Pathogenesis and treatment of renal osteodystrophy.

Renal osteodystrophy is the term used to describe the many different patterns of the skeletal abnormalities that occur in patients with chronic kidney disease. The main two conditions are osteitis fibrosa, characterized by high bone turnover, increased osteoclastic and osteoblastic activity, and high levels of circulating parathyroid hormone (PTH) and adynamic bone disease characterized by low bone turnover and low levels of circulating PTH. Retention of phosphorus, decreased levels of calcitriol in blood, decreased levels of serum ionized calcium, reduced numbers of vitamin D receptors and ca

[Plasma exchange in rapidly progressive glomerulonephritis (author's transl)].

The combination of plasma exchange and immunosuppression appears to result in considerable improvement of therapeutic measures in rapidly progressive glomerulonephritis. In contrast to the unfavourable prognosis prior to the introduction of plasma exchange, 8 out of 13 patients (idiopathic rapidly progressive glomerulonephritis, n = 4; Goodpasture syndrome, n = 2; Wegener's granulomatosis, n = 4; lupoid nephritis, n = 3) showed recompensation of terminal renal insufficiency. Treatment of 4 patients who were already anuric led to reversal of renal function in only one case. There was a clear co

Reduced activity of the kallikrein-kinin system predominates over renin-angiotensin system overactivity in all conditions of sodium balance in essential hypertensives and family-related hypertension.

OBJECTIVE: To study the renin-angiotensin-aldosterone and kallikrein-kinin systems in essential hypertensives and offspring of hypertensive … ypertensives but also in the offspring of hypertensive parents. This imbalance probably affects the renal circulation and sodium homeostasis, since there was reduced effective renal plasma flow in both populations compared with normotensive subjects. The positive effect of ACEI, resulting in normalization of the effective renal plasma flow in essential hypertensive …

Proenkephalin predicts kidney function and major adverse kidney events in CKD.

BACKGROUND AND HYPOTHESIS: The endogenous opioid proenkephalin (PENK) has been identified as an independent risk marker for acute cardiovascular diseases and acute kidney injury. However, its regulation, its association with other cardiometabolic markers and its prognostic role in patients with chronic kidney disease (CKD) have not been thoroughly investigated. METHODS: PENK serum levels were cross-sectionally related to renal and cardiometabolic markers across three large cohorts (total N = 4722), including the Leipzig-CKD cohort (N = 581), as well as the population-based LIFE-Adult (N = 309

Fetal hydronephrosis and renal artery blood velocity.

The renal artery blood velocity waveform was recorded by Doppler ultrasound in 17 fetuses with hydronephrosis in the third trimester of preg … operative intervention in the neonatal period. Doppler ultrasound is a valuable tool for evaluating renal circulation, but the results suggest limited clinical value in fetuses with hydronephrosis.

Imaging Manifestations of Hematologic Diseases with Renal and Perinephric Involvement.

The kidneys and perinephric tissues can be affected by a variety of hematologic disorders, which usually occur in the setting of multisystem involvement. In many of these disorders, imaging is used to evaluate the extent of disease, guide biopsy, and/or monitor disease activity and patient response to therapy. Lymphoma, leukemia, and multiple myeloma commonly manifest as multiple parenchymal or perinephric lesions. Erdheim-Chester disease and Rosai-Dorfman disease, rare forms of multisystemic histiocytosis, are often identified as perinephric and periureteral masses. Renal abnormalities depict

Extracorporeal removal techniques for the poisoned patient: a review for the intensivist.

The kidney is able to rapidly eliminate many water-soluble xenobiotics (exogenous chemicals). However, in the face of extraordinary serum concentrations of these xenobiotics or renal dysfunction, alternative elimination techniques often become necessary. Extracorporeal removal (ECR) techniques are used to increase the clearance of xenobiotics. These techniques include hemodialysis (HD), charcoal hemoperfusion (HP), and modalities grouped under the heading of continuous renal replacement therapy (CRRT): continuous venovenous hemofiltration (CVVH) and continuous venovenous hemodiafiltration (CVV

[Angiotensin II and the kidney].

Angiotensin II (AngII) plays a central role for maintenance of GFR and Na balance particularly in volume depletion, when AngII preferentially increases the resistance of efferent arterioles as compared to afferent arterioles, enhancing the glomerular perfusion pressure. In addition, AngII enhances tubular reabsorption of sodium in proximal tubules directly and indirectly as a consequence of glomerulotubular balance. AngII also stimulates Na reabsorption in the collecting ducts by stimulating the release of aldosterone from the adrenal cortex. AngII augments tubuloglomerular feedback. Recently,

Renal transplantation: the normal morphological and Doppler ultrasound examination.

