROUND: Proximal femoral fractures (PFF) in the increasingly elderly population have been associated with higher rates of fracture-related infection (FRI) and periprosthetic joint infection (PJI), which are likely to result in permanent functional loss and mortality. This retrospective study seeks to identify independent predisposing factors associated with infection and mortality following surgical intervention in these fractures in the elderly.
METHODS: A single-center retrospective cohort study was conducted from January 2019 to December 2023 involving elderly patients who underwent su

BACKGROUND: Stable intertrochanteric fractures of the hip are common injuries, particularly among the elderly population. Effective surgical intervention is crucial for improving patient outcomes and recovery. Two widely used fixation techniques are the Dynamic Hip Screw (DHS) and Proximal Femoral Nailing (PFN). Both methods aim to provide stability and help early mobilization, yet they differ in their biomechanical properties and clinical implications.
OBJECTIVE: This review article aims to compare the efficacy and outcomes of DHS and PFN in the treatment of stable intertrochanteric fracture

BACKGROUND: Global prevalence of osteoporosis and fragility fractures is increasing due to the aging population. Proximal femoral fractures are among the most common orthopedic conditions in elderly that significantly cause health deterioration and mortality. Here, we aimed to evaluate the mortality rates and risk factors, besides the functional outcomes after these injuries.
METHODS: In a retrospective cohort study, all patients admitted with a femoral neck or intertrochanteric fracture between 2016 and the end of 2018 were enrolled in this study. Medical records were reviewed to include pat

BACKGROUND: Treatment of pertrochanteric femoral fractures is often associated with significant blood loss. It has already been demonstrated that the administration of tranexamic acid (TXA) for endoprosthetic procedures reduces blood losses and leads to a decreased frequency of postoperative complications. The aim of this study is to demonstrate whether the administration of TXA as part of osteosynthesis treatment for pertrochanteric fractures using a proximal femoral nail reduces perioperative blood losses and haemorrhage-related complications.

BACKGROUND: The standard proximal interlocking screw (SS) configuration for antegrade intramedullary nail (IMN) fixation of femoral shaft fractures is lateral to medial or from the greater to less trochanter. Some authors argue for the routine use of the reconstruction screw (RS) configuration (oriented up the femoral neck) instead to prevent femoral neck complications. The purpose of this study was to compare a matched cohort of patients receiving these screw configurations and subsequent complications.
