Irritable bowel syndrome and dysthymia. Is there a relationship?

Irritable bowel syndrome (IBS) has been reported in 10% to 22% of adults. The authors compared patients seeking treatment for dysthymia (N = 59) in an outpatient setting to an age- and sex-matched comparison group of patients (N = 54) seeking treatment in a general physician's office for other medical illnesses. The comparison group did not have any Axis I disorders. IBS was diagnosed by using the criteria established by Drossman and colleagues. Of the patients screened, 59.32% of the patients with dysthymia met criteria for IBS in contrast to 1.85% of the comparison group (P < 0.000005). IBS

Sexuality and sickle cell disease.

Chronic illness can impact negatively on sexuality and sexual satisfaction. A group of 44 patients with sickle cell anemia and a control group of 42 individuals with no chronic illness completed the Derogatis Sexual Functioning Inventory (DSFI). The sickle cell anemia group also completed the Psychosocial Adjustment to Illness Scale--Self-Report (PAIS-SR). Cumulative scores on the PAIS-SR were used to divide the sickle cell anemia group into two subgroups--those who were poorly adjusted to their illness and those who were well adjusted to their illness. Analysis of the scores showed significan

Psychological factors affecting reported pain in juvenile rheumatoid arthritis.

Examined the extent to which psychological variables are correlated with pain reported by children with juvenile rheumatoid arthritis (JRA). In a hierarchical multiple regression analysis with pain as the dependent variable, four psychological measures of child and family functioning resulted in a significant increase in R2 = .31, p < .0001, after the effects of disease characteristics were considered. Greater emotional distress in the child, greater emotional distress of the mother, and greater family harmony were related to higher reported pain. Findings suggest that more attention should be

[Undetected physical diseases in new psychiatric admission].

We report on clinical observations from a 10-bed psychiatric acute ward where predominantly patients with concurrent physical diseases are treated. Of 287 patients admitted within 18 months we detected serious physical illness previously unrecognized or at least not sufficiently diagnosed in 15 cases (5.2%; age 17-88 years; 4 m, 11 f). Six patients suffered from acute neurological disease. One or more extracerebral disorders were found in nine cases. 14 of the 15 patients presented with organic mental syndromes classified according to ICD-9 (acute organic psychosis: n = 8, chronic organic psyc

The prevalence of anxiety disorders among patients with mitral valve prolapse syndrome and chest pain.

Nineteen patients from a cardiology practice with complaints of chest pain and with mitral valve prolapse syndrome were compared with 26 patients with chest pain but no discernible cardiac disorder. Instruments included a truncated form of the Diagnostic Interview Schedule, the symptom checklist 90 revised (SCL-90-R), the McGill Pain Questionnaire, and life events, physical activity, and family history questionnaires. Neither panic disorder nor self-rated anxiety were more common in the mitral valve prolapse group. This study failed to confirm the reported high association between mitral valve

Disease experience and psychosocial adjustment in children with juvenile rheumatoid arthritis: children's versus mothers' reports.

Adjustment in children with juvenile rheumatoid arthritis (JRA) has been evaluated most frequently by parental or teacher reports and with reference to disease severity. In this study, 38 children, ages 7 to 13 with JRA, and their mothers were interviewed. Modest correlations were found between children and mothers in their assessments of children's perceived competence in several domains (i.e., athletic competence, social acceptance, physical attractiveness, and global self-worth) and in their perceptions of how JRA is experienced by children and families. Children's perceptions of the diseas

Coping with breast cancer--a longitudinal prospective study.

In a prospective longitudinal study over several years, 58 patients with breast cancer are compared to 52 patients with fibrocystic disease and 24 patients with mastodynia. Results of coping (as assessed with the Bernese Coping Modes) are presented for the illness course of the first 6 months: (1) There is considerable variation of coping depending on illness situation and illness state. A core group of coping modes is predominant in most situations: 'attention & care', 'problem analysis', and 'Tackling'. In average 10 different coping modes were used by patients per given illness situation. (

Why do some psychiatric patients somatize?

A study was made of a series of 139 outpatients referred by the medical and surgical services of a general hospital for evaluation by the psychiatry unit. In accordance with established criteria, this population was divided into somatizers (56) and non-somatizers (75), and the socio-demographic and clinical characteristics of both groups were comparatively analyzed. The results show that the group of somatizers was younger, had more histrionic personality traits and more stress factors related with alterations in interpersonal relationships or death or disease of relatives. It is emphasized th

Central infusion of prostaglandin E2 reveals a unified representation of sickness in the mouse insular cortex.

During infections, vertebrates develop stereotypic symptoms such as elevated body temperature, reduced appetite, and lethargy. These changes, collectively known as sickness syndrome, are orchestrated by the brain in response to immune mediators released during systemic inflammation. While the roles of subcortical regions, including the hypothalamus and brainstem, in regulating sickness symptoms are well established, the contribution of the neocortex to the encoding and modulation of sickness remains less well understood. We examined the neuronal correlates of sickness in the neocortex of awake

Serum ACE activity and plasma ACE concentration in patients with SARS-CoV-2 infection.

Significant controversy has arisen over the role of the renin-angiotensin-aldosterone system (RAAS) in COVID-19 pathophysiology. In this prospective, observational study, we evaluated plasma angiotensin converting enzyme (ACE) concentration and serum ACE activity in 52 adults with laboratory-confirmed SARS-CoV-2 infection and 27 non-COVID-19 sick controls. No significant differences were observed in ACE activity in COVID-19 patients versus non-COVID-19 sick controls (41.1 [interquartile range (IQR): 23.0-55.2] vs. 42.9 [IQR 13.6-74.2] U/L, p = .649, respectively). Similarly, no differences wer

Building components contributors of the sick building syndrome.

The sick building syndrome has been widely discussed from epidemiological perspectives. Although there is considerable difference in opinion regarding the concrete and objective evidence to support a distinct sick building syndrome and/or building-related illness, much data indicates that numerous variables within buildings can potentially influence human health. In this paper, we discuss in detail not only the potential and unique infectious diseases caused by Legionella, Pontiac fever, Q fever, and influenza, but also the data implicating noninfectious etiologies of sick building syndrome an

[Nihilodermia in psychodermatology].

"Nihilodermia" refers to a group of difficult "problem" patients in dermatology without objective findings but with recurrent symptoms and stubborn demand for medical examination. These primary emotional disorders are somatoform disorders, but the patients usually strictly deny a psychosocial aspect and expect purely somatic treatment. Clinical patterns include pruritus, pain, paresthesias, feelings of disfiguration, eco-syndromes, erythrophobia and psychogenic pseudoeffluvium. The relevant somatoform disorders in dermatology can be differentiated as somatization disorders, hypochondriacal dis

Current issues in fitness for work certification.

This paper explores some current issues for certifying medical practitioners in the United Kingdom (UK), particularly general practitioners (GPs), who provide medical advice to their patients on fitness for work. Medical statements that doctors use to record this advice, such as form Med 3 and form Med 4, may be used by patients as evidence to support claims for financial benefits, including Statutory Sick Pay (SSP) and state incapacity benefits. The UK employment context for sick or disabled people of working age is beginning to change, but many barriers to work retention and work resumption

[Illness perception in general practice--in relation to patients with somatization disorder].

In this paper different perceptions of disease and illness in general practice and possible implications of these perceptions for the GP's clinical work are described and discussed. The focus is on patients with medically unexplained symptoms and on the question whether the illness perception of the GP to some extent can explain the often inappropriate treatment of these patients.
