Patterns of health-care utilization and morbidity in a rural community near Lahore, Pakistan.

Patterns of health-care utilization and of morbidity were investigated in a demographically defined community: the 1400 inhabitants of a rural village near Lahore, Pakistan. The data collected, by semi-monthly clinic surveys from February 1982 to January 1983, showed that 42% of the study population sought health care during the year-long study period, between one and 10 (mean 2.1) times each. Females attended the clinics twice as often as males. The 1193 person-visits that were recorded at the field clinics yielded a total of 1354 cases of various diseases. The cumulative number of clinic-vis

Women's and health-care providers' views of maternal practices and services in rural Nigeria.

Maternal mortality and morbidity estimates in Nigeria continue to be dramatically high largely because maternal services, especially in rural areas, are often deficient and inappropriate to women's situations. The Safe Motherhood Project in Zone A examined the pregnancy-related knowledge, attitudes, and practices of community members, and women's use of community maternal health services. Focus-group discussions and interviews confirmed a number of recent findings by other studies; they also documented extensive hostility between the two most commonly used health-care providers: traditional bi

Intestinal parasitic infestation in rural communities: a focus for primary health care in Nigeria.

A study carried out in four villages in two local government areas of Oyo State in the southwestern part of Nigeria, revealed that intestinal helminthiasis is still very prevalent in Nigeria. Ascaris is the most common helminth encountered and multiple infestation is also quite common. Schoolchildren and pre-school children are the greatest sufferers of Ascaris, while students and farmers are the greatest target for hookworm. The usefulness of a community-based distribution programme of health services using trained traditional birth attendants and voluntary health workers for data collection

The training of traditional birth attendants in Ghana: experience of the Danfa Rural Health Project.

Three quarters of deliveries in Ghana are still attended by untrained personnel, including traditional birth attendants (TBA's), and maternal mortality remains high. The Ministry of Health is exploring ways of improving this situation. Recently, training traditional birth attendants to improve maternal and child health practices and promote family planning has been considered. Most TBA's in rural Ghana are elderly illiterate farmers. The Danfa Project has identified and trained 57 of these TBA's with the objective of developing a programme that might be replicated elsewhere in the country. Thi

A health and social survey of a group of children from rural New South Wales.

Between the middle of May and middle of July, 1972, 177 children aged up to 15 years were admitted to a nursing home in Sydney on referral from their doctors in country areas of New South Wales for specialized investigation and treatment mainly of respiratory infections, congenital malformations and orthopaedic and ophthalmic conditions. Specimens of faeces from these children were examined for bacterial pathogens and parasites. These tests revealed the presence of intestinal parasites (mostly Giardia lamblia) in 91 of the children and of bacterial pathogens in 14 (a salmonella in one and shig

Voices from the field: perspectives from community health workers on health care delivery in rural KwaZulu-Natal, South Africa.

BACKGROUND: As the primary link between the formal health care sector (clinics, physicians, and nurses) and the household level in South Africa, community health workers (CHWs) provide a critical perspective on barriers that exist in rural health care delivery. METHODS: A 30-question written questionnaire assessing CHW perspectives on human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS), tuberculosis (TB), and potential CHW program improvement was administered to 120 CHWs in the Outer West Region of KwaZulu-Natal, South Africa. Interviews with CHWs, administrators of t

New approaches to sexual health services in a rural health board area: involving service users and primary care professionals.

Research was undertaken to inform the future development of sexual health services for the population of a rural health board. Three separate surveys were carried out: (i) users of local and regional genitourinary medicine (GUM) services, (ii) users of local family planning clinics, and (iii) all local general practitioners (GPs) and practice nurses-to obtain views on current sexual health services and the optimum future configuration of services. Respondents to the GUM survey (n=137) attended GUM service for a range of sexual health reasons; confidentiality was the most important factor affec

Barriers and facilitators of cervical cancer screening literacy among rural women with HIV attending rural public health facilities in East Central Uganda: a qualitative study using the integrated model of health literacy.

BACKGROUND: Several rural public health facilities in East Central Uganda have sub-optimal, below 50%, levels of uptake of cervical cancer screening services among women with HIV. This is attributed to low cervical cancer screening literacy: limited ability to access, understand, appraise, and apply cervical cancer screening information. This research identified multi-level (health facility, community, interpersonal and individual) barriers, and facilitators of accessing, understanding, and applying cervical cancer screening information among rural women with HIV attending rural public health

The association between health insurance status and utilization of health services in rural Northern Ghana: evidence from the introduction of the National Health Insurance Scheme.

