In an attempt to avoid the complications associated with taking autologous bone graft for use in the treatment of non-union of the scaphoid, a clinical trial of the use of totally de-proteinized heterologous bone graft has been carried out. Ten patients with established non-union of the scaphoid were treated with internal fixation and grafting using one such commercially available material, Pyrost. Bipartite carpal scaphoid is a rare congenital anomaly. The authors report on a 56-year-old man who presented with bilateral wrist pain without a history of trauma. X-ray films demonstrated bilateral symmetric bipartition of the scaphoid into a large distal ossicle and a smaller proximal ossicle. As a source of corticocancellous grafts for treating scaphoid nonunions, the anterolateral corner of the distal radial metaphysis has several advantages over other alternatives: it provides good-quality corticocancellous bone, it allows one to harvest the graft and treat the scaphoid through the same incision, it does not require general anesthesia, and it has less morbidity than occurs when obtaining the graft from the iliac crest.

OBJECTIVE: To evaluate the clinical and radiological outcome of anterior vascularized bone graft in the treatment of primitive nonunion of the scaphoid.
METHODS: This is a retrospective study of 26 scaphoid nonunion cases treated by this technique between 2004 and 2009. The mean age was 38 years. Nonunion was of types IIA and IIB according to the Alnot's classification and only one case showed a proximal pole necrosis (type IV). The fixation was ensured by K-wire.
