OBJECTIVE: Gastroparesis is a disorder of delayed gastric emptying that is often chronic in nature. Up to 50% of type 1 diabetic subjects have symptoms of gastroparesis, which include nausea, vomiting, and early satiety. A schema is developed that outlines criteria to consider more than medical therapy for patients with severe pancreatitis that develop disabling abdominal pain. If the symptomatic patient has severe chronic pancreatitis that reaches the Cambridge Class V "marked" stage of image severity then endotherapy is indicated. OBJECTIVE: To demonstrate the feasibility and safety of pylorus-preserving gastrectomy (PPG) accompanied by complete suprapyloric and infrapyloric lymph node dissection.
DESIGN: Retrospective review.

We report a clinical case of a woman, 56 years old, with a history of duodenal ulcer and esophagitis, under medication with H2 receptors blockers during 3 years. Plasma basal gastrin was near normal levels. A stimulation test with secretin was compatible with gastrinoma. CT scan and selective angiography didn't localize the tumor. At laparotomy we found a pyloric nodule that prove to be the gastrinoma. Eighteen months after surgery the patient is asymptomatic and with a normal endoscopic examination and secretin stimulation test.

The organ-preserving operations were performed in 387, gastric resection--in 142, closure or excision of an ulcer without vagotomy--in 132 patients with bleeding pyloroduodenal ulcers. The lethality after gastric resection was 11.3%: after the emergency operations--16.3%; after early elective interventions--9.1%; after the organ-preserving operations--2.6, 5.3, 0.85%, respectively.

Double pyloroduodenal ulcers were treated in 86 patients. Active surgical tactics, improvement of the methods of local ulcer removal were the principles of treatment. The classification of the ulcers of double location by a topographic sign is suggested.

In 29 chloralose-urethane anesthetized dogs, a manometric assembly was inserted through a gastrostomy to monitor pressure of the pyloric region with a sleeve sensor. The case of a 40-year old woman who displayed 2 types of developmental anomalies is described. Besides the absence of the cholecyst aberrant pancreas was found in the pyloric duct. Pancreatic tissue was found also in one of the lymph nodes along the lesser curvature. In connection with the case the incidence of the developmental anomalies and symptomatology as well as the difficulties of the diagnosis and therapeutic possibilities are dealt with. No description of the simultaneous occurrence of the 2 developmental anomalies was found in the available literature.

The case of a haemophilic baby boy who developed a pyloric haematoma due to trauma caused by a feeding gastrostomy catheter balloon is presented. Intramural gastric haematomas are rare and the authors are not aware of any previous reports of intramural haematomas affecting the pylorus alone in this age group. The barium meal appearances of the pyloric haematoma were identical to those seen in hypertrophic pyloric stenosis but ultrasound demonstrated a circumferential pyloric haematoma.

The effect of glucagon on the stomach, pylorus and duodenum was evaluated within the first 30 minutes p.i. Glucagon was administered intravenously in a dose of 0.1, 0.2, 0.4, 0.6 and 1 mg. The hypotonic effect was greater on the duodenum than on the stomach. The dose response of the pylorus was not uniform. It was possible to point out a direct relationship between the intensity and duration of the hypotonic effect and the administered dosage.

Domperidone stimulates esophageal motility and increases gastric emptying rates. In the present study the effect of 8 mg domperidone i.v. on antroduodenal motility was investigated double blind and crossover in 10 healthy subjects. Domperidone increased significantly the frequency, amplitude and duration of duodenal and the duration of antral contractions (p less than 0.05). Pyloric motility was not affected. It is concluded that the stimulatory effect of domperidone on antroduodenal motility may contribute to the beneficial effect of the drug in gastroesophageal reflux and gastric ulcer.
