CT evaluation of bone dehiscence of the superior semicircular canal as a cause of sound- and/or pressure-induced vertigo.

PURPOSE: To describe the computed tomographic (CT) findings at different collimation widths associated with superior semicircular canal (SSC) dehiscence syndrome and to determine the frequency of these findings in a control population. MATERIALS AND METHODS: Temporal bone CT scans with 1.0-mm and/or 0.5-mm collimation were obtained in 50 patients with sound- and/or pressure-induced vestibular symptoms. The control population consisted of 50 patients undergoing CT at 1.0-mm collimation and 57 patients undergoing CT at 0.5-mm collimation for other reasons. RESULTS: SSC dehiscence was documente

Acute effects of semicircular canal destruction on the cochlea, with and without preceding Pseudomonas aeruginosa exotoxin A treatment.

Acute electrophysiological and morphological changes in the cochlea following destruction of the semicircular canals (SCCs) were investigated to elucidate differences in cochlear vulnerability to surgical procedure under two conditions: normal healthy condition and a pathological condition induced by Pseudomonas aeruginosa exotoxin A (PaExoA). Frequency-specific auditory brainstem response recording and examination by light microscopy (LM) and scanning electron microscopy (SEM) were performed in the acute stage, immediately following, and 2, 5, and 10 days after the intervention. All normal he…

Efficient Endolymphatic Sac-Directed Gene Delivery Using AAV8BP2 and Posterior Semicircular Canal Injection.

Mutations in SLC26A4, which encodes the anion transporter pendrin, represent one of the most common genetic causes of hereditary hearing loss, including Pendred syndrome and DFNB4. Pendrin plays a critical role in maintaining ion homeostasis within the inner ear, particularly in the endolymphatic sac (ES), making it an important therapeutic target for gene replacement strategies. However, efficient delivery of therapeutic genes to relevant inner ear structures remains a major challenge for clinical translation. In this study, we evaluated the inner ear transduction profile of AAV8BP2, an engin

Vestibular Aqueduct Hypoplasia Identifies Semicircular Canal Dehiscence Patients Who Progress to Meniere's Disease.

OBJECTIVE: Semicircular canal dehiscence (SCD) and vestibular aqueduct (VA) hypoplasia are developmental anomalies associated with distinct inner ear syndromes-SCD syndrome and Meniere's disease (MD). Our previous work found frequent SCD in MD patients with VA hypoplasia, suggesting a shared developmental origin. To further explore this association, we adopted a reciprocal approach by assessing VA hypoplasia prevalence in patients diagnosed with SCD. STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary referral center. PATIENTS: A total of 219 ears from 173 patients (mean age 53.5 ye

A Rare Case of Anterior Semicircular Canal BPPV Resistant to Treatment: A Case Report and Literature Review.

Background and Clinical Significance: Benign paroxysmal positional vertigo (BPPV) most commonly involves the posterior semicircular canal (PSC), whereas anterior semicircular canal BPPV (ASC-BPPV) is rare, accounting for only 1-3% of cases. Most ASC-BPPV cases respond well to particle repositioning maneuvers (PRMs), with refractory presentations being exceptional and diagnostically challenging, particularly when differential diagnoses such as apogeotropic posterior semicircular canal BPPV (PSC-BPPV) or central causes must be excluded. Case Presentation: A 43-year-old woman presented with verti

Chronic otitis media with cholesteatoma: A retrospective observational study on the management of lateral semicircular canal fistulas.

Labyrinthine fistulas are a common complication of cholesteatomatous chronic otitis media (COM), with the lateral semicircular canal (LSCC) being the most frequently affected site. This retrospective cohort study aims to evaluate the clinical management of cases with LSSC fistula secondary to cholesteatomatous COM in a tertiary care hospital and analyze the obtained results along with the literature. Between January 2009 and June 2024, patients who developed LSSC fistula due to cholesteatomatous COM and underwent surgical treatment at our center were retrospectively analyzed. The study evaluat

A novel mouse model of acute vestibular dysfunction via semicircular canal injection of absolute ethanol.

OBJECTIVE: Various vestibular pathology models have been developed to elucidate the underlying mechanisms of vestibular diseases; however, there is a notable absence of minimally invasive mouse model of vestibular dysfunction. In this study, we established a novel model and conducted a comprehensive evaluation of vestibular function alongside various behavioral assessments. METHODS: We developed a surgical procedure involving canalostomy followed by injection of absolute ethanol into the semicircular canal of mice. Histological analysis and auditory brainstem response (ABR) testing were condu

Model of superior semicircular canal dehiscence: asymmetrical vestibular dysfunction induces reversible balance impairment.

