Differential Risk for Homelessness Among US Male and Female Veterans With a Positive Screen for Military Sexual Trauma.

IMPORTANCE: Military sexual trauma (MST) is associated with adverse physical and mental health outcomes following military separation. Recent research suggests that MST may be a determinant in several factors associated with postdeployment homelessness. OBJECTIVE: To evaluate MST as an independent risk factor for homelessness and to determine whether risk varies by sex. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study of US veterans who used Veterans Health Administration (VHA) services between fiscal years 2004 and 2013 was conducted using administrative data from the Departm…

Sexual risk behaviors by relationship type and trauma history among HIV-positive men who have sex with men.

The association of trauma exposure and coping style to sexual risk behavior has yet to be fully examined in the context of primary and casual sexual partnerships. The current study assessed a high risk sexual behavior-unprotected anal intercourse (UAI)-in a high risk population of HIV-positive men who have sex with men (MSM) with a history of trauma. Using audio computer-assisted self-interview technology, 132 HIV-positive MSM completed measures of trauma exposure, trauma symptoms, coping strategies, and sexual risk behavior. Hierarchical logistic regression analyses indicated that completing

Perspectives of Rural and Urban Veterans with Military Sexual Trauma Seeking Psychotherapy Through Community Care: A Qualitative Study.

The Veterans Community Care Program (VCCP) has become essential to how the Veterans Health Administration (VHA) delivers care. Although VCCP expands Veterans' access to community providers, ongoing process inefficiencies create barriers to timely care. Veterans affected by military sexual trauma (MST) are at heightened risk for behavioral health challenges, especially when care is delayed or fragmented. These delays may be particularly harmful for Veterans with MST living in rural communities where structural barriers exacerbate health risks. In this study, the primary author interviewed 38 Ve…

Perpetrator types, severity, and cumulative trauma: a population-based study of childhood sexual abuse characteristics and lifelong health.

BACKGROUND: The influence of child sexual abuse (CSA) severity and perpetrator type on health problems in adulthood is poorly understood. We examined health outcomes associated with CSA according to severity, number of perpetrators, and perpetrator type. METHODS: We analysed data from a nationally representative sample of 8503 participants. We assessed the influence of four levels of severity and eight types of perpetrators on mental disorders, health risk behaviours, and physical conditions, using multivariable models adjusted for confounders including other maltreatment. FINDINGS: CSA was

Polycystic ovary syndrome: quantifying the psychosocial burden and the predictive role of sexual trauma history in clinical outcomes.

Polycystic ovary syndrome (PCOS) is a complex endocrine disorder that needs to be addressed through holistic management of metabolic, hormonal, and lifestyle factors. This research study, a cross-sectional examination, reports on clinical and psychological aspects of PCOS and PCOD in a group of 75 educated women analyzed with descriptive statistics, non-parametric tests, and binary logistic regressions. To carry out this analysis, we integrated the symptom severity index (SSI) and a binary measure of past sexual trauma history. The study observed irregular menstruation in 85.3% and weight gain…

A Network Analysis of Embodied Trauma: Self-Destructive Behaviors Among Survivors of Childhood and Adolescent/Adult Sexual Assault.

This study capitalized on a network analysis to examine the interrelations between childhood sexual abuse (CSA), adolescent/adult sexual assault (AASA), and self-destructive behaviors (i.e., binge eating, self-harm, and substance use) within a network framework, guided by the principles of embodiment theory. It further aimed to explore how these patterns vary across gender identities by estimating and comparing three distinct networks for men, women, and gender-diverse individuals. Data were drawn from the International Sex Survey (N = 82,243; M_age = 32.39, SD = 12.52) conducted across 42 cou

The Impact of Resilience on Employment Among Post-9/11 Veterans With and Without Military Sexual Trauma Exposure.

INTRODUCTION: Successful employment is a functional outcome of high importance for veterans after military discharge. There is a significant rising concern regarding exposure to military sexual trauma (MST) and related mental health outcomes that can impair functional outcomes, such as employment. Although resilience training is a key component of preparing for military service, to date the impact of resilience on employment outcomes for veterans with exposure to MST has yet to be examined. We sought to examine the relationship between resilience and employment in a national sample of post-9/1…

Disclosure of Sexual Trauma to Romantic Partners: Examining PTSD Clusters, Partner Reactions, Perceived Partner Responsiveness, and Relationship Satisfaction.

The present study explored the relationship between disclosure of sexual trauma to a romantic partner and relationship satisfaction for individuals who have experienced unwanted or forced sexual contact. We examined whether posttraumatic stress disorder (PTSD) clusters, partner reactions, and perceived partner responsiveness (PPR) mediated that relationship. Relationship satisfaction was significantly, positively correlated with trauma disclosure, positive partner reactions, and PPR, and significantly, negatively correlated with negative partner reactions and the PTSD clusters of avoidance and…

Trauma and sexual abuse in children-Epidemiology, challenges, management strategies and prevention in lower- and middle-income countries.

Trauma is rising as a cause of morbidity and mortality in lower- and middle-income countries (LMIC). This article describes the Epidemiology, Challenges, Management strategies and prevention of pediatric trauma in lower- and middle-income countries. The top five etiologies for non-intentional injuries leading to death are falls, road traffic injuries, burns, drowning and poisoning. The mortality rate in LMICs is twice that of High-Income Countries (HICs) irrespective of injury severity adjustment. The reasons for inadequate care include lack of facilities, transportation problems, lack of preh

Mental contamination, disgust, and other negative emotions among survivors of sexual trauma: Results from a daily monitoring study.

