Radiotherapy for cancer is used increasingly. Because skin cells undergo rapid turnover, the ionizing radiation of radiotherapy has collateral effects that are often expressed in inflammatory reactions. Some of these reactions-radiodermatitis and recall phenomenon, for example-are very familiar to dermatologists. Other, less common radiotherapy-associated skin conditions are often underdiagnosed but must also be recognized.

Basal-cell epitheliomas of the scalp are frequently observed on pre-existing chronic lesions: radiodermatitis or Jadassohn's sebaceous naevus. In cases where no irradiation has been given, basal-cell epitheliomas of the scalp should be treated by simple surgical excision. A 10-year follow-up of 10 patients with precancerous cutaneous alterations that could be expected to recur (actinic keratoses, radiodermatitis, leukoplakia) revealed that dermabrasion gave excellent results in four cases and at least some benefit in four others. While planing is not a panacea for the premalignant skin, this study suggests that it is of prophylactic value in the control of this condition in a reasonable proportion of cases.

Late somatic effects of ionizing radiation can be divided into non-stochastic effects with relatively high threshold doses (cataract, fertility problems, chronic radiodermatitis, and radiogenic skin cancer) and non-threshold stochastic effects. Theoretical and epidemiologic aspects of radiocarcinogenesis and various clinical types of radiation-induced neoplasms (leukemia, thyroid cancer, breast cancer) and genetic effects are discussed with special reference to radiation protection measures.

Internal malignancies present with metastases to the skin 0.4% of the time. Cutaneous metastases not uncommonly develop in areas of damaged skin such as radiodermatitis and surgical scars. This report involves a 78-year-old male with metastatic adenocarcinoma from an unknown primary site that presented in a split-thickness skin graft donor site. This appears to be the first reported case of internal malignancy metastatic to skin altered by grafting.
