[Indications for the treatment of bone rarefactions of endodontic origin].

Bone rarefactions arising from dental lesions are commonly thought to be sustained by the microbiol component or by the toxins spread out into the surrounding bone by the disintegrated pulp. In thise case endodontic therapy is the only one capable of obtaining a full recovery. In our opinion the most suitable endodontic procedure is that described by Schilder: with his technique infact a great deal of substantial results can be achieved. Bone rarefactions are to be surgically treated only when endodontic therapy does not guarantee the complete removal of microrganisms and that of the tissue de

Effect of continuous vs sequential chelation on the mechanical properties of root dentin: An ex vivo study.

OBJECTIVES: To evaluate the mechanical properties of root canal dentin treated with sodium hypochlorite (NaOCl) in combination with hydroxyethylidene diphosphonic acid (HEDP) or ethylenediaminetetraacetic acid (EDTA). METHODS: For testing fracture resistance, 45 single-rooted teeth were instrumented and irrigated with NaOCl/HEDP, NaOCl/EDTA, or distilled water. Fifteen untreated teeth served as control. After obturation, specimens from the experimental groups were thermocycled, dynamically-loaded, and then statically-loaded in a universal testing machine until failure. For flexural strength a

Nonsurgical root canal therapy treatment with apparent indications for root-end surgery.

The recent introduction of the surgical microscope to the practice of endodontics, especially for surgery, has allowed clearer visualization of the periapex during root-end resection and filling. However, despite this and other technologic advances, it has not been demonstrated that in the absence of thorough canal debridement the success rate of periapical surgery has improved over the 50% to 60% demonstrated in most long-term prognosis studies. Therefore it remains important to fully instrument and obturate the root canal system with conventional therapy before surgery is considered; this co

Non-surgical endodontic treatment for dens invaginatus type III using cone beam computed tomography and dental operating microscope: a case report.

Dens invaginatus is a morphological abnormality of the tooth in which the coronal tooth enamel and dentin fold inwards towards the pulp cavity. Dens invaginatus type III (Oehlers: 1957) is characterized by infolding of the enamel and dentin as far as the root apex. This report describes a case of surgical and non-surgical endodontic therapy for a maxillary lateral incisor with type III dens invaginatus, necrotic pulp, and an associated large periradicular lesion. The patient was a 16-year-old man. Periapical radiographs suggested the presence of an untreated area of invagination. Cone beam com

The use of limited cone beam computed tomography in the diagnosis and management of a case of perforating internal root resorption.

AIM: To describe the use of cone beam computed tomography (CBCT) in the diagnosis and management of a maxillary lateral incisor with perforating internal root resorption (IRR). SUMMARY: Root resorption is the loss of dentine or cementum as a result of osteoclastic cell action. IRR occurs exclusively as a result of pulpal inflammation. Until very recently, the diagnosis of internal and external resorptive defects has been limited to the information obtained from conventional radiographic techniques. This case report describes the use of CBCT in the diagnosis and treatment planning of a case of

Impact of a biological barrier in pulpectomies of primary molars.

PURPOSE: The purpose of this in vitro study was to determine whether placement of a resorbable collagen barrier at the apical one third of the root canal could prevent extrusion of pulpectomy filling material. METHODS: Twenty-five extracted primary mandibular second molars, with at least two thirds of root length remaining, were used. The apical third of each root was covered with utility wax, and teeth were embedded in acrylic. Carious tooth structure was removed, crowns were reduced to a flat surface, and canals were accessed and preflared. Working lengths were established by reducing file

Diagnosis and root canal treatment in a mandibular premolar with three canals.

This paper presents a case report of a left mandibular second premolar with three canals and three different apical foramina. A 39-year-old male patient presented to our clinic with pain in the mandibular left second premolar. Initially, pain was caused by cold stimulus and later was spontaneously. The intraoral clinical examination revealed a fractured amalgam restoration with occlusal caries. Percussion and cold (Endo-Frost) tests were positive. The radiographic examination showed the presence of two roots. The probable diagnosis was an acute pulpitis. After access cavity, it was observed re

An in vitro model to investigate filling of lateral canals.

Aims of this work were to examine lateral canals in extracted teeth, to propose a new technique to produce artificial lateral canals, and to compare two obturation techniques. Cleared roots were examined to record measure and shape of lateral canals. Artificial lateral canals were prepared on human demineralized teeth before final clearing. Specimens were divided in two groups: canals of group 1 were filled with Schilder's technique, canals of group 2 were filled with vertical compaction with apical backfilling. Stereomicroscopic analysis of lateral canal filling revealed lower filling rates i

Clinical strategies for successful palato-mesiobuccal canal management-Report of 2 cases.

AIM: To describe the management of the palato-mesiobuccal (PMB) canal in maxillary second molars with fused roots using conventional techniques. SUMMARY: Root canal treatment success hinges upon effectively addressing the intricate and variable anatomy of molar teeth. Failure to do so can lead to persistent infections and treatment failure. Recent advancements in imaging technologies have provided unparalleled insights into dental anatomy, especially in molars. Among these discoveries is the PMB canal, a unique anatomical variant recently reported for the first time in Endodontic literature.

Emerging Paradigms in Internal Root Resorption Management: Harnessing the Power of Bioceramics.

Internal root resorption is a pathological activity involving dentin deterioration within the root canal walls. Numerous variables, including traumatic injury, infection, and orthodontic therapy, can trigger this process. Traditional materials such as mineral trioxide aggregate (MTA) have been utilized to treat internal root resorption but have limitations such as tooth discoloration and handling challenges. Bioceramic materials, such as Bio-C Repair, have emerged as possible MTA substitutes. This case study outlines the effective management of idiopathic extensive perforating internal root re
