Using texture analysis of head CT images to differentiate osteoporosis from normal bone density.

OBJECTIVES: To investigate the use of texture analysis for the detection of osteoporosis on noncontrast head CTs, and to explore optimal sampling regions within the craniofacial bones. METHODS: In this IRB-approved, retrospective study, the clivus, bilateral sphenoid triangles and mandibular condyles were manually segmented on each noncontrast head CT, and 41 textures features were extracted from 29 patients with normal bone density (NBD); and 29 patients with osteoporosis. Basic descriptive statistics including a false discovery rate correction were performed to evaluate for differences in t

Enhancing Approaches to Inverted Papilloma Through Computed Tomography-Based Hyperostosis Analysis.

OBJECTIVES: The postoperative recurrence rate of sinonasal inverted papilloma (IP) is high, and the residual tumor at the attachment site (the pedicle of the tumor) is considered the main cause of recurrence. Therefore, a surgical approach tailored to the tumor attachment is crucial. Localized hyperostosis is an imaging characteristic of the attachment observed on computed tomography (CT). This study aimed to determine the tendency of hyperostosis at the IP attachment location according to the detailed anatomical site and to improve the prediction accuracy of the preoperative attachment site.

Craniofacial fibrous dysplasia complicated by acute, reversible visual loss: report of two cases.

We report two cases of craniofacial fibrous dysplasia which presented with acute visual loss. The first patient had a sphenoid sinus mucocele compressing the optic chiasm. In the second patient the optic canal was narrowed by dysplastic bone. In both cases optic nerve decompression restored vision to normal.

The Occipital Recess of the Sphenoidal Sinus-A Novel Anatomical Possibility.

There are reported here multiple rare anatomical variants of the sphenoidal sinus found in a 53-year-old male. The sinus exhibited uncommon features, including a prominent occipital recess extending bilaterally into the basilar part of the occipital bone. A narrow posterior isthmus connected the main chamber to 2 recesses: a superior postsellar recess pneumatising the dorsum sellae, and a postero-inferior occipital recess. The latter extended into the right basilar occipital bone, closely associated with the right hypoglossal canal and coursing alongside the right internal carotid artery. In a

3D CBCT anatomy of the pterygopalatine fossa.

The anatomy of the pterygopalatine fossa keeps a traditional level and is viewed as constant, even though a series of structures neighboring the fossa are known to present individual variations. We aimed to evaluate on 3D volume renderizations the anatomical variables of the pterygopalatine fossa, as related to the variable pneumatization patterns of the bones surrounding the fossa. The study was performed retrospectively on cone beam computed tomography (CBCT) scans of 100 patients. The pterygopalatine fossa was divided into an upper (orbital) and a lower (pterygomaxillary) floor; the medial

Transclival cerebrospinal fluid fistula in a patient with Marfan's syndrome.

Marfan's syndrome is a disease associated with reduced structural integrity of connective tissues. We report a 36-year-old patient with Marfan's syndrome who presented with rhinorrhoea, occipital headache and vomiting. Physical examination revealed typical Marfan's syndrome features including dolicocephalous, mandibular micrognathia, tall stature, disproportionately long limbs and digits, and hypermobility of the joints. A high-resolution CT scan demonstrated pneumoencephalous, cerebrospinal fluid (CSF) filling the sphenoidal sinus, and a small bone defect of the clivus. Surgery performed thro

A tomographic study of the skull base in primary spontaneous cerebrospinal fluid leaks.

INTRODUCTION: This study aims to evaluate the existence of anatomic abnormalities in the skull base that could contribute to the origin of primary spontaneous cerebrospinal fluid leaks (PSL). METHODS: Twenty PSL patients were compared with 20 healthy individuals. The following features were measured through an analysis of computed tomography scans: the angles of the petrosal bones and skull base in both the sagittal and coronal planes; the anteroposterior and mediolateral diameters of the anterior skull base, sella, and sphenoid sinus; the depth of the olfactory fossa; the pneumatization of t

A surgical modification for performing orbitozygomatic osteotomies: technical note.

