Chronic myelomonocytic leukemia (CMML) is an uncommon disorder characterized by monocytosis of the peripheral blood, absence of the Philadelphia chromosome, fewer than 20% blasts, and one or more lineages showing dysplastic features. Splenomegaly is frequently seen and may be massive. A 56-year-old man with stable CMML and moderate splenomegaly presented to the emergency department with generalized abdominal pain and abrupt drop in hemoglobin. Abdominal imaging revealed splenic rupture and emergency splenectomy was undertaken, with complete recovery. Atraumatic rupture of the spleen has rarely…

Rupture of a splenic artery aneurysm remains an uncommon cause of hypovolaemic shock in the emergency department. This case report highlights that rapid resuscitation, diagnostic imaging, surgical consultation, and subsequent laparotomy remain the priorities in patient management.

Spontaneous splenic rupture is an uncommon but life-threatening complication of hematologic malignancies, despite the frequent involvement of the spleen in these diseases. It has been reported in patients with acute and chronic leukemia, Hodgkin's disease, non-Hodgkin's lymphoma, and histiocytic lymphoma. A 50-year-old previously healthy man presented with acute symptoms of spontaneous splenic rupture. Emergency splenectomy with liver biopsy was performed. The clinicopathologic features suggested a hepatosplenic gammadelta T-cell lymphoma (HSgammadeltaTL). Here, we report a rare case of sponta…

Infectious mononucleosis is an acute, viral, illness associated with a high incidence of splenomegaly. Spontaneous splenic rupture is a rare but life-threatening complication of infectious mononucleosis. The authors report the case of a 19-year-old patient with an infectious mononucleosis causing a spontaneous splenic rupture. When rupture occurs the mortality has been significant. The spleen may be vulnerable for the histopathologic changes that occur as a result of this illness. Two thirds of patients with infectious mononucleosis develop an enlarged spleen, but in only 0.5% of all patients …

Acute hematocele is commonly associated with direct testicular trauma. Blood within the tunica vaginalis may infrequently accompany blunt abdominal injury in the presence of a communicating hydrocele. Optimal management involves early recognition and treatment of the abdominal source of bleeding. Elective repair of the communicating hydrocele/hematocele should follow. We report 2 cases of boys with scrotal swelling due to communicating hematoceles. Both cases were associated with a patent processus vaginalis and splenic laceration secondary to blunt trauma.

We report a rare case of acute abdomen caused by the spontaneous rupture of a splenic hydatid cyst into the abdominal cavity, causing a massive hemoperitoneum due to accompanying rupture of the spleen which required splenectomy. A review is made of the literature on this rare entity, its treatment and its consequences and their prevention and treatment.

Spontaneous rupture of the splenic vein secondary to hepatic cirrhosis is a rare condition with a high mortality rate. Two case histories are presented with a review of the two other cases in the literature. Survival following rupture depends upon early recognition and immediate operation.

The authors present the history of a seven-year-old male child suffering from delayed splenic rupture and fat embolism syndrome simultaneously. The conservative treatment of splenic ruptures as well as incidence and early diagnosis of fat embolism syndrome in childhood will be discussed.

Splenic lymphangiomas are benign neoplasms resulting from congenital malformations of lymphatic channels manifesting as cystic lesions, occurring mostly in childhood. This process usually involves additional sites in a diffuse or multifocal fashion but although rare, can also present isolated to the spleen. The clinical picture varies from asymptomatic identified incidentally to nonspecific symptoms from compression of adjacent organs. Spontaneous rupture of these lesions can lead to hemoperitoneum, acute abdomen and hemorrhagic shock. We present the case of a 33-year-old male who required urg

This study reports a case of spontaneous subcapsular splenic ruptured hematoma (with hemoperitoneum) and provides update on this rare disease. Non-traumatic ruptures can be fatal. Diagnosis is sometimes difficult. They often reveal an underlying pathology including infections, tumors or hematologic disorders. Symptomatology commonly has an abrupt onset but progressive forms are also possible. The majority of patients undergo splenectomy.

BACKGROUND: Rupture of a splenic artery aneurysm during pregnancy is a rare and serious event, occurring mainly during the third trimester. The risk of rupture for an existing splenic artery aneurysm is very high during pregnancy (from 20% to 50%). When it does rupture, the maternal mortality rate is around 75% and the fetal mortality rate 95%. Of 110 cases reported in the literature, only one ruptured during the first trimester.
OBJECTIVES: The aim of this case report is to make emergency physicians aware of this diagnosis, because only if it is considered can it be managed rapidly and appro

BACKGROUND: Epidermoid splenic cysts are uncommon lesions of the spleen. They are known to become symptomatic as a consequence of enlargement, infection, or rupture, the latter being an exceedingly rare complication traditionally treated with open splenectomy. We herein report a unique case of a giant epidermoid splenic cyst that ruptured spontaneously and was successfully treated with the laparoscopic approach.
CONCLUSION: Laparoscopic surgery may be considered an initial treatment option in cases of very large epidermoid cysts even when rupture occurs.

HISTORY AND FINDINGS: A pale looking 33-year-old man with a history of perforated gastric ulcer and pancreatitis with surgically drained pseudocyst was admitted because of painless anal bleeding. Heart rate was 100/min, blood pressure 90/60 mm Hg.
INVESTIGATIONS: Haemoglobin concentration (6.3 g/dl) and RBC count (2.4 mill./microliters) indicated anaemia due to bleeding. Sonography and computed tomography demonstrated chronic calcifying pancreatitis and thrombosis of splenic and mesenteric veins. There were grade 1 oesophageal varices on endoscopy. The source of bleeding was found by coloscop

A male, 52 years old, presenting acute abdominal pain and hypovolemic shock, was diagnosed by ultrasound and CAT of fissured aneurysm of the splenic artery. Emergency laparotomy confirmed that the aneurysm had ruptured into the peritoneal cavity.
