Nickel: the search for alternative, optimal and non-irritant patch test preparations: Assessment based on laser Doppler flowmetry.

BACKGROUND/AIMS: Several contact allergens including nickel are also irritants. Ideal patch test preparations should not miss cases of contact allergy and should be non-irritant. The currently used nickel preparation (5% nickel sulfate in petrolatum; 0.19 m) is a compromise. The present aim was to compare the skin-irritant potential of various nickel preparations (sulfate, chloride, nitrate in petrolatum and in water) by using the objective, noninvasive laser Doppler technique for assessment. METHODS: Preparations with various nickel concentrations and vehicle controls were applied under occl

Responsive corneosurfametry following in vivo skin preconditioning.

Skin is subjected to many environmental threats, some of which altering the structure and function of the stratum corneum. Among them, surfactants are recognized factors that may influence irritant contact dermatitis. The present study was conducted to compare the variations in skin capacitance and corneosurfametry (CSM) reactivity before and after skin exposure to repeated subclinical injuries by 2 hand dishwashing liquids. A forearm immersion test was performed on 30 healthy volunteers. 2 daily soak sessions were performed for 5 days. At inclusion and the day following the last soak session,

Contact dermatitis for primary care providers.

Contact dermatitis is an eczematous dermatitis developing at a site where the skin has been in direct contact with the cutaneous irritant or allergen from the environment. Irritant contact dermatitis (ICD) is the most common form. Allergic contact dermatitis (ACD) is inflammation of the skin caused by an antigen that elicits a type IV hypersensitivity reaction. Clinical evaluation and judgment are a critical part of the process. Patch testing in an objective method to aid in differentiating ACD from ICD and of verifying responsible allergens.

Final report on the safety assessment of Stearamide DIBA-Stearate.

Stearamide DIBA-Stearate is a substituted dihydroxyisobutylamine (DIBA) that functions in cosmetic formulations as an opacifying agent, a surfactant-foam booster, and a viscosity increasing agent. Stearamide DIBA-Stearate was reportedly used in four cosmetic formulations, at concentrations of 1% to 3%. Few data on this ingredient were available. Data on related ingredients, including Dibutyl Adipate, Diisopropyl Adipate, Stearamide DEA, and Stearamide MEA, were considered in the assessment of safety. A formulation containing 1.3% Stearamide DIBA-Stearate (further diluted to 4% of the formulati

Irritant effects of a detergent in wash and chamber tests.

Irritant properties of detergents can be tested by using patch and chamber tests and various kinds of use tests. The aim of the present study was to compare the results of use and 12 mm Finn Chamber tests. Study subjects (10 atopic and 11 non-atopic medical students) washed the outer aspect of 1 upper arm with liquid detergent for 1 minute 2 x daily for 1 week. 48-h chamber test with 5 concentrations of the same detergent in water were concurrently applied to upper back skin. Transepidermal water loss (TEWL), electrical capacitance and skin blood flow were measured to quantify reactions on day

Dermal and ocular irritation and skin sensitization studies of fullerene C60 nanoparticles.

CONTEXT: Widespread production and use of nanomaterials have caused the release of increasing amounts of nanomaterials into the environment. The introduction of novel materials into industry requires safety evaluations as well as an understanding of the impact of the nanomaterials on human health, because the unique properties and size of nanomaterials may also result in unique health risks. Skin and eyes have the highest risk of exposure to nanomaterials, because deposition to the superficial organs has the potential to be a major route of exposure during the manufacturing, use, and disposal

Allergy and irritation: an adverse association in patients with atopic eczema.

The pathomechanism of atopic eczema is complex. Two of the most important exogenous factors for atopic eczema are allergenic and irritant substances. In this study we investigate the combined effect of topical aeroallergens and irritation on the skin of atopic individuals. We performed patch testing with several aeroallergens (atopy patch test) and with an irritant, sodium lauryl sulphate, on clinically unaffected skin of 30 sensitized patients with atopic eczema. Application was conducted alone and as a consecutive application. Healthy volunteers served as controls. Evaluation was made by mea

Differential irritant skin responses to topical retinoic acid and sodium lauryl sulphate: alone and in crossover design.

Topically applied all-trans retinoic acid (RA) is often associated with skin irritation. A detailed quantification of RA-induced functional changes in stratum corneum is, however, still limited. Using non-invasive bioengineering techniques of measurements of transepidermal water loss (TEWL), stratum corneum hydration and cutaneous blood flow (CBF), we quantified the irritant effects of 0.05% and 0.1% RA in ethanol on normal skin compared with 1% sodium lauryl sulphate (SLS) in water as a model irritant in a 24-h occlusive patch-test assay. Additionally, in order to document data possibly relat

Skin irritancy from nonanoic acid.