BACKGROUND: Superior semicircular canal dehiscence (SSCD) is a vestibular-cochlear disorder in humans in which a pathological third mobile window of the otic capsule creates changes to the flow of sound pressure energy through the perilymph/endolymph. The primary symptoms include sound-induced dizziness/vertigo, inner ear conductive hearing loss, autophony, headaches, and visual problems. We have developed an animal model of this human condition in the Mongolian Gerbil that uses surgically created SSCD to induce the condition. A feature that is unique in this model is that spontaneous resurfac

Association of the superior semicircular canal and tegmen tympani dehiscences and its relationship with the pneumatisation of the temporal bone.

PURPOSE: To analyse the degree of pneumatisation of the temporal bone when there is an association between dehiscence of the superior semicircular canal and dehiscence of the tegmen tympani. MATERIALS AND METHODS: We analysed a retrospective CT study of 124 selected cases. A single inclusion criterion was applied: the presence of a dehiscence of the tegmen tympani. On the other hand, the degree of temporal pneumatisation was assessed by axial and coronal planes, and has been divided into the following grades O, I, II and III, according to the status and relationship of the mastoid, the bony l

New model of superior semicircular canal dehiscence with reversible diagnostic findings characteristic of patients with the disorder.

BACKGROUND: Third window syndrome is a vestibular-cochlear disorder in humans in which a third mobile window of the otic capsule creates changes to the flow of sound pressure energy through the perilymph/endolymph. The nature and location of this third mobile window can occur at many different sites (or multiple sites); however, the most common third mobile window is superior semicircular canal dehiscence (SSCD). There are two essential objective diagnostic characteristics needed to validate a model of SSCD: the creation of a pseudoconductive hearing loss and cVEMP increased amplitude and decr

Evaluation of Superior Semicircular Canal Dehiscence Anatomical Location and Clinical Outcomes: A Single Institution's Experience.

BACKGROUND: Superior semicircular canal dehiscence (SSCD) is becoming increasingly recognized as a pathology underlying various auditory and vestibular complaints. To date, our understanding of the pathology has yet to attribute specific symptoms to the anatomic location of dehiscence in patients with SSCD. This study aims to address this issue by evaluating the relationship between symptomatology and anatomic location of dehiscence. METHODS: A single-institution retrospective review of SSCD patients was performed. Information was collected on patient demographics, symptomatology, and anatomi

Bilateral superior semicircular canal dehiscence: bilateral conductive hearing loss with subtle vestibular symptoms.

Superior semicircular canal dehiscence is caused by a bone defect on the roof of the superior semicircular canal. The estimated prevalence when unilateral varies between 0.4% and 0.7% and is still unknown when bilateral. Patients may present with audiologic and vestibular symptoms that may vary from asymptomatic to disabling. We report a case of a 72-year-old Caucasian woman presented to otolaryngology department reporting imbalance, bilateral pulsatile tinnitus, hypoacusis while being very sensitive to certain sounds. Physical examination was unremarkable, except for the Rinne test that was n

Activation of GABAB receptors results in excitatory modulation of calyx terminals in rat semicircular canal cristae.

Previous studies have found GABA in vestibular end organs. However, existence of GABA receptors or possible GABAergic effects on vestibular nerve afferents has not been investigated. The current study was conducted to determine whether activation of GABAB receptors affects calyx afferent terminals in the central region of the cristae of semicircular canals. We used patch-clamp recording in postnatal day 13-18 (P13-P18) Sprague-Dawley rats of either sex. Application of GABAB receptor agonist baclofen inhibited voltage-sensitive potassium currents. This effect was blocked by selective GABAB rece…

Multiple Sclerosis Presenting as an Anterior Semicircular Canal Benign Paroxysmal Positional Vertigo: Case Report.

We present the case of a 31-year-old woman with isolated symptomatology accusing positional vertigo. The videonystagmography (VNG) including Dix-Hallpike testing we have performed, highlighted atypical eye movements. We have observed a positional downbeating nystagmus with characteristics that could be accounted for anterior semicircular canal benign paroxysmal positional vertigo. Furthermore, examining the atypical nystagmus at the measurements performed during VNG recordings, we suspicioned a central positional vertigo. The abnormalities observed at the positional nystagmus were the lack of
