[Management of patients with dyspepsia].

Dyspepsia is a common clinical problem with an extensive differential diagnosis and a heterogeneous pathophysiology. Dyspepsia affects up to 40 % of the general population and significantly reduces quality of life. According to the Rome III criteria, dyspepsia is defined as one or more of the following symptoms: epigastric pain and/or burning (classified as epigastric pain syndrome), postprandial fullness and/or early satiation (classified as postprandial distress syndrome). Initial evaluation should focus on the identification and treatment of potential causes of symptoms such as gastroesopha…

Inhibition of cocaine self-administration by fluoxetine or D-fenfluramine combined with phentermine.

Instrumental responding for intravenous cocaine in rats at 85% of free-feeding weight was significantly decreased 50% by D-fenfluramine plus phentermine (D-Fen/Phen, 5 mg/kg of each for 1 day). A similar effect was obtained in normal-weight rats self-administering a cocaine-heroin mixture. Treating normal-weight animals with fluoxetine (5 mg/kg) for 4 days also significantly decreased cocaine self-administration by half, and then adding phentermine caused an additional decrease in cocaine intake. Animals that were well trained to self-administer drug did not self-administer intravenous D-Fen/P

Neurochemical mechanism of action of anorectic drugs.

Studies with dexfenfluramine, an anorectic agent which releases 5-hydroxytryptamine (5-HT) from nerve terminals and inhibits its reuptake, have considerably increased our knowledge of the role of 5-HT in feeding control. 5-HT1B receptors mediate the satiating effect of dexfenfluramine, whereas the mechanism by which 5-HT uptake inhibitors such as fluoxetine and sertraline cause anorexia is not clear. Anorexia induced by (+)-amphetamine, phentermine, diethylpropion and phenylpropanolamine seems to be the result of their ability to increase the release of noradrenaline and/or dopamine from nerve

Effects of antianxiety and antipsychotic drugs on DRL responding for brain stimulation.

Satiated rats could be trained to give stable rates of responding for rewarding stimulation of the lateral hypothalamus delivered on differential reinforcement of low rate (DRL) schedule requiring 2 to 8 sec interresponse intervals for reinforcement (DRL-2 to 8). The performance on a DRL-8 schedule was tested 30 min after the oral administration of benzodiazepines. Diazepam (5 and 10 mg/kg) and meprobamate (200 mg/kg) caused significant increases in response rates during the first 5 min of a session, but not thereafter. Bromazepam (1 and 5 mg/kg) also caused a significant increase in the rates

System organization of alimentary behavior in the newborn and the developing cat.

This review deals with the structural, behavioral, and EEG correlates of searching for food and food intake. Data were collected on freely b … of 3-6 or 6-8 cps; and the behavioral correlate is purposeful search for the mother's teat or food. Satiation is associated with activity of the synchronizing structures in the forebrain and lower brain stem, as well as in the relay nuclei that transmit the sensory flow from oral afferents; EEG activity consists of regular rhythms of 10-14 and 2-4 cps; and …

Central amylin acts as an adiposity signal to control body weight and energy expenditure.

The pancreatic B-cell hormone amylin has been proposed to be both a satiation signal and an adiposity signal. The effects of peripheral amylin on energy balance are well investigated, but the effects of central amylin are less clear. We determined the effects of low doses of amylin administered into the 3rd cerebral ventricle (i3vt) on food intake, body weight and other indices of energy balance. Amylin (2 pmol/h) significantly lowered body weight compared to saline after 2 weeks of infusion, independent of whether prior body weight was decreased by fasting, increased by voluntary overfeeding …

Control of thirst and salt appetite in rats: early inhibition of water and NaCl ingestion.

Most previous studies on the controls of thirst and salt appetite in rats have focused on systemic factors. Our recent experiments suggest t … lasma osmolality or decreased plasma volume), and therefore it resulted from inhibition rather than satiation. This inhibition appeared to derive from signals related to the cumulative volume of ingested fluid in the stomach and small intestine. Similar findings were obtained in studies of NaCl solution intake by NaCl-deprived adrenalectomized rats. These a …

Rumination and vomiting in the developmentally disabled: a critical review of the behavioral, medical, and psychiatric treatment research.

Medical and psychiatric research regarding the treatment of vomiting and rumination among developmentally disabled individuals was selective … ntal support including oral hygiene, differential reinforcement of incompatible behaviors, and food satiation procedures. These behavioral interventions are the treatments of choice when organic causes of rumination cannot be identified. Suggestions for future research and applications are discussed.

Chronic pramlintide decreases feeding via a reduction in meal size in male rats.

Amylin, a pancreatic hormone, is well-established to suppress feeding by enhancing satiation. Pramlintide, an amylin analog that is FDA-approved for the treatment of diabetes, has also been shown to produce hypophagia. However, the behavioral mechanisms underlying the ability of pramlintide to suppress feeding are unresolved. We hypothesized that systemic pramlintide administration in rats would reduce energy intake, specifically by reducing meal size. Male rats were given b.i.d. administration of intraperitoneal pramlintide or vehicle for 1 week, and chow intake, meal patterns, and body weigh…

Type-2 Diabetes, Pancreatic Amylin, and Neuronal Metabolic Remodeling in Alzheimer's Disease.

Type-2 diabetes raises the risk for Alzheimer's disease (AD)-type dementia and the conversion from mild cognitive impairment to dementia, yet mechanisms connecting type-2 diabetes to AD remain largely unknown. Amylin, a pancreatic β-cell hormone co-secreted with insulin, participates in the central regulation of satiation, but also forms pancreatic amyloid in persons with type-2 diabetes and synergistically interacts with brain amyloid β (Aβ) pathology, in both sporadic and familial Alzheimer's disease (AD). Growing evidence from studies of tumor growth, together with early observations in ske…

Viral depletion of calcitonin receptors in the area postrema: A proof-of-concept study.

