Spinal disorders lead to paralysis, numbness, pain, etc., due to nerve compression within the spinal canal, affecting quality of life (QOL). Identifying nerve compression is vital for accurate diagnosis. The pulsation of the dura mater, which contains cerebrospinal fluid (CSF) from the choroid plexus, is sometimes evaluated with ultrasound during surgery. Despite its clinical relevance, the physical characteristics of this pulsation remain largely unexplored due to advancements questioning traditional bulk flow theories. This study aimed to analyze the dynamics of dura mater pulsation for a be

The immature autonomic nervous system (ANS) in premature infants regulates heart rate (HR) and respiration different during quiet sleep (QS) and active sleep (AS). Little information is available about ANS regulation in these subjects. The aim of this study was to investigate changes in autonomic regulation and cardiorespiratory coupling during AS and QS in five very preterm neonates with gestational age (GA) 26-31 weeks, applying univariate and bivariate linear and non-linear dynamics methods to the recorded cardiorespiratory signals. During QS univariate linear indices revealed lower standar

BACKGROUND: The Home or Hospital in Heart Failure Study (HHH) is a European Community funded trial (QLGA-CT-2001-02424) which compares usual care of heart failure (HF) with three home-based interventions in a multicenter, multicountry (Italy, Poland and UK), randomized controlled clinical trial. Home telemonitoring (HT) of clinical parameters represents a potential alternative (or addition) to traditional home care models. Nocturnal respiratory disorders (periodic breathing, sleep apnea) are very common in HF, and are associated with increased morbidity and mortality. We developed an integrate

The principal methods of the treatment of the hyperventilation syndrome (HVS) are reviewed. The following methods of the action on the patient are distinguished: psychotherapy, pharmacotherapy, remedial gymnastics, correction of respiratory disorders according to the principle of the biological feed-back mechanism, recurrent respiration or carbon dioxide inhalations. Emphasis is laid on the importance of an individual approach to the institution of the treatment measures. The relationship between the HVS and respiratory diseases is discussed. The authors advance an opinion that every patient m

Thirty healthy volunteers were tested for an extended period of time (up to 4 hours) on modified parallel swings during a head-down tilt (-8 degrees) study. The pattern of motion sickness that developed in this situation was evaluated in terms of physiological parameters (electrocardiography, rheoencephalography, arterial pressure, respiration rate electrooculography, etc.), hormonal status, fluid-electrolyte balance, sensory function. The test subjects developed a vestibulo-hemodynamic syndrome that included vestibular disorders, spatial illusions, fluid displacement in the cranial direction,

We studied maturational process of the photo-evoked eyelid microvibration (PEMV) in 166 newborns, 72 children and 10 adults free of any neurologic disorders. The results indicate that the latency of PEMV shortens during growth due to the myelination of the nerve fibers of the involved pathway. Age-related normal values were established. The pathway of PEMV probably consists of retina, optic nerve, pretectum, facial nerve, and orbicularis oculi muscle. We also evaluated the effect on this reflex of retinoblastoma, facial nerve paralysis, holoprosencephaly anomalad, and a muscle relaxant used fo

The upper airways are a complex structure with multiple functions; many of the muscles participate in the act of respiration. The neural control of upper airway muscles is distinct from that of chest wall muscles under a variety of circumstances. Coordinated activation of upper airway muscles, both regionally and inter-regionally, results in changes in upper airway size and resistance, alterations in the route of airflow, and increases in the ability of the airways to resist collapse. Several disorders have now been described in which neuromuscular drive abnormalities and/or mechanical dysfunc

We determined the respiration rate, respiratory control and ADP/O ratios, with different substrates in mitochondria isolated from seven patients with chronic spinal muscular atrophy and compared them with normal human muscle. In all cases studied, a severe alteration of the respiratory control with variable derangement of oxidative phosphorylation was found. Similar findings have been described in other neuromuscular disorders including the so-called "mitochondrial myopathy". We believe that this disturbance of mitochondrial function is non specific and only the hypermetabolic syndrome of Luft

Breathing can be disrupted by pathological neurological conditions, such as seizures. Ictal respiratory changes comprise central or obstructive apnea, tachypnea, bradypnea, hypoventilation, and hypoxemia. Ictal central apnea (ICA) has been described in association with focal seizures, especially of temporal origin, occurring in 36%-40% of ictal events. Longer ICA duration typically produces hypoxemia, being observed in 33-41% of cases. PCCA (post-convulsive central apnea) has been described in relation to generalized seizures, suggesting brainstem dysfunction following a generalized tonic-clon

While mechanical ventilation (MV) can lead to ventilator-induced diaphragmatic atrophy due to diaphragm inactivity, electrical phrenic nerve stimulation (PNS) can keep the diaphragm active and therefore prevent diaphragmatic weakness. To quantify the effectivity of PNS, an identification experiment during PNS is presented, and its data is used in Gaussian process regression (GPR) of the tidal volume based on the constant voltage amplitude of the stimulation pulses. The measurements were split into training data of variable size and test data for cross validation. For variable training sizes an

PURPOSE OF REVIEW: In numerous neuromuscular disorders (NMDs), respiratory muscle weakness is present, and acute or chronic respiratory failure may evolve. Very often, respiratory involvement substantially adds to the burden of disease, impairs quality of life, or reduces life expectancy. This article summarizes new aspects of both diagnosis and management of respiratory muscle weakness in patients with NMDs.
RECENT FINDINGS: Drugs like deflazacort, ataluren, eteplirsen, and nusinersen are now approved treatments for Duchenne Muscular Dystrophy and Spinal Muscular Atrophy, and others are on t

STUDY OBJECTIVES: The gold standard test for diagnosis of sleep related breathing disorders (SRBD) in children is diagnostic polysomnography (PSG). This is often followed by a titration PSG to identify optimal non-invasive ventilation (NIV) pressures. Access to pediatric PSG is limited, resulting in delays to diagnosis and initiation of treatment. Split-night PSGs (snPSG) combine a diagnostic and titration PSG into a single night study. Although described in adults, the pediatric literature on this topic is sparse. The objective of this study was to describe a large cohort of children who util

The nose exerts many functions, mainly for the respiration and the olfaction and represents the first doorway for the oxygen, but also for pathogens. The present Supplement reports some clinical experiences concerning the use of a new internal nasal dilator in different settings, including nasal obstructive disorders, obstructive sleep apnea syndrome, continuous positive active pressure (CPAP), and sport activity. The outcomes support the concept that a healthy nose should be maintained ever patent and free from secretions, as impaired nasal function can significantly affect quality of life. T

Exhaled nitric oxide (NO) originates from the upper airways, and takes action, to varying extents, in regulation, protection and defense, as well as in noxious processes. Nitric oxide retains important functions in a wide range of physiological and pathophysiological processes of the human body, including vaso-regulation, antimicrobial activity, neurotransmission and respiration. This review article reports the ongoing investigations regarding the source, biology and relevance of NO within upper respiratory tract. In addition, we discuss the role of NO, originating from nasal and paranasal sin