The addition of orbitozygomatic osteotomies to the fronto-temporo-sphenoidal craniotomy minimizes brain retraction required to reach deep seated pathology by allowing additional soft tissue dissection and strategic cranial bone removal. We report a modification of this technique in order to reduce soft tissue and cosmetic morbidity while increasing the efficiency with which this technique is performed. A two piece fronto-temporo-sphenoidal craniotomy combined with orbitozygomatic osteotomies was analyzed via cadaver dissection. The craniotomy and orbitozygomatic osteotomies were performed usin

[Basic Approach : Transsphenoidal Approach (Nasal and Sphenoid Phase)].

Endoscopic endonasal transsphenoidal surgery has become a cornerstone of minimally invasive skull base surgery, particularly for pituitary neuroendocrine tumors. Advances in endoscopic visualization, navigation systems, and surgical instruments have enabled safe and effective use of a direct transnasal corridor to the sella and adjacent skull base structures. However, successful execution of this approach requires a precise understanding of nasal and paranasal sinus anatomy, as well as meticulous surgical technique during the nasal and sphenoid phases of the procedure. This chapter describes t

Endoscopic approach to the upper cervical spine and clivus: an anatomical study of the upper limits of the transoral corridor.

BACKGROUND: Recent advances in endoscopic techniques have allowed minimally invasive approaches to the cranio-vertebral junction (CVJ) throu … to the sphenoid-occipital junction, where the basilar portion of the occipital bone joins with the sphenoid bone's body. This ranged from 12.7 to 18.9 mm above the line of the hard palate. This was achieved without having to transgress any of the palatine structures. CONCLUSIONS: The sphenoid-occipital junction represents the rostral limit of endoscopic trans …

[Sphenotemporal aneurysmal bone cyst. A new case and review of the literature].

Aneurysmal bone cysts (ABC) are benign tumors of the bones, formed by multiple cysts filled with blood and eroding the bone. They are rarely located at the skull. We report a case of a right temporal ABC in a 5-year-old girl. On admission, the physical examination showed a tough and fixed tumor, with no inflammatory signs nor vascular characteristics. The neurological examination was normal. Imaging studies showed an heterogeneous extracerebral mass, eroding the temporal bone and the greater wing of the sphenoid bone. The treatment consisted in an extensive tumor resection, associated with rec…

Severe Headache and Deterioration of Vision in Left Eye in a Chronic Hemodialysis Patient Revealing a Brown Tumor of Sphenoid Sinus.

BACKGROUND/OBJECTIVES: Brown tumors are rare bone lesions associated with hyperparathyroidism, particularly secondary hyperparathyroidism (SHPT), in chronic renal failure. While brown tumors commonly affect bones rich in marrow, the involvement of the sphenoid sinus is extremely rare and can present with neurological symptoms. This study reports a case of a sphenoid sinus brown tumor in a patient on hemodialysis, highlighting its clinical presentation and diagnostic challenges. METHODS: A 31-year-old woman undergoing chronic hemodialysis presented with a severe headache, diplopia, and progres

The extended maxillotomy and subtotal maxillectomy for excision of skull base tumors.

An approach to tumors of the middle compartment of the skull base is described with three case reports. It is accomplished by extending the subtotal maxillectomy or maxillotomy to include removal of a part of the malar bone, coronoid process of the mandible, nasal turbinates, ethmoid and sphenoid sinuses, posterior nasal septum, and pterygoid plates. Extension of the incision through the anterior tonsillar pillar and lateral pharyngeal wall into the retropharyngeal space will assist to expose the craniocervical region from the sphenoid roof to the fifth cervical vertebra and the skull base bet

[Sphenoid sinusitis and panhypopituitarism].

The case is reported of a patient who presented with panhypopituitarism simultaneously with sphenoid sinusitis due to Aspergillus fumigatus. Despite the absence of radiologically demonstrable suprasellar or intrasellar lesions, and without visible bone destruction, the authors conclude on the possibility of a pituitary lesion by contiguity. The pathogenic mechanisms are discussed.

Telangiectatic osteosarcoma of the skull base.

We present a patient with a distinctive lesion of the skull base, which, at imaging, showed expansile destruction of the sphenoid bone, hemorrhage with fluid-fluid levels, and contrast enhancement of a solid portion. These features ordinarily suggest either giant cell tumor or aneurysmal bone cyst; however, pathologic examination confirmed instead a rare variant of osteosarcoma of the telangiectatic type. Although imaging findings can indicate any of these possibilities, biopsy is an essential step in arriving at the diagnosis.