If ultrasound imaging and image-directed Doppler examination of the renal allograft is to provide information with respect to graft complications and graft failure, then a clear understanding of normal characteristics is required. Criteria for abnormality of a graft will include change in size, in parenchymal pattern, and in blood flow characteristics in the main and branch renal arteries and within the renal vein. Normal measurements for the renal allograft are presented as well as morphological and numerical data relating to cortico medullary differentiation. A wide range of values for indic

Sodium-sensitive essential hypertension: emerging insights into an old entity.

Essential hypertension has long been assumed to be a multifactorial disease. However, recent evidence suggests that it is a syndrome rather than a disease with a common symptom--an elevated blood pressure. One large segment of the hypertensive population--approximately 60%--has in common an increased blood pressure sensitivity to salt intake. Further analysis of this subgroup suggests that it is also heterogeneous, consisting of at least six major entities: renal parenchymal disease, bilateral renal artery stenosis, primary aldosteronism, acromegaly, low renin essential hypertension, and the m

A modification of the supraclavicular approach to the central circulation.

Traditional anterior or posterior triangle approaches to the central circulation may be at times unsuccessful in the patient whose anatomy makes cannulation difficult. We used a supraclavicular approach to cannulation of the central circulation in 100 patients for intraoperative monitoring or insertion of renal dialysis catheters. We have had only two failures, both in patients for dialysis catheter placement, and no complications. Review of the literature reveals a similar history of success with variations of this approach. The supraclavicular approach is an easy cannulation technique for th

[Changes in the nervous system in kidney diseases].

A total of 150 patients with renal pathology (chronic glomerulonephritis, pyelonephritis, urolithiasis, etc.) accompanied by the development of psychoneurological disturbances were examined. In the initial stage of the renal insufficiency the authors observed neurasthenic, radicular, polyneuritic, renovisceral syndromes, in a more remote stage--encephalopathies and disturbances of the brain circulation. The important part in the above-mentioned disturbances is played by azotemia, metabolic acidosis, disturbances of fluid-electrolyte and albumine balance, as well as arterial hypertension.

Physiological changes occurring with positive pressure ventilation: Part one.

Critically ill patients requiring mechanical ventilation are subject to a variety of complications and adverse effects associated with positive pressure ventilation. An awareness of the major physiological effects is important, as recognition, prevention and appropriate treatment of complications is critical to optimizing patient outcome. Pierson (1990) suggests that complications due to ventilation occur with greater frequency than is generally appreciated, and can be a response to suboptimal ventilatory management as a result of poor communication and lack of understanding. This may also adv

[Cardiogenic shock following yew needle poisoning : Digoxin immune fab, va-ECMO and albumin dialysis for the treatment of a suicidal yew leaf poisoning].

We present the case of a 46-year-old male who developed refractory bradycardia with cardiogenic shock after attempting suicide by ingestion of yew leaves. Due to delayed availability of the Digoxin immune fab, a va-ECMO was established to maintain sufficient circulation. Administration of the digoxin fab resulted in recovery of spontaneous circulation. Continuous venovenous hemodiafiltration with hemoadsorption and albumin dialysis were initiated with the intention to remove immune fab-toxin complexes and as organ support in acute kidney and liver failure. Impaired Coronary and Renal Vascular Function in Spontaneously Type 2 Diabetic Leptin-Deficient Mice.

BACKGROUND: Type 2 diabetes is associated with macro- and microvascular complications in man. Microvascular dysfunction affects both cardiac and renal function and is now recognized as a main driver of cardiovascular mortality and morbidity. However, progression of microvascular dysfunction in experimental models is often obscured by macrovascular pathology and consequently demanding to study. The obese type 2 diabetic leptin-deficient (ob/ob) mouse lacks macrovascular complications, i.e. occlusive atherosclerotic disease, and may therefore be a potential model for microvascular dysfunction. [The role of venous drainage in uro-genital pathology].

Problems of venous hemodynamics underlie several uro-andrological conditions, such as idiopathic or acquired varicocele, erective impairment from venous leakage, intercaval collateral pathways induction from venous obstructive processes (thrombosis, embolism, external caval compression). Particularly, the renal venous thrombosis seems to be ascribed to a primary membranous glomerulo-nephritis rather than to a reverse mechanism. Moreover, the caval obstruction proximal to the hepatic confluence can induce a portal system's impairment, such as in the Budd-Chiari syndrome. Some diagnostics (US, R
