Intra-arterial thrombolysis for central retinal artery occlusion: a systematic review.

INTRODUCTION: This review provides an updated analysis of the evidence supporting intra-arterial thrombolysis (IAT) for the treatment of central retinal artery occlusion (CRAO). METHODS: A systemic review of the literature indexed by Ovid MEDLINE was performed for the following medical subject headings: central retinal artery occlusion, thrombolysis, intra-arterial, fibrinolysis. Studies were analysed individually and categorised by study type, and extracted data grouped together for statistical analysis. Only studies with five patients or more were included in the review. RESULTS: Twenty-th…

Central retinal artery occlusion in association with an aneurysm of the internal carotid artery.

PURPOSE: To report an unusual case of central retinal artery occlusion in a 41-year-old woman. DESIGN: Interventional case report. METHODS: A 41-year-old woman presented with sudden and painless decrease of visual acuity of the left eye. Indirect ophthalmoscopy revealed the typical fundus findings of a central retinal artery occlusion. The fovea was spared by a cilioretinal arteriole. RESULTS: Cerebral angiography showed an aneurysm of the left internal carotid artery, measuring approximately 1.7 x 1.5 x 1.7 cm. The aneurysm was located in close proximity to the ophthalmic branch. CONCLUSI…

[Central retinal artery occlusion. Importance of selective intra-arterial fibrinolysis].

BACKGROUND: Conventional treatment of a central retinal artery occlusion usually has a poor prognosis but intra-arterial fibrinolysis (IF) of the ophthalmic artery is an invasive treatment option. The importance of IF was evaluated in 62 patients with central retinal artery occlusion and in addition the risk spectrum for this disease was considered. MATERIAL AND METHODS: Data from charts of 62 patients were retrospectively analysed. Visual recovery after IF (n = 17) was compared to conventional treatment (e.g. decrease of IOP, improvement of rheological conditions, n = 45). Patients were excl…

Selective intra-arterial fibrinolysis of acute central retinal artery occlusion.

BACKGROUND AND PURPOSE: Occlusion of the central retinal artery (CRAO) causes a sudden decrease of monocular vision. Because early restoration of blood flow may improve outcome, we attempted to treat CRAO with selective intra-arterial fibrinolysis. METHODS: Intra-arterial fibrinolysis was performed within 6 hours after symptom onset in 17 patients with thromboembolic CRAO. Symptoms were painless, acute and severe decrease of vision. Urokinase (100 000 to 900 000 IU) was given through a microcatheter into the ophthalmic artery over 10 to 90 minutes. For comparison, the history and visual outco…

CENTRAL RETINAL ARTERY OCCLUSION AFTER CARDIOVERSION OF ATRIAL FIBRILLATION.

PURPOSE: To describe a rare case of unilateral central retinal artery occlusion in the postprocedural period of electrical cardioversion (CV) for atrial fibrillation. METHODS: History and ophthalmic examination. RESULTS: A 79-year-old female underwent electrical cardioversion. This resulted in the restoration of sinus rhythm. Ten days after the cardioversion, the patient experienced sudden vision loss in her right eye. She was diagnosed with a right occipital infarct. and a right central retinal artery occlusion. CONCLUSION: Central retinal artery occlusion is a rare complication of cardiov…

Central Retinal Artery Occlusion at a Canadian Academic Center.

BACKGROUND: Central retinal artery occlusion (CRAO) is a retinal stroke with poor visual prognosis and frequent association with life-threatening conditions. Clinical guidelines and treatment options are in evolution, and Canadian data regarding CRAO are limited. METHODS: Patients with CRAO between June 1, 2019, and May 31, 2023, were included. The medical chart was reviewed for demographics, presentation factors, investigations, interventions, secondary prevention referrals and outcomes. RESULTS: Seventy-six patients were included. Median age was 68.1 (61.4-81.8) years, and 60.5% were male.…

Retinal Microvascular Profile of Patients with Coronary Artery Disease.

Background and Objectives: Screening, primary prevention, and the early identification of high-risk individuals are crucial for minimising the burden of cardiovascular diseases (CVDs). In this study, we aimed to evaluate the association of retinal microvascular features with myocardial dysfunction and CVD risk factors in a group of patients with significant coronary artery disease (CAD) compared to patients with newly diagnosed isolated arterial hypertension and healthy controls. Materials and Methods: We performed a single-centre cross-sectional study on 214 individuals divided into three gro

Mitral Papillary Fibroelastoma Revealed by Central Retinal Artery Occlusion.

Papillary fibroelastoma (PFE) is a very rare benign primary cardiac tumor that preferentially affects heart valves and may lead to severe embolic events. Its diagnosis is now easily suggested by echocardiography, and surgical excision is a safe and effective treatment to prevent recurrence. We report the case of a patient referred for the etiological evaluation of a central retinal artery occlusion, in whom transthoracic and transesophageal echocardiography (TEE) revealed a fibroelastoma located on the mitral valve. This case highlights the importance of a comprehensive cardiac evaluation in a…

Clinical characteristics of recurrent non-arteritic retinal artery occlusion.

OBJECTIVES: To investigate the recurrent non-arteritic retinal artery occlusion (RAO) in the same or opposite eye. METHODS: We searched the RAO registry at Seoul National University Bundang Hospital and included patients with recurrent RAO in the present study. Ophthalmic and systemic features were analysed to identify risk factors and visual outcomes. RESULTS: Of the 850 patients in the non-arteritic RAO cohort, 11 (1.3%) experienced a second RAO recurrence, either in the same (5 patients; 0.6%) or opposite (6 patients; 0.7%) eye. The same eye group experienced an earlier recurrence (1-2 mo…

Branch Retinal Artery Occlusion Following Rhinoplasty: A Case Report.

Central retinal artery occlusion (CRAO) is a sudden and vision-threatening condition with catastrophic consequences unless managed immediately by reestablishing the retinal circulation. Even though CRAO is a common ocular disorder, it is a very rare complication after non-ocular surgeries; only a few cases have been reported in the literature. Cardiac and spine surgeries are considered the most common causes of postoperative vision loss. In this case report, we present the case of a young female patient diagnosed with central retinal artery occlusion after a septoplasty. This is considered the…

Progress in central retinal artery occlusion: a narrative review.

Central retinal artery occlusion (CRAO) is a catastrophic ophthalmic emergency that severely impairs a patient's visual function, often reducing visual acuity to counting fingers or worse. Progress in CRAO research has provided new information regarding its epidemiological characteristics and led to useful assessments through various ophthalmic examinations. Additional insights about CRAO have been gained through studies of its pathophysiological mechanisms, improving intervention timing and enhancing patient prognosis. Treatment for CRAO has evolved, particularly with assistance from surgical…

Interventions for acute non-arteritic central retinal artery occlusion.

BACKGROUND: Acute non-arteritic central retinal artery occlusion (CRAO) occurs as a sudden interruption of the blood supply to the retina and typically results in severe loss of vision in the affected eye. Although many therapeutic interventions have been proposed, there is no generally agreed upon treatment regimen. OBJECTIVES: To assess the effects of treatments for acute non-arteritic CRAO. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (which contains the Cochrane Eyes and Vision Trials Register) (2022, Issue 2); Ovid MEDLINE; Embase.com; PubMed;…