100 hospitalized patients with skin disease were patch tested with graded dilutions of nonanoic acid (NON), sodium lauryl sulfate (SLS) and benzalkonium chloride (BENZ). Smooth dose-response curves were obtained. The concentration needed to produce a discernible irritant reaction in 50% of the population (ID50) was found to be lower in cases of irritant than of allergic contact dermatitis; this finding warrants further study. Björnberg's observation that it is not possible to predict the strength of the reaction to one irritant by knowing the strength of the reaction to another was confirmed.

Assessment of skin irritancy in man by laser Doppler flowmetry.

It is desirable to use more objective methods than visual scoring for the assessment of skin irritancy reactions. In the present study the blood flow in skin sites exposed to sodium lauryl sulfate (SLS) was recorded by a laser Doppler flowmeter. The irritant was applied to the volar forearm of healthy volunteers in concentrations ranging from 0.001% to 5% under occlusion for 24 h. The test sites were scored visually and the blood flow was recorded at 3 different times: 26 h, 48 h and 72 h after the application of SLS. Approximately 950 recordings were performed and a clear relationship was obs

The vulvar epithelium differs from the skin: implications for cutaneous testing to address topical vulvar exposures.

Vulvar tissue is more permeable than exposed skin due to differences in structure, occlusion, hydration and susceptibility to friction. The safety assessment of products that contact the vulva should account for this potentially heightened permeability. Standard clinical patch tests may not sufficiently mimic vulvar exposures. Because testing on the vulva is not routinely feasible, we are investigating new and modified cutaneous test methods to increase the degree of conservatism of the safety assessment. To this end, we have 1) developed a method to assess chemical and frictional effects by m

Lymphocyte subsets and Langerhans cells in allergic and irritant patch test reactions: histometric studies.

This study has attempted to distinguish between allergic and irritant reactions to patch tests by semiquantitative histological methods. The extent of perivascular chronic inflammatory infiltrate at 72 h in irritant patch test reactions to sodium lauryl sulphate was shown to be small and very consistent, whereas in allergic reactions to nickel sulphate it was generally larger and more variable in size (p less than 0.02). The two major lymphocyte subsets (T4 and T8) were randomly intermixed in both types of reaction and formed the major component of both the perivascular and diffuse dermal infi

Occupational skin diseases in the San Francisco Bay Area.

From answers by one-third of the practicing dermatologists in the San Francisco Bay Area to a questionnaire on occupational skin diseases, contact dermatitis due to irritants and sensitizers was found to rank first. Poison oak, which is the leading reported cause on "Doctor's First Report of Work Injury" received by the California Department of Industrial Relations, was sixth on the list of the survey, trailing solvents, cleansing agents, petroleum products and epoxy resins. A history of atopic dermatitis was often noted in current cases of occupational diseases of the skin. Avoidance of expos

Clinical studies of sweat rate reduction by an over-the-counter soft-solid antiperspirant and comparison with a prescription antiperspirant product in male panelists.

BACKGROUND: Individuals with axillary hyperhidrosis have much higher than average sweat rates and are often prescribed anhydrous aluminum chloride (AlCl(3)) solutions. Topical application of these solutions can be irritating to the skin, resulting in poor compliance and lower than desired efficacy. OBJECTIVE: Demonstrate the efficacy of an over the counter "clinical strength" soft-solid antiperspirant using a night time application regimen and compare to a prescription aluminum chloride (6.5%) antiperspirant using male panelists. METHODS: Gravimetric hot room efficacy testing (100 F and 35%

[Lymphocyte stimulation test, a possible alternative for verifying chloroacetophenone sensitization].

We report on a case of pronounced sensitization to chloroacetophenone tear gas that developed after repeated occupational skin exposure in a 57-year-old police officer. Mainly in the presence of moisture and occlusion, cutaneous application of chloroacetophenone leads to severe irritant, and often also allergic, skin reactions. In patch testing the demonstration of allergic contact dermatitis in response to chloroacetophenone is hampered by the irritative potential of this substance even at low concentrations. This diagnostic bias can be overcome by the lymphocyte proliferation assay.

Stoma dermatitis: prevalent but often overlooked.

Peristomal dermatoses commonly afflict the area around stoma openings in ostomy patients. These complications, however, are often unreported by patients and remain untreated for years, thus affecting maintenance and recovery from the surgery. These dermatoses can have chemical, mechanical, irritant, bacterial, immunologic, or disease-related etiologies. Examples of common forms of dermatitis that occur peristomally include fecal or urine irritant contact dermatitis, chronic papillomatous dermatitis, mechanical dermatitis, and allergic contact dermatitis. This article summarizes various skin ir
