One of the major challenges of cranial base surgery is reconstruction of basal dural defects. Ineffective reconstruction may result in potentially life-threatening complications such as cerebrospinal fluid leak, meningitis, and tension pneumocephalus. Goals of reconstructive procedures are to repair the dural defect and to separate intracranial contents from bacteria-laden secretion of the mouth, nose, and sinus. Different reconstructive techniques have been reported. Multiple factors may influence the surgical choice of reconstructive technique and its outcome. Regional pedicled flap and vasc

BACKGROUND: The authors' goal was to develop an updated and comprehensive algorithm for skull base reconstruction based on data from the 10-year period following their initial report.
METHODS: Reconstructive outcomes were analyzed from 250 patients undergoing skull base reconstruction from 2000 to 2009.
RESULTS: Thirty-nine local or regional pedicled flap reconstructions and 211 free flap reconstructions were performed. Free flaps were usually selected over pedicled flaps for patients with a history of prior surgery, irradiation, or chemotherapy (p = 0.003, p < 0.001, and p = 0.04, respectiv…

BACKGROUND: Mucormycosis is a relatively rare form of fungal infection that can be rapidly progressive. Clinical presentation depends on the anatomic area of involvement; however, the commonest form is that of rhinocerebral.
CASE DESCRIPTION: This is the first case of extensive and progressive destruction of the skull vault despite antifungal therapy. The patient required aggressive surgical resection to arrest the disease progression and complex reconstruction using a custom made titanium cranioplasty.
CONCLUSIONS: This report highlights the need for early aggressive resection for cases of …

Anomalous sutures of the parietal bone are exceedingly rare events. We present the case of a child admitted to the hospital after a near-drowning incident who subsequently expired. He was diagnosed with a parietal skull fracture by computed tomography scan before death. This was found to be an anomalous suture by postmortem histologic examination. Division of the parietal bone is an unusual event with anomalous sutures, which can be mistaken for fractures on radiographic or postmortem examination. Although fractures are much more common, it is prudent to evaluate all fractures and/or fracture-…

BACKGROUND: Fibrous dysplasia of the skull is difficult to manage when the cranial nerves, skull base, and face are involved. The procedure for the surgical treatment of this lesion needs to be studied clinically because of the low incidence rate and its operative complicacy. Faced with this, combined with our clinical experience, we explore the surgical strategy and operative procedures of the skull fibrous dysplasia.
METHODS: Twelve patients with skull fibrous dysplasia have been studied with neuroimaging and underwent operative removal of the lesion and cranioplasty and/or reestablishment …

The aim of this study was to perform a morphometric analysis of the skull base to understand the mechanism of asymmetry between malformational and deformational plagiocephaly. Since 1988, we have carried out 102 CT scans with three-dimensional reconstruction of the endocranial side of the skull base. There were 20 cases of malformational plagiocephaly, consisting of 18 unicoronal synostoses (UCS) and two unilambdoid synostoses (ULS), and 82 cases of plagiocephaly without synostosis, made up of 60 cases of frontal plagiocephaly (FP) and 22 of occipital plagiocephaly (OP). The skull base asymmet…

The frontal sinus is fractured in 30% of patients sustaining maxillofacial injury. The CT scan has simplified the treatment plan allowing the surgeon to be more selective in the treatment choice, surgical approach, and follow-up. Fractures may involve the anterior, posterior, or both tables of the sinus. The frontal sinus repair or surgery takes place when there is minimal displacement and the frontal ducts are in tact. Obliteration of the sinus is suggested when the ducts are injured, but there is no significant bone loss or comminution of the posterior table. More extensive injuries of the p

BACKGROUND: The incidence of multiple skull base meningiomas varies from 1 to 3% in different series. Skull base meningiomas are rare. The pathogenetic role of low-dose radiation seems to be fairly well established in the oncogenesis of meningiomas. Calvarial location and multiplicity seem to be among the distinctive features of radiation-induced meningiomas. Skull base location is a very rare occurrence, mainly because the path of irradiation does not significantly involve this region.
CASE REPORT: We describe a rare case of simultaneous occurrence of two skull base meningiomas in a 66-year-…

