Chronic post-traumatic erosion of the skull base.

Delayed post-traumatic erosion of the skull base is reported in three patients who presented as adults with cerebrospinal fluid fistulae and a history of recurrent meningitis. These skull defects were associated with herniation of the subarachnoid space into the diploe of the skull base, the paranasal sinuses and the orbit. This rare complication of head injury is assumed to have occurred as the result of a dural tear at the time of trauma. Its site probably determines whether a resulting meningocele widens the intradiploic space or broaches the cranial floor.

The infratemporal fossa approach to skull base surgery.

The infratemporal fossa approach, in conjunction with the application of microsurgical technique and improved perioperative care, has permitted significant advances in lateral skull base surgery. The glomus jugulare tumor is the prototypical neoplasm resected by this approach, although this technique can be applied to a host of additional benign and malignant lesions of the skull base. This approach entails identification and control of the cranial nerves and great vessels in the neck, anterior transposition of the facial nerve, and infralabyrinthine petrosectomy. Intracranial tumor extension …

Lockjaw secondary to skull base osteochondroma: CT findings.

Osteochondromas arising from the long bones, pelvis, or scapulae are common. However, osteochondromas originating from the base of the skull are extremely unusual. Although these tumors are histologically benign, intracranial extension and their close proximity to the cranial nerves may require complex surgery. We present a case in which CT clearly delineated an osteochondroma arising from the base of the skull. The tumor had created a fusion with the ipsilateral coronoid process of the mandible, thus causing lockjaw.

[Angiography of face and skull base (author's transl)].

The anatomic and technical conditions of the angiography of the face and the skull base are described. The significance of the catheter technique for selective demonstration of branches of the external carotid artery is stressed. The communications between the internal and the external carotid artery are discussed as well as the angiographic problems of arteriovenous fistulae and of hypervascular tumors. The selective angiography of the internal and the external carotid artery is an important tool of neuroradiology. By exact anatomic correlation of individual vessels and pathologic processes o…

The Role of 3D Tractography in Skull Base Surgery: Technological Advances, Feasibility, and Early Clinical Assessment with Anterior Skull Base Meningiomas.

Introduction  May-Thurner syndrome (MTS) is a vascular anatomic variant resulting in compression of the left common iliac vein by the right common iliac artery, affecting approximately 22% of the population. In adults, following acute deep vein thrombosis (DVT) of the iliofemoral veins, the incidence of postthrombotic syndrome (PTS) and recurrent DVT are high if treated with anticoagulation alone, warranting adjunctive treatment with thrombolysis and stent placement. However, there is paucity of literature documenting the course of treatment and associated outcomes in pediatric patients with M

The relationship between scaphocephaly at the skull vault and skull base in sagittal synostosis.

HYPOTHESIS: The skull vault is scaphocephalic in sagittal synostosis but little is known about the deformity at the skull base. If differential progressive deformity occurs between the vault and base, this might affect decision making regarding the timing of surgical intervention. We used 3-dimensional CT (3DCT) scans to compare deformity at the vault, base and posterior fossa in sagittal synostosis. Cephalic index (CI) was measured in 34 consecutive cases of isolated sagittal synostosis and 16 controls using predefined landmarks on the 3DCT volume data set. Planes were generated by a Vitrea™ …

Educational Utility of a Full-Skull 3-Dimensional Model for Skull Base Surgery Training.

BACKGROUND AND OBJECTIVES: Skull base surgery requires precise 3-dimensional (3D) anatomic understanding and advanced drilling techniques within a confined operative space. Although cadaveric dissection remains the gold standard for training, its availability is limited. This study aimed to evaluate the educational utility of a commercially available full-skull 3D model for skull base surgery training. METHODS: A full-skull 3D model reproducing the cranial base and brain was used in a hands-on training session involving 8 neurosurgeons, including 2 residents and 6 board-certified surgeons. Mu…

Simple rigid skull base reconstruction with resorbable unsintered hydroxyapatite/poly-l-lactide plate in endoscopic endonasal skull base tumor surgery: illustrative case.

BACKGROUND: Skull base reconstruction following an extended endoscopic endonasal approach (EEA) is essential for preventing postoperative CSF leakage. Despite recent advancements in surgical techniques and materials, CSF leakage remains a significant complication. This report presents a rigid reconstruction technique utilizing a resorbable unsintered hydroxyapatite/poly-l-lactide (u-HA/PLLA) plate to reduce postoperative CSF leakage. OBSERVATIONS: Between May 2024 and June 2025, this method was used in 10 cases of extended EEA involving high-flow intraoperative CSF leaks and bone defects more…

Single-layer Endoscopic Repair of Anterior Skull Base Defects.

This article reviews contemporary strategies for single-layer endoscopic repair of anterior skull base cerebrospinal fluid leaks, which are important for small defects and when multiple layers using vascularized tissue options are not available. Successful reconstruction requires restoring a watertight barrier between the intracranial compartment and the sinonasal tract, with technique selection guided by defect size, location, flow dynamics, and tissue availability. When appropriately selected, patients undergoing single-layer endoscopic repair achieve closure rates exceeding 90%. The article…

Anatomy and Imaging of the Orbit and Skull Base.

Imaging anatomy of the orbit and skull base can seem daunting at first glance. Navigating the many skull base foramina, compartments, and variants can be challenging without an in-depth knowledge of the structures themselves. This article will give a detailed review of the normal anatomy of the skull base and orbit, as well as important variants when considering procedural interventions in the orbit or skull base.

Recurrent Cholesteatoma with Skull Base Erosion: A Case Report.

Cholesteatomas are non-malignant, expandable diseases that, if left untreated, can destroy the middle ear, temporal bone, and other structures. Their recurrence can cause disastrous effects, for example, skull base destruction, acute mastoiditis, and brain abscess, etc. Therefore, they need early diagnosis, and it might necessitate surgical intervention according to the complications. We present the case of a 30-year-old male patient who came to the emergency department with a picture of acute right-sided mastoiditis. The patient complained of a one-day history of right ear pain, ear swelling,…

Skull Base Osteomyelitis: A Case Report and Literature Review.

Skull base osteomyelitis (SBO) is a rare but potentially life-threatening serious infection. It is most often caused by Pseudomonas aeruginosa and typically affects elderly people with diabetes or weakened immune systems. We present an 80-year-old man with diabetes who was diagnosed with SBO with left facial nerve involvement after initial misdiagnosis and inadequate response to treatment for otitis externa and Bell's palsy. Imaging and transmastoid biopsy confirmed the diagnosis. Although prolonged antibiotic therapy suppressed the infection, the left facial nerve palsy remained unresolved. T…

Skull Base Rhabdomyosarcoma Mimicking Osteomyelitis in a Pediatric Patient.

Rhabdomyosarcoma (RMS) is a rare malignant tumor, affecting 4.58 per 1 million children, with approximately 35% occurring in the head and neck. Skull base RMS commonly presents at advanced stages and delays diagnosis due to its overlapping features with other skull base pathology, and difficulty accessing the lesion for biopsy. This case illustrates these challenges in skull base RMS mimicking osteomyelitis of the petrous apex. Case: A 6-year-old immunocompetent female, with a history of two acute otitis media episodes, presented with a 3-week history of sixth cranial nerve palsy and sudden-on…
