Characteristics of optical coherence tomographic hyperreflective foci in retinal vein occlusion.

PURPOSE: To evaluate the hyperreflective foci in branch retinal vein occlusion and central retinal vein occlusion depicted by spectral-domain optical coherence tomography (OCT). METHODS: Consecutive series of 73 eyes of 73 patients with retinal vein occlusion (58 branch retinal vein occlusion and 15 central retinal vein occlusion) who had Spectralis OCT images obtained were retrospectively reviewed, comparing color fundus photographs and fluorescein angiography. RESULTS: Hyperreflective foci were detected in 54 eyes (74.0%) by spectral-domain OCT, and hard exudates were detected in 17 eyes (…

Hyperhomocysteinemia and C677T MTHFR genotype in patients with retinal vein thrombosis.

INTRODUCTION: Elevated homocysteine (Hcy) is associated with the risk of deep vein thrombosis, pulmonary embolism, ischemic heart disease, and stroke. Several studies have suggested that hyperhomocysteinemia (HHcy) may predispose to retinal vein thrombosis (RVT) development. The aim of this study is to investigate the relationship between Hcy, C677T methylenetetrahydrofolate reductase (MTHFR) genotype, and RVT in patients compared with controls. MATERIALS AND METHODS: We evaluated the Hcy plasma level of 3114 consecutive participants in 2 Italian centers during a 2-year period. Hyperhomocyste…

[Antiphospholipid syndrome and bilateral retinal artery and vein occlusion: case report].

The antiphospholipid syndrome (APS) has been related to venous and arterial thrombosis of large and small vessels. Ocular vessels can also be involved causing transient visual loss, diplopia, ischemic optic neuropathy and central artery and venous occlusion. We describe here a case of antiphospholipid syndrome with occlusion of central retinal artery and vein to call the attention to this diagnosis.

Effect of nitroprusside on arteriolar constriction after retinal branch vein occlusion.

PURPOSE: The development of extended areas of nonperfused capillaries after branch vein occlusion (BVO) is correlated to the secondary constriction of the arteriole crossing the occluded area. The decrease in nitric oxide (NO) in tissue that occurs early after BVO accounts for the secondary arteriolar constriction. The present study shows that the administration of an NO donor can reverse the secondary arteriolar vasoconstriction observed after BVO. METHODS: Simultaneous preretinal NO profiles and arteriolar diameter measurements were performed in miniature pigs after experimental BVO. The ef

Preretinal neovascularization induced by experimental retinal vein occlusion in albino rats.

Retinal ischemia and neovascularization have been demonstrated in several animal models. To determine 1) whether the retinal or preretinal neovascularization can be induced in albino rats by retinal vein occlusion and 2) the type and rate of occurrence on neovascularization, we occluded retinal veins in albino rats by photodynamic thrombosis. After anesthesia, each of 36 rats received an injection of rose bengal photosensitive dye, and their veins underwent argon green laser treatment. Half or all the major retinal veins were occluded in 12 eyes and in 24 eyes, respectively. Ten control rats u…

Central retinal vein occlusion associated with drusen of the optic disc.

A 43-year-old man had gradual visual changes OD. Funduscopic examination revealed central retinal vein occlusion OD. Complete ocular examination revealed bilateral drusen of the optic disc, confirmed by b-scan ultrasonography. Complete systemic evaluation showed no other etiology for the central retinal vein occlusion. Over a ten-year follow-up, in the absence of systemic disease or other local ocular anomalies, we concluded that central retinal vein occlusion can arise from external compression secondary to drusen of the optic disc.

Hemi-central retinal vein occulsion. Pathogenesis, clinical features, and natural history.

A two-trunked central retinal vein (CRV) in the anterior part of the optic nerve may persist as a congenital abnormality in a certain proportion of humans. One of the two trunks, like the CRV, may get occulded in the optic nerve to produce hemi-CRV occulsion (hemi-CRVO). It is shown that hemi-CRVO is a distinct entity, clincially and pathogenetically closely related to CRVO, and unrelated to branch retinal vein occlusion because of fundamental differences between the two. Hemi-CRVO clinically presents as either venous stasis retinopathy (VSR) or as hemorrhagic retinopathy (HR), usually involvi…

Increased intraocular inflammation in retinal vein occlusion is independent of circulating immune mediators and is involved in retinal oedema.

We aim to understand the link between systemic and intraocular levels of inflammatory mediators in treatment-naïve retinal vein occlusion (RVO) patients, and the relationship between inflammatory mediators and retinal pathologies. Twenty inflammatory mediators were measured in this study, including IL-17E, Flt-3 L, IL-3, IL-8, IL-33, MIP-3β, MIP-1α, GRO β, PD-L1, CD40L, IFN-β, G-CSF, Granzyme B, TRAIL, EGF, PDGF-AA, PDGF-AB/BB, TGF-α, VEGF, and FGFβ. RVO patients had significantly higher levels of Flt-3 L, IL-8, MIP-3β, GROβ, and VEGF, but lower levels of EGF in the aqueous humor than cataract…

Retinal microstructural disorganization in persistent macular edema secondary to branch retinal vein occlusion: an adaptive optics OCT study.

Persistent macular edema (ME) secondary to branch retinal vein occlusion (BRVO) may show a limited anatomical response to anti-vascular endothelial growth factor (VEGF) therapy despite apparent control of vascular leakage. In this non-interventional observational study, we examined whether retinal microstructural alterations beneath foveal cystoid spaces are associated with anatomical characteristics of persistent macular edema following standard anti-vascular endothelial growth factor therapy. Thirty eyes of 30 patients with persistent BRVO-related ME (≥ 6 months after onset) underwent adapti…

Unilateral branch retinal vein occlusion and contralateral branch retinal artery occlusion: a case report and comprehensive literature review.

Branch retinal vein occlusion (BRVO) and branch retinal artery occlusion (BRAO) are two distinct yet serious retinal vascular disorders that often present with acute visual impairment. While both conditions are commonly encountered independently, their simultaneous or sequential occurrence in opposite eyes of the same patient is exceedingly rare and poses unique diagnostic and therapeutic challenges. Here, we present the case of a 65-year-old male patient with a history of systemic hypertension who developed BRAO in the right eye and BRVO in the left eye within a short time interval. Detailed …