The area postrema (AP), located in the caudal hindbrain, is one of the primary binding sites for the endocrine satiation hormone amylin. Amylin is co-secreted with insulin from pancreatic ß-cells and binds to heterodimeric receptors that consist of a calcitonin core receptor (CTR) paired with receptor-activity modifying protein (RAMP) 1 or 3. In this study, we aim to validate a CTR-floxed (CTRfl/fl) mouse model for the functional and site-specific depletion of amylin/CTR signaling in the AP and the nucleus tractus solitarius (NTS). CTRfl/fl mice were injected in the NTS with adeno-associated v…

Habitual Preference for the Nondrug Reward in a Drug Choice Setting.

For adaptive and efficient decision making, it must be possible to select between habitual alternative courses of action. However, research in rodents suggests that, even in the context of simple decision-making, choice behavior remains goal-directed. In contrast, we recently found that during discrete trial choice between cocaine and water, water-restricted rats preferred water and this preference was habitual and inflexible (i.e., resistant to water devaluation by satiation). Here we sought to test the reproducibility and generality of this surprising finding by assessing habitual control of…

New developments in the treatment of gastroparesis and functional dyspepsia.

Functional dyspepsia (FD) and gastroparesis are frequent causes of upper gastrointestinal symptoms such as postprandial fullness, early satiation, epigastric pain or burning, upper abdominal bloating, bothersome belching, nausea and vomiting. The underlying pathophysiological mechanisms are heterogeneous and involved mechanisms such as abnormal gastric motility (accommodation, emptying), visceral hypersensitivity, low grade mucosal inflammation and cellular changes in enteric nerves, muscle or interstitial cells of Cajal. Patient-reported outcomes for evaluating treatment efficacy in these con…

Differential effects of chronic social stress and fluoxetine on meal patterns in mice.

Both chronic stress and antidepressant medications have been associated with changes in body weight. In the current study, we investigate me … fluoxetine. Taken together, these results indicate that stress increases food intake by decreasing satiation, and that fluoxetine can reverse stress-induced changes in meal patterns.

Early life exposures are associated with appetitive traits in infancy: findings from the BiTwin cohort.

PURPOSE: To determine the stability of appetitive traits during infancy and their association with early life exposures. METHODS: Participants were from the BiTwin birth cohort (longitudinal study of Portuguese infants). Appetitive traits at 3 months were measured using the Baby Eating Behaviour Questionnaire (n = 347) and at 12 months with the Children's Eating Behaviour Questionnaire for toddlers (n = 325). Stability was assessed with multi-level models. The association of early life exposures (weight for gestational age, mode of feeding, prematurity, smoking during pregnancy, maternal pre-

Habituation as a determinant of human food intake.

Research has shown that animals and humans habituate on a variety of behavioral and physiological responses to repeated presentations of food cues, and habituation is related to amount of food consumed and cessation of eating. The purpose of this article is to provide an overview of experimental paradigms used to study habituation, integrate a theoretical approach to habituation to food based on memory and associative conditioning models, and review research on factors that influence habituation. Individual differences in habituation as they relate to obesity and eating disorders are reviewed,

Sensory processing in the brain related to the control of food intake.

Complementary neurophysiological recordings in rhesus macaques (Macaca mulatta) and functional neuroimaging in human subjects show that the primary taste cortex in the rostral insula and adjoining frontal operculum provides separate and combined representations of the taste, temperature and texture (including viscosity and fat texture) of food in the mouth independently of hunger and thus of reward value and pleasantness. One synapse on, in the orbitofrontal cortex, these sensory inputs are for some neurons combined by learning with olfactory and visual inputs. Different neurons respond to dif

Normal values for the satiety drinking test in healthy children between 5 and 15 years.

In adults, a slow caloric drinking test has been proposed as a non-invasive tool to estimate gastric accommodation and to quantify meal-induced symptoms in functional dyspepsia (FD). The same test has been proposed for paediatric FD, but normal values are only available for adolescents and adults. The aim of the study was (i) to establish normal values for the satiety drinking test in young children and (ii) to study the influence of demographic factors. In all, 59 healthy children [27 girls; age range 5-16 years, body mass index (BMI) 17.4 +/- 2.5 kg m(-2)] were studied in the morning after a

Low serotonin and dopamine metabolite concentrations in cerebrospinal fluid from bulimic patients with frequent binge episodes.

Cerebrospinal fluid neurotransmitter metabolite levels were studied to assess whether measures of central serotonin, dopamine, or norepinephrine function are associated with severity of abnormal eating patterns in patients with bulimia nervosa. In comparison with healthy controls (N = 17), hospitalized bulimic patients with a history of binge eating more frequently than twice daily (N = 11) had significantly lower CSF concentrations of 5-hydroxyindoleacetic acid and homovanillic acid. For the total patient group (N = 29), levels of both metabolites were significantly inversely correlated with

Perceptivity and paraceptivity during measurement of gastric emptying in anorexia and bulimia nervosa.

Gastric emptying of a mixed meal was measured in 22 patients with anorexia nervosa, ten with bulimia nervosa and ten controls. Ratings on a visual analogue scale (VAS) were made during the test. Patients with anorexia nervosa felt fuller and less hungry than controls, although satiety was not related to gastric emptying rate, controls showed a correlation of about 0.6 between gastric contents and reported hunger and satiety. Patients with anorexia nervosa had significantly lower correlations between gastric contents and hunger, but normal correlations with fullness. Significant correlations we