Mental contamination (MC)-feelings of dirtiness triggered by internal sources-is a potentially important yet understudied factor for survivors of sexual trauma. MC has been linked to disgust and other negative emotions (e.g., shame, guilt) cross-sectionally and in lab-based paradigms but not yet examined in ecological contexts. Additionally, links between MC and distinct negative emotions have not been studied systematically. The present study thus modeled relationships between MC and specific emotions both across and within days over a daily monitoring period. Forty-one females with sexual tr…

Alexithymia, Prior Trauma, Alcohol Use, and Sexual Aggression Perpetration: A Cross-Cultural Comparison of a Moderated Mediation Model.

In this study, we examined the relationships between prior trauma, alexithymia, and sexual aggression perpetration among 610 U.S. college students and 107 college students from the Philippines utilizing a cross-sectional retrospective design. We tested a moderated mediation model with alexithymia as the mediator between prior trauma and sexual aggression perpetration, and alcohol use as a moderator of the alexithymia-sexual aggression link. Moreover, given that cultural norms may influence these relationships, we also examined the structural invariance of the proposed moderated mediation model

Examining the Influence of Trauma Exposure on HIV Sexual Risk Between Men and Women in Post-Conflict Liberia.

The adverse impact of exposure to war-related traumatic events on mental health is well documented. Few studies, however, have focused on the impact of exposure to war-related traumatic events on HIV sexual risk behavior in post-conflict countries such as Liberia. We investigated whether exposure to war-related traumatic events was linked to HIV sexual risk behavior, and identified potential mediators of this relationship, including stressful life events, problematic alcohol use, and intimate partner violence (IPV) among women and men in Liberia. Data were collected from a sample of 395 partic

How do men with severe sexual and physical childhood traumatization experience trauma-stabilizing group treatment? A qualitative study.

Background: Exposure to potentially traumatizing events, defined as events involving actual or threatened death or serious injury, is associated with an elevated risk of developing enduring physical, psychological and social problems. Complex post-traumatic stress disorder (PTSD) is a disorder that can occur after prolonged and repeated trauma. At least 30% of the sexually abused population is male, but in spite of this fact, treatment research focusing on male victims is virtually non-existent in comparison to research on female victims. Objective: Trauma-stabilizing group treatment is an inc

The relationship between sexual trauma, peritraumatic dissociation, posttraumatic stress disorder, and HIV-related health in HIV-positive men.

This study tested a novel extension of P. P. Schnurr and B. L. Green's (2004) model of the relationships between trauma symptoms and health outcomes with specific application to HIV-positive men. A diverse sample of 167 HIV-positive men recruited from San Francisco Bay Area HIV clinics completed demographic, medical, trauma history, and symptom questionnaires. Mediation analyses were conducted using the method proposed by R. Baron and D. Kenny (1986). Regression analyses found that sexual revictimization (SR) significantly mediated the relationship between child sexual abuse and peritraumatic

Prior Sexual Trauma and Adjustment Following the Virginia Tech Campus Shootings: Examination of the Mediating Role of Schemas.

A sizable body of research supports trauma's cumulative nature. However, few studies have evaluated potential mechanisms through which the experience of multiple traumas leads to elevated distress. The current study sought to evaluate differences between sexual trauma victims and women who had not experienced sexual trauma in their adjustment following a mass trauma (college women exposed to the 2007 Virginia Tech campus shooting). In addition, the study examined whether maladaptive schema change (lower self-worth and less belief in benevolence) and social support mediated the relationship bet…

The dialectics of trauma and political conscientization: A psychosocial study of activism for supporting sexual and gender minoritized communities in Brazil.

This qualitative study examined the dialectical association between psychosocial trauma and political conscientization in the lives of activists advocating for persons with marginalized sexual orientations and gender identities (2SLGBTQIA+) in São José dos Campos, Brazil. Drawing on participatory research and grounded in liberation psychology and intersectionality, the study involved collaboration with 10 activists through participation in advocacy activities, semistructured interviews, and collective reflection. Our reflexive thematic analysis indicated four main themes: (a) addressing psycho

Innocence betrayed: The impact of grooming and family betrayal on trauma symptoms in adult survivors of child sexual abuse.

OBJECTIVE: This study examined the relationships between different forms of sexual grooming, emotional betrayal, and trauma symptoms among adult survivors of child sexual abuse (CSA). METHOD: An anonymous online retrospective survey was completed by 342 adult CSA survivors. Participants reported on three types of grooming methods used by perpetrators (Verbal Coercion, Threats/Violence, and Drugs/Alcohol) and two types of emotional betrayal (Perpetrator Betrayal and Family Betrayal). Path analysis was conducted to examine the direct and indirect effects of grooming types on trauma symptoms thr

Trauma and Maternal Caregivers as Risks for Executive Function Deficits Among Youth Who Have Sexually Harmed.

Despite robust research linking trauma events to the commission of sexual harm by youth, the majority of victims do not become victimizers, imploring us to study potential interceding influences. Some research indicates that intermediary factors like attachment characteristics, trauma symptomatology, and executive functioning may be critical in understanding sexual harm committed by youth. This study explored relationships between trauma events, trauma symptoms, and attachment characteristics, and their relationship to executive functioning in a sample of 196 youth who committed sexual harm. The "Wall of Fear": the bridge between the traumatic event and trauma resolution therapy for childhood sexual abuse survivors.

A multitude of published books and papers on child sexual abuse (CSA) describe symptoms, long-term effects, and therapy for survivors of abuse. However, the parallels between the nature of the sexual trauma event(s) as originally experienced by the victim and the therapeutic process into which the survivor later becomes engaged have not been reported. This paper attempts to fill that gap and proposes that the concept of a Wall of Fear is the bridge connecting the two. In the first part of the paper, a model of the CSA experience based upon Furniss (1991) will be explained in order to point out…
