Posterior scleritis after biweekly aflibercept intravitreal injections.

PURPOSE: To describe a case of posterior scleritis following anti-vascular endothelial growth factor (VEGF) injection. OBSERVATIONS: A 51-year-old male receiving biweekly anti-VEGF injections for a central retinal vein occlusion complicated with cystoid macular edema presented with new-onset severe pain, photophobia, and tearing. Ophthalmological examination revealed choroidal folds and B-scan ultrasound confirmed significant thickening of the sclera, leading to a diagnosis of posterior scleritis. This resolved under topical and systemic corticosteroids. CONCLUSIONS AND IMPORTANCE: Intravitr

Scleritis: Differentiating infectious from non-infectious entities.

Scleritis is a rare painful ocular disorder, associated with severe ocular pain and tissue destruction. Although a majority of these cases are immune mediated and at least half of these are associated with systemic immune-mediated diseases, a smaller minority are due to infections of the sclera. The two conditions closely mimic each other, and a thorough knowledge of the subtle differences is necessary in order to reach a timely diagnosis. Diagnostic delay can lead to a poor outcome both due to the destruction caused by the uncontrolled infection and also due to propagation of the infection wi

IgG4-Related Disease Presenting as Isolated Scleritis.

A rare case of IgG4-related disease (IgG4-RD) manifesting as nodular scleritis is presented in a 20-year-old female. Patient complained of left eye pain and redness for one week. Ocular examination together with ancillary testing led to the diagnosis of nodular scleritis. Since the patient did not show apparent improvement after one week of systemic steroidal treatment, she underwent a biopsy of the affected area revealing histopathological characteristics of IgG4-RD. Long-term treatment with corticosteroids and a steroid-sparing agent (methotrexate) led to significant improvement in signs and

Scleritis and Takayasu's disease - rare combined presentation.

INTRODUCTION: Takayasu's disease (TD) (pulseless disease, aortic arch syndrome) is a rare but potentially life-threatening chronic giant cell vasculitis. The major complications attributed to the disease include Takayasu's retinopathy, secondary hypertension, aortic regurgitation, and arterial aneurysm. The aim of this study is to describe a patient with Takayasu's disease and scleritis and to show its association. CASE REPORT: We describe a 44-year-old female patient with Takayasu's disease who presented with bilateral anterior scleritis in the form of painful red eye. Patient did not have a

Refractory necrotizing scleritis successfully treated with adalimumab.

Necrotizing scleritis is the most severe and destructive form of scleritis with vision-threatening sequelae. It is divided into with inflammation and without inflammation (scleromalacia perforans). Adalimumab is a tumour necrosis factor (TNF)-inhibiting anti-inflammatory medication licensed for the treatment of rheumatoid and psoriatic arthritis, ankylosing spondylitis and inflammatory bowel disease (in the USA). We report two cases of necrotizing scleritis successfully treated with adalimumab.

Posterior scleritis presenting with angle closure glaucoma.

PURPOSE: To report a case of posterior scleritis associated with angle closure glaucoma and transient myopia. METHODS: A previously healthy 29-year-old male was referred for YAG laser iridotomy to treat angle closure glaucoma. He was suffering from severe pain and vision loss in his right eye. Findings on examination were hyperemia, proptosis, and myopia measuring approximately 6 diopters. Diagnosis of posterior scleritis was made due to presence of choroidal thickening, sub-Tenon effusion and the classical "T" sign observed on orbital ultrasonography. RESULTS: Treatment was initiated using

Surgically induced necrotising scleritis after pterygium excision.

BACKGROUND: Surgically induced necrotising scleritis (SINS) presents as a focal area of intense scleral inflammation next to the site of previous scleral or limbal excision. There may be or no associated connective tissue disease. It may also follow Pterygium excision with use of antimetabolites. OBJECTIVE: To report an uncommon complication following pterygium excision, one of the commonest surgeries performed by Ophthalmologists. CASE REPORT: Our patient was a 62-year-old retired teacher who had a left eye nasal primary pterygium excised with Mitomycin C (MMC) dab topically. He presented 2

Resolution of anterior scleritis after periodontal therapy.

Scleritis is a chronic inflammation that involves the outermost coat and the skeleton of the eye. Scleritis may be associated with a systemic or immune mediated disease and it might be caused by an infection, trauma or drug reaction. This case presents a patient with generalized chronic periodontitis and anterior diffuse scleritis. A 30-year-old female complained of pain and persistent scleritis in the left eye was referred to the Periodontology department due to her periodontal problems. She was treated with oral non steroidal anti-inflammatory drugs and topical medications (corticosteroid) f

Giant nodular posterior scleritis simulating choroidal melanoma.

A 45-year-old woman, complaining of sudden visual loss in the right eye (best-corrected visual acuity: 17/200), was suspected of having a malignant melanoma of the choroid. Fundus examination revealed a massive pale-colored subretinal mass temporal to the fovea, with adjoining choroidal folds and exudative retinal detachment. Ultrasonography was suggestive of posterior scleritis. Systemic evaluation was unremarkable. The patient was treated with high-dose systemic corticosteroids. The mass lesion resolved completely, with visual recovery to 20/20. Posterior scleritis should be suspected when e

Posterior scleritis secondary to Pseudomonas aeruginosa keratitis.

PURPOSE: To report a case of Pseudomonas aeruginosa keratitis that progressed to posterior scleritis. METHODS: Observational case report. RESULTS: A 64-year-old woman was treated for P. aeruginosa keratitis. Twelve days later, she was diagnosed with posterior scleritis and treated with oral ciprofloxacin. Her condition progressed, ultimately resulting in enucleation. Histopathology confirmed posterior scleritis, a previously unreported complication of P. aeruginosa keratitis. CONCLUSION: P. aeruginosa keratitis may progress to posterior scleritis, a potentially devastating complication.

Nodular anterior scleritis associated with ocular trauma.

It is accepted that ocular trauma may be the precipitant of non-infectious uveitis. We report the case of a patient who developed unilateral nodular anterior scleritis following ocular trauma. Infection and systemic inflammatory diseases were excluded as causes of the inflammation. Our observations suggest the possibility that ocular trauma may act as a trigger for scleritis.

Mycobacterium chelonae conjunctivitis and scleritis following vitrectomy.

The atypical, or nontuberculous, mycobacteria are opportunistic pathogens that usually cause infection following accidental trauma or surgery. These organisms are ubiquitous in nature but have been found with increasing frequency in other environments that include medical offices and surgical suites. Management of atypical mycobacterial ocular infections can be difficult because in vitro antibiotic activity does not always correlate with in vivo efficacy and because normal immune defenses against mycobacteria may work too slowly to prevent irreversible damage to infected ocular tissues. This r