Surgery for posterior skull base tumors may be associated with high morbidity and mortality because of the complex anatomy, irregular bony topography, and vital neurovascular structures in this region. We experienced three benign posterior skull base tumors. These were petroclival and foramen magnum meningiomas and a jugular formen neurinoma. Three dimensional computed tomography (3 D-CT) in addition to the conventional CT, magnetic resonance imaging (MRI), and cerebral angiography were performed preoperatively. Preoperative embolizations for the tumors were also done, and intraoperative neuro…

Because of their critical location, invasive nature, and aggressive recurrence, skull base chordomas are challenging and, at times, frustrating tumors to treat. Both radical surgical removal and high-dose radiation therapy, particularly proton beam therapy, reportedly are effective in tumor control and improve survival rates. The authors posit that these tumors are best treated with radical surgery and proton-photon beam therapy. During the last 5 years, they treated 25 patients (15 females and 10 males) who harbored pathologically diagnosed skull base chordomas. The mean age of the patients w…

Cloverleaf skull or kleeblattschadel usually occurs as a result of premature closure of the cranial sutures, with accompanying hydrocephalus, proptosis, cranial base and midface hypoplasia. The incidence of this abnormality is rare, with less than 130 reported cases in the literature, and is associated with various congenital syndromes. Conventional surgical management advocates shunting prior to definitive craniotomy and bone remodeling. We report an unusual case of Pfeiffer syndrome with associated cloverleaf skull deformity, in which early surgery was vital for decompression of raised intra…

Mobilisation of the zygoma has been reported to offer excellent exposure of the cranial base with minimal brain retraction. This study describes our operative technique and presents our experiences of 22 patients. Fifteen patients had a skull base neoplasm and 7 had a complex cerebral aneurysm. To facilitate access to these lesions, two separate bone flaps were created: (1) A free frontotemporal bone flap; and (2) en bloc removal of the superior and lateral orbital rims with the attached zygomatic arch. Because the latter bone flap includes both the orbital roof and the posterolateral wall of …

The authors studied computed tomographic (CT) scans obtained in 40 patients with transsphenoid basilar skull fractures to establish if there were reproducible patterns of fracture along lines of weakness. Medical records were reviewed. Four major fracture patterns were identified: anterior transverse (n = 22), lateral frontal diagonal (n = 7), posterior transverse (n = 16), and mastoid diagonal (n = 3). Eight patients had two fracture patterns. Eleven of 40 patients (28%) died as a result of their head injury. The diagonal patterns were statistically significantly more frequently associated wi…

We describe unique monozygotic twins with Crouzon's disease and cloverleaf-shaped skull deformities who have been closely followed since birth. Their abnormal skull shapes were identified during antenatal ultrasound examination. The twins had gross exophthalmos and hydrocephalus with papilledema, so early calvarial decompression surgery was required. Although born to healthy parents of normal appearance, a third cousin of the twins had a milder form of Crouzon's disease, and there was a family history of high-arched palate on the twins' paternal side. Because Crouzon's disease is an uncommon c…

Three patients who presented cloverleaf skull are described. The first one associated with tanatophoric dwarfism, the second case was an isolate anomaly and the third one was associated with facial fissures. The three patients showed the outstanding diversity of the clinic manifestations encountered in this entity. The findings at central nervous system level, by computarized tomographic and ultrasonographic studies, are described. The relevance of genetic counselling is emphasized.

A case of osteoblastic skull metastasis of lung cancer is reported. A 56-year-old female patient was admitted to our hospital with complaints of headache and tumor of the right parietal bone. A plain skull X-ray showed hyperostotic feature of the right parietal bone. CT scan displayed that right parietal bone became thick and osteoblastic. Soft tissue was shown in the hyperostotic bone under MRI. An external carotid angiogram showed that the skull tumor was fed by the middle meningeal artery. The skull tumor and 2 solid intracerebral tumors were extirpated. Histological examination revealed ad…

Micromelic bone dysplasia with cloverleaf skull is a well-described entity with short, straight tubular bones, hydrocephalus and cloverleaf deformity of the skull, narrow chest and platyspondyly. A further two unrelated patients are presented, in whom the shape of the head varied. Both cases also showed a narrow foramen magnum, hypoplastic cerebellum, atrial septal defect, hypoplastic adrenals, and ureteric malformations. The findings in the skeleton show similarities to other types of generalised bone dysplasias but are distinguishable from these. This well-defined bone dysplasia should proba…
