Management of lower limb fractures in patients with spinal cord injuries.

BACKGROUND: Low-energy fractures of the lower limb in patients post spinal cord injury (SCI) present significant management challenges. This is the first Australian study to review the experience from a tertiary referral SCI centre and aims to identify trends and suggest therapeutic options. METHODS: Retrospective review over a 5-year period in patients with SCI treated for a lower limb fragility fracture. Patient demographics, spinal injury severity scores, fracture characteristics and treatment were assessed. Time to union, length of stay and treatment-related complications were also examin

Traumatic spinal cord injuries in geriatric population: etiology, management, and complications.

BACKGROUND: The relationship of traumatic spinal cord injury (SCI) and the geriatric population is not emphasized in current literature. Our objective was to evaluate mechanisms of injury, outcomes, and complications of geriatric patients with traumatic SCI. METHODS: Patients with traumatic spinal cord injuries admitted to the inpatient rehabilitation unit of a level I trauma center from 2003 to 2013 were reviewed. Inclusion criteria were ages ≥65 years old and availability of complete medical records. Patient demographics, mechanism of injury, diagnoses, American Spinal Injury Association (A…

Outcome of subtrochanteric femur resection in patients with spinal cord injuries.

OBJECTIVE: To analyse the clinical outcome of subtrochanteric femur resection in patients with infected hip joints caused by pressure ulcers (PUs) in patients following spinal cord injury (SCI). METHOD: A retrospective cohort study, carried out between January 2004 and December 2015. Only patients receiving a subtrochanteric femur resection were included. SCI patients were treated for a hip joint infection caused by a PU. Primary outcome measures were to work out the revision rates and the rate of heterotopic ossification occurrence. RESULTS: At the time of admission, 37 out of the 56 partic

Improving Sexual Satisfaction in Persons with Spinal Cord Injuries: Collective Wisdom.

Sexuality is an important part of life, and it is necessary for clinicians to have a specific format in which to address sexual issues with their patients. A systematic approach to working with patients with spinal cord injury (SCI) to improve their sexual functioning and response is presented. Nonjudgmental communication about sexual concerns is followed by a detailed pre- and postinjury medical, psychosocial, and sexual history. If preexisting sexual issues are present, it is recommended that the patient be referred for assessment and treatment of these separate from the patient's SCI-relate

Traumatic spinal cord injuries in horseback riding: a 35-year review.

BACKGROUND: Spinal cord injury (SCI) is a potentially disabling neurologic injury that can occur in horseback riding. To date, no published study has examined the epidemiology of SCI from horseback riding in the United States, and few international studies exist on this topic. Several studies have described traumatic brain injuries, spine fractures, and extremity injuries; however, SCI patterns and outcomes in horseback riders are poorly understood. PURPOSE: This study was undertaken to characterize the demographics, SCI patterns, and neurologic outcomes of persons with SCIs related to horseb

Functional outcome after incomplete spinal cord injuries due to blunt injury.

During a 3-year period, 19 patients with incomplete spinal cord injuries caused by blunt trauma were admitted to a single rural referral centre. The mean age was 50 years. Injury mechanisms included falls in eight, road traffic accidents in five, diving mishaps in two, and miscellaneous in four. The level of spinal cord injury was cervical in 11, thoracic in five, and thoracolumbar in three. Initial management included a standard high-dose methylprednisolone protocol for 24 h after injury in eight patients treated since May, 1990. Neurosurgical procedures were performed in 11 patients. There w…

Bull riding-related brain and spinal cord injuries--Louisiana, 1994-1995.

Rodeos are popular sporting events in the southern and western United States, and bull riders sustain 37% of all rodeo-related injuries--more than participants in any other rodeo event. During 1994-1995 in Louisiana, five cases of central nervous system trauma associated with riding bulls in rodeo events were identified through the Louisiana Central Nervous System Injury Registry, a statewide, population-based surveillance system addressing brain and spinal cord injury incidence, etiology, and outcome. To further characterize these injury events, the Office of Public Health, Louisiana Departme

Silent autonomic dysreflexia during voiding in men with spinal cord injuries.

