Compressive Optic Neuropathy Caused by a Cholesterol Granuloma of the Sphenoidal Sinus.

Cholesterol granuloma is a foreign body giant cell reaction to cholesterol deposits. The pathology is rarely seen in the paranasal sinuses. In this report we describe a case of compressive optic neuropathy caused by cholesterol granuloma arising from the sphenoidal sinus. A 46-year-old lady presented with painless progressive decrease in vision in her left eye over the past 7 years. Magnetic resonance imaging of the brain and orbit with gadolinium enhancement showed a homogenous T1 and T2 hyperintense lesion causing expansion of the sphenoidal sinus compressing the ethmoidal sinus and also the

Invasive Aspergillus Sinusitis Masquerading as Hypertrophic Pachymeningitis Complicated by a Ruptured Infectious Internal Carotid Artery Aneurysm.

A 76-year-old man presented with headache, progressive left-sided visual loss, and cranial neuropathy. Initial MRI and cerebrospinal fluid findings mimicked those of hypertrophic pachymeningitis. Corticosteroid therapy provided only a transient improvement before clinical deterioration, including right-eye blindness. A re-evaluation revealed destructive sphenoid sinus disease, and surgical decompression confirmed invasive Aspergillus sinusitis. The course was complicated by a ruptured infectious internal carotid artery aneurysm, which required emergency endovascular embolization. Despite the c

Metastatic Colon Cancer to the Sphenoid Wing and Ethmoid Sinus Mimicking Orbital Cellulitis: A Rare Presentation.

Metastatic lesions to the orbit are most commonly seen with breast, lung, and prostate cancer, but are less commonly seen with colon cancer. Furthermore, the presence of metastatic colon cancer involving the sphenoid wing has only been reported once previously. The authors present a case of a 68-year-old woman with right upper and lower eyelid edema and erythema along with decreased vision, relative afferent pupillary defect, limitation of extraocular movements, and chemosis suggestive of orbital cellulitis. Imaging revealed an erosive lesion of the sphenoid wing along with unilateral ethmoid

Management of Capillary Hemangioma of the Sphenoid Sinus.

Background and objectives: Capillary hemangiomas are rare, benign vascular tumors that mainly affect the skin and soft tissue, with scarce appearance within the nasal cavities and paranasal sinuses. Materials and methods: We present a case report of capillary hemangioma of the sphenoid sinus and a review of the literature in the last ten years. Results: Clinical and endoscopic examination of the nose, radiologic assessment and particular histologic features contribute to the correct diagnosis of capillary hemangioma of the nose and paranasal sinuses. Conclusions: Transnasal endoscopic resectio

Pneumosinus dilatans multiplex, mental retardation, and facial deformity.

Pneumosinus dilatans is a term used to describe enlargement of one or more paranasal sinuses without radiological evidence of localized bone destruction, hyperostosis, or mucous-membrane thickening. To date, many cases have been reported that involved frontal, ethmoid, sphenoid, and maxillary sinus. However, no case has been reported that involved all paranasal sinuses. Our case involved mastoid air cells as well as all paranasal sinuses. It is named pneumosinus dilatans multiplex by us. This is the first case to be reported in English literature that has this syndromic condition of pneumosinu

Meningioma of the paranasal sinuses.

Meningiomas originating in the paranasal sinuses are rare. These tumors are thought to arise from embryonal arachnoid nests which were pinched off and left behind during embryonic development. We have described various radiographic findings of two patients with meningioma arising in the paranasal sinuses (frontal sinus origin in a 65-year-old female, and sphenoid sinus origin in a 26-year-old female). The paranasal origin of meningioma was accurately determined on the basis of CT and arteriography. A review of the 11 cases previously reported and our cases indicates that there are no specific

Hyperaeration of the frontal sinus.

The nasal cavity and paranasal sinuses constitute an anatomical and functional unit. Paranasal sinuses communicate with the nasal cavities, which are covered by the same mucosa, via small openings and narrow ducts that allow both aeration and sinus drainage. Anatomical variations of the paranasal sinuses and nasal cavity are important in sinus diseases. Computerized tomography (CT) plays a critical role in evaluation of the patients with paranasal sinus pathology and variations. A 38-year-old man admitted to our clinic with headache and postnasal dripping for 1 year. His paranasal CT scanning

Neighbouring relations of the posterior ethmoid studied by axial computed tomography.