BACKGROUND: Many households in low- and middle-income countries face financial hardships due to payments for health care, while others are pushed into poverty. Risk pooling and prepayment mechanisms help to lessen the impact of the costs of care as well as assisting to achieve universal health coverage (UHC). Ghana implemented the National Health Insurance Scheme (NHIS) for the promotion of access to health services for all Ghanaians. In this paper, we examined the association between health insurance status and utilization of outpatient and inpatient health services in rural poor communities.

Integration of Behavioral Health by Community Health Workers Into a Lifestyle Intervention That Addresses Reduction of Risk for Cardiovascular Disease in Rural Communities.

PURPOSE AND OBJECTIVES: Cardiovascular disease (CVD) is the leading cause of death and disability in the US, with higher rates in rural areas. Although community health workers (CHWs) are effective in improving CVD outcomes, limited evidence exists on their role in addressing behavioral health needs within lifestyle change interventions. This evaluation assessed the feasibility of integrating behavioral health components focusing on psychologic distress, anxiety, and depression into the Colorado Heart Healthy Solutions program (CHHS) in rural communities by using 3 domains of the RE-AIM (Reach

Telepsychiatry and Rural Mental Health Access in U.S. Health Care: An Integrative Review of Appointment Access, Utilization, and Care Continuity.

BACKGROUND: Rural mental health care access in the United States (U.S.) has remained limited because of psychiatrist shortages, geographic isolation, transportation barriers, and long appointment wait times. Sixty-five percent of nonmetropolitan counties lack a practicing psychiatrist, rising to 80% in the most remote noncore counties. The objective of this work was to evaluate whether telepsychiatry was associated with appointment access, health care utilization, appointment completion, and continuity of care among rural U.S. populations between 2015 and 2026. METHODS: An integrative review

The Relationship between Health Literacy, Quality of Life, and Subjective Health: Results of a Cross-Sectional Study in a Rural Region in Germany.

Low health literacy is associated with poorer health and quality of life. An open question is whether a regional integrated healthcare system whose management strives to enable and empower its members with regard to health issues can contribute to a higher level of health literacy in the population. Against this background, in a cross-sectional study we surveyed a random selection of members of an integrated healthcare system in southwestern Germany (n = 180). The standardized questionnaire included, inter alia, questions on health literacy and subjective health. In this sample we wanted to (1

Rural Dermatology in the Subtropics: A Cross-Sectional Study of Patterns of Dermatoses, Follow-Up Visits, and Treatment Compliance at a Rural Health Center in North India.

Dermatological conditions pose a significant global public health burden, with a paucity of data from rural-based dermatology services providing longitudinal care. The present prospective study was conducted between November 1, 2022 and April 30, 2023, to study the pattern of dermatoses, treatment adherence, and follow-up among patients seen by the dermatology service at a rural health center in north India. After appropriate ethical clearance and informed consent, all patients were screened for dermatological diseases, and clinicodemographic details were recorded. On subsequent visits, compli

Community Coalitions Implementing and Sustaining Policy, Systems and Environment Strategies for Rural Health Equity.

Rural communities have assets that can be leveraged to address health disparities, particularly through complex policy, systems and environment (PSE) change approaches. Eleven rural community coalitions across Georgia participated in The Two Georgias Initiative, a 5-year health equity project from 2017 to 2022 designed to address rural-urban health disparities. The coalitions addressed community health concerns through implementing county-specific PSE and programmatic strategies tracked through a Community Change Tracking Tool. This mixed-methods analysis assessed the variation in strategies i

Multidimensional stress among rural older adults in China: status, typology, and mental health associations.

BACKGROUND: With the intensification of population aging in China, research on stress among older adults and its associations with mental health has become increasingly important. While previous studies have examined economic and health stress among older adults, research on stress related to caregiving, family relations, and children’s development remains insufficient. Therefore, this study aimed to classify stress and its factors, and further investigate the relationship between stress and mental health. METHODS: Based on data of 728 participants from the “Comprehensive Social Survey on the