PURPOSE: We determined whether symptoms of autonomic dysreflexia correlated with elevations in blood pressure in men with spinal cord injuries. MATERIALS AND METHODS: During a routine yearly urodynamic evaluation 45 consecutive men with complete spinal cord injuries above T6 underwent simultaneous monitoring of blood pressure and symptoms of autonomic dysreflexia. Those with systolic blood pressure of greater than 160 mm. Hg or diastolic blood pressure of greater than 90 mm. Hg during voiding were assigned to the hypertensive group. During voiding 35 men (78%) had significant hypertension. R…

Residual urine following intermittent catheterisation in patients with spinal cord injuries.

Residual urine following catheterisation was studied using ultrasonography (Ultramark II Scientific Medical System) in 12 patients with spinal cord injury, for an average of 4 months (range 3-7 months) after the time of injury. All of the patients were examined on three occasions. Urinary tract infections were also registered, and the functional type of the urinary bladder was identified by a cystometric examination. Ultrasonography showed residual urine volumes after 25 of a total of 36 catheterisations (70%). In seven of these catheterisations the residual urine volume exceeded 50 ml, and in

Urological long-term follow-up in women with spinal cord injuries.

One hundred eight women and 434 men admitted with spinal cord injuries between 1973 and 1987 were observed prospectively to determine the effects of age, sex, neurological classification, and method of bladder management method on renal function and urological complications. The primary method of bladder management for women throughout the study period was indwelling urethral catheterization as compared to condom drainage for their male counterparts. Renal function was determined by effective renal plasma flow and urological complications were determined by clinical symptoms and/or objective f…

Management of athletic injuries of the cervical spine and spinal cord.

Injuries to the cervical spine among athletes present inherent difficulties, especially in advising for return to contact sports. Experience with the acute care of 63 patients who sustained cervical spine injuries while participating in organized sporting events is analyzed. Forty-five patients had permanent injury to the vertebral column and/or spinal cord, while 18 suffered only transient spinal cord symptoms. Football mishaps accounted for the highest number of injuries, followed by wrestling and gymnastics. Twelve patients had complete spinal cord injury, 14 patients had incomplete spinal

Evaluation of cervical spinal cord injuries with metrizamide myelography-CT scanning.

In the past, patients with injuries of the cervical spine and spinal cord have been diagnosed by means of myelography and polytomography. In an attempt to improve the radiographic evaluation of patients with cervical spinal cord injuries the authors performed computerized tomography (CT) scanning of the cervical spine following injection of metrizamide into the spinal subarachnoid space. In 23 patients with cervical spinal cord injuries, metrizamide myelography was performed via a C1-2 puncture. Myelography was used only for localization of the lesion and to determine the site of CT scanning. …

[State of the vertebral canal in vertebral and spinal cord injuries].

The results of the analysis of 128 patients who had died after trauma of the spine and spinal cord are discussed. Total disturbance of the anatomical intactness of the vertebral canal walls was demonstrated on all specimens. In 85% of patients there were fractures with or without displacement of the injured vertebra; 15% of patients had suffered injury only to the ligamentous-capsular apparatus, the disk included, with no displacement--dislocation of the vertebra. The shape of the vertebral canal was distorted only in cases with displacement of the vertebra. The area of the canal was reduced t

A model for the future care of acute spinal cord injuries.

This is a review of the total care of those acute spinal cord injury patients in Ontario during the years 1969 and 1970, from extrication and transportation following the accident to death, or the completion of primary definitive rehabilitation. Information was extracted from the available ambulance records, the patients and many of the responsible physicians were interviewed personally. The study was detailed and intensive and included a review of each patient's hospital records in each hospital up to discharge from the rehabilitation programme into the community, or to a chronic care unit.