The anatomy of the paranasal sinuses is variable and it is important to appreciate their clinical and surgical significance. During both posterior ethmoid and sphenoid sinus surgery, but also for endoscopic approach of pituitary, a good knowledge of the anatomy is essential for understanding the pathological process and to prevent surgical complications (optic nerve, internal carotid artery injury during surgery). Although coronal Computerized Tomography (CT) is the routine procedure performed for functional endoscopic sinus surgery (FESS), axial CT is most useful in demonstrating the anatomy

Arrested Pneumatization of the Sphenoid Sinus.

The sphenoid bone is a complex structure in terms of its embryological origin. At birth, the sphenoid sinus is nonpneumatized. Arrested pneumatization of the sphenoid sinus is considered a normal anatomic variant but may be mistaken for disease in imaging studies. We report 2 cases of arrested pneumatization of the sphenoid sinus, a normal variant commonly misdiagnosed as a serious disease of the skull base. A 29-year-old man with a complaint of dizziness visited a local clinic for assessment. Computed tomography (CT) of the paranasal sinuses (PNS) showed a noneroding, nonexpansile, and nonhom

A Novel International Endoscopic Sphenoid Surgery Classification (IESSC): A Delphi Consensus.

BACKGROUND: Advancements in endoscopic sinus and skull base surgery created a need for standardized terminology to describe sphenoid sinus surgery. Although classification systems exist for other sinuses, one for endoscopic sphenoid sinus surgery is lacking. Developing such a system would standardize procedure descriptions and promote a common language among surgeons. This study aimed to develop a new classification system for endoscopic sphenoid surgery. METHODS: Consensus on a novel endoscopic sphenoid surgery classification system by running the Delphi procedure with 16 rhinology experts f

Series of isolated sphenoid disease: Often neglected but perilous.

Isolated sphenoid sinus disease (ISSD) is an uncommon entity and may present with non-specific symptoms. As sphenoid sinus is located adjacent to vital structures, a diseased sphenoid sinus can potentially lead to devastating complications such as cranial neuropathies, cavernous sinus thrombosis and intracranial abscess. Herein, we report a case series of three patients who presented with different localization of headache as their main symptom. Endoscopic nasal assessment showed abnormal nasal cavity findings with mucosal thickening and soft tissue mass occupying the sphenoid sinus revealed b

Synthetic polyurethane foam for sphenoid packing after endonasal skull base surgery: propensity-matched trends in cerebrospinal fluid leakage and sphenoid mucosal changes.

BACKGROUND: Postoperative cerebrospinal fluid (CSF) leakage and sphenoid sinusitis remain major concerns after endoscopic endonasal approaches (EEAs) for midline skull base tumors. To our knowledge, this study is among the first to directly compare synthetic polyurethane foam (SPF) with balloon catheters (BCs) for sphenoid sinus packing, assessing efficacy and safety, including in high‑flow CSF leaks. METHODS: This retrospective, single-center, observational study included 116 patients who underwent EEAs between April 2016 and March 2025. Since May 2021, SPF has replaced BCs for sphenoid sinus

Conventional sinus radiography compared with CT in the diagnosis of acute sinusitis.

OBJECTIVES: To evaluate the value of plain film radiography in a prospective investigation of patients with clinical suspicion of acute sinusitis, using standard CT as a gold standard. METHODS: 47 consecutive patients were examined. Each patient underwent conventional X-ray and standard dose CT examinations on the same day. The sensitivity and specificity of the plain film examination were calculated. RESULTS: The specificity of the plain film examination was high, but the sensitivity was low except for the maxillary sinus (sensitivity 80%). Thus, for maxillary sinusitis, plain film examinat

Distribution of CGRP and TRPV2 in Human Paranasal Sinuses.

Immunohistochemistry for protein gene product 9.5 (PGP 9.5), calcitonin gene-related peptide (CGRP) and the transient receptor potential cation channel subfamily V member 2 (TRPV2) was performed on human paranasal sinuses. It was found that in the paranasal sinuses, mucous membranes contain PGP 9.5-immunoreactive (PGP 9.5-IR) nerve fibers. Such nerve fibers terminated around large blood vessels as fine varicosities. Isolated PGP 9.5-IR nerve fibers were scattered beneath the epithelium. Glandular tissues were also innervated by PGP 9.5-IR nerve fibers. These fibers were numerous in the maxilla
